New multi-cannula pedestal device for micro-injection of drugs into brain tissue or cerebral ventricle.

A new multiple-cannula-pedestal system for micro-injection of drugs directly into either brain tissue or cerebral ventricle is described. Its features include ease of construction from commonly available materials, no specialized machining required, durability and economy. A special aspect of the cannula system is a protective cap containing a bolt which threads onto a nut fixed within the pedestal base. Since the cap cannot be dislodged, potential damage to indwelling stylets and exposed guide tubes is prevented. Moreover, an aseptic preparation is therefore provided so that test compounds ca…

Combined NRB and Nasal Cannula Oxygen Delivery Is Inferior to Flush Rate NRB for Emergency Preoxygenation.

OBJECTIVES: Although preoxygenation with a non-rebreather (NRB) mask at flush rate (≥ 50 L/min) is an effective and straightforward technique, it is not always feasible in situations where airway management must be performed away from the ambulance, when only portable oxygen tanks are available. To achieve a combined total flow of ≥50 L/min, some experts have advocated the use of a NRB mask in combination with a nasal cannula, both at 25 L/min. This technique has not been compared to preoxygenation using flush rate oxygen. This study sought to determine whether preoxygenation using both a NRB …

Early Use Experience of the Crescent Right Atrial Cannula for Neonatal Veno-Venous Extracorporeal Membrane Oxygenation.

Veno-venous (VV) extracorporeal membrane oxygenation (ECMO) is associated with fewer neurological complications and decreased mortality compared to veno-arterial (VA) ECMO in neonatal respiratory failure. The Crescent right atrial (RA) cannula is the only dual-lumen cannula for neonatal VV ECMO designed to have the tip in the right atrium. The purpose of this study is to describe the experience with early use of the Crescent RA cannula. We performed a retrospective cohort study of 58 neonates and infants cannulated from September 2021 through August 2023 at 15 institutions represented within t…

[How to Insert the Cannula for Central or Peripheral Venous-arterial Extracorporeal Membrane Oxygenation(VA-ECMO)?].

Gold standard cannulation for peripheral veno-arterial extracorporeal membrane oxygenation (VA-ECMO) is from the femoral artery and vein by using ultrasound guided puncture technique percutaneously. The Seldinger technique by using the guide wire is the major approach to place the cannula. Direct cut-down exposure technique is another option if the percutaneous approach is difficult. The size selection of the cannula is also important to get sufficient flow for peripheral VA-ECMO. If the lower limb had ischemia, the extra distal perfusion should be added. Central ECMO by perfusing the ascendin…

Effect of High-Flow Nasal Cannula Flow on Intrapharyngeal Pressure During Fiberoptic Bronchoscopy Under Deep Sedation.

BACKGROUND: The flow reaching the vocal folds may be lower than that at the output of high-flow nasal cannula (HFNC) system. This could be due to upper-respiratory obstruction, oxygen leakage, or other factors. The objective of this study was to observe the effect of flow through a nasopharyngeal airway on intrapharyngeal pressure (IPP) in subjects undergoing fiberoptic bronchoscopy (FOB). METHODS: Patients scheduled for FOB were invited to participate. Measurements were performed at flows of 0-60 L/min; the subjects wore WN-N95 folding medical protective masks (N95) and either underwent FOB …

Comparison of Robot-Assisted and Manual Cannula Insertion in Simulated Big-Bubble Deep Anterior Lamellar Keratoplasty.

This study aimed to compare the efficacy of robot-assisted and manual cannula insertion in simulated big-bubble deep anterior lamellar keratoplasty (DALK). Novice surgeons with no prior experience in performing DALK were trained to perform the procedure using manual or robot-assisted techniques. The results showed that both methods could generate an airtight tunnel in the porcine cornea, and result in successful generation of a deep stromal demarcation plane representing sufficient depth reached for big-bubble generation in most cases. However, the combination of intraoperative OCT and robotic…

Clinical Application of 24 G Cannula Needle and 3-0 Polypropylene Suture in Vas Deferens Exploration.

The goal of this article is to present the application of a 24 G cannula and 3-0 polypropylene suture as a simple method for the exploration of the vas deferens. During the exploration of the vas deferens, a 24 G cannula needle was used to puncture it. The fluid in the smear confirmed the presence of sperm, to determine whether or not there was concomitant obstruction at the junction of the epididymis and the vas deferens. Then, a 3-0 polypropylene suture (this suture specification has the advantages of a smooth surface, robust quality, and can be passed through a 24 G cannula needle) was pass…

Monitoring Changes in Oxygen Muscle during Exercise with High-Flow Nasal Cannula Using Wearable NIRS Biosensors.

Exercise increases the cost of breathing (COB) due to increased lung ventilation (V˙E), inducing respiratory muscles deoxygenation (∇SmO2), while the increase in workload implies ∇SmO2 in locomotor muscles. This phenomenon has been proposed as a leading cause of exercise intolerance, especially in clinical contexts. The use of high-flow nasal cannula (HFNC) during exercise routines in rehabilitation programs has gained significant interest because it is proposed as a therapeutic intervention for reducing symptoms associated with exercise intolerance, such as fatigue and dyspnea, assuming that …
