Massive orofacial hemorrhage mostly occurs in younger individuals unilaterally and is associated with extensive LeFort fractures. Here we describe a 91-year-old man who sustained a life-threatening hemorrhage of the bilateral internal maxillary artery (IMA) system with a comminuted LeFort I fracture caused by a road traffic accident while he was driving. His injury severity score was high (34), but no severe brain injury was sustained. Transarterial embolization (TAE) was performed to control the intractable hemorrhage. The patient survived and has an acceptable outcome. Although rare, this ca

An 11-year-old girl with type IIIA open fracture of the femoral shaft and 4.5 cm bone loss, was treated by global reconstruction using a reamed, interlocking, intramedullary titanium nail, following meticulous primary debridement with pulsed lavage irrigation. The nail entry was carefully chosen at the lateral transtrochanteric point in order to avoid any vascular damage to the head of femur. The osteophilic nature of the titanium nail, in addition to the thick periosteum of the paediatric bone, helped satisfactory union despite a hostile environment. The child had 0-90 degrees flexion of the

BACKGROUND Intertrochanteric fractures can be managed using insertion of a proximal femoral nail anti-rotation (PFNA) intramedullary nail inserted into the femur. The helical blade used with the PFNA technique has the advantage of less bone removal and good biomechanical properties, but significant hidden blood loss (HBL) can occur. This study compared postoperative HBL in 219 elderly patients with intertrochanteric fractures using PFNA fixation, stratified by greater trochanter comminution status (comminuted vs non-comminuted). MATERIAL AND METHODS We retrospectively analyzed data on 219 elde

Fixation of a comminuted mid-distal femur fracture is challenging in clinical practice due to a high degree of comminution and malformation. Here we describe a procedure involving a series of steps for accurate fracture fixation in a patient diagnosed with comminuted mid-distal femur fracture. Emergency traction of the tibia tubercle was initially performed using a large distractor for temporary external fixation to maintain the femoral force line and to reduce intraoperative fractures. A three-dimensional printed model was then used as a preoperative simulation tool to preshape the customized

BACKGROUND: Fibular allograft (FA) augmentation is proposed to address the lack of medial column support in plate-fixed proximal humeral fractures; however, the role of FA augmentation in these fractures remains inadequately elucidated. Our previous clinical trial did not show additional benefit in inserting an FA when treating adults with medial column comminuted proximal humeral fractures. Nonetheless, it is possible that some subgroups may still derive benefits from FA. This study aimed to investigate whether there were any subgroups benefit from FA.
MATERIALS AND METHODS: Our randomized c

Background/Objectives: Various surgical methods have been proposed for the treatment of comminuted Mason III/IV radial head fractures. In particular, the advantages and disadvantages between prosthesis implantation (RHA) or radial head resection (RHR) are not sufficiently quantified in the current literature. Methods: A systematic literature search was conducted using PubMed Web of Science, Cochrane Library, and Embase in February 2024. Studies conducted on patients with Mason type III or IV radial head fractures and studies relating to surgical methods, including radial head resection or Radi

BACKGROUND: Minimally invasive plate osteosynthesis (MIPO) is one of the generally accepted surgical techniques for the treatment of humeral shaft fractures. However, despite the high bone union rate, a variety of complications are still prevailing. Moreover, the current literature lacks data comparing the anterolateral MIPO approach using dynamic compression plates accommodating different numbers of screws. The aim of this study was to analyze the biomechanical performance of comminuted humeral shaft fractures fixed with dynamic compression plates using either two or three screws per fragment

PURPOSE: Comminuted intra-articular fractures and fracture dislocations of the metacarpophalangeal (MCP) and interphalangeal joints are challenging. Dynamic external fixation, permitting early joint motion while still minimizing forces across the healing joint, can result in acceptable postoperative active range of motion (AROM). However, some fractures are not initially stable enough for early dynamic motion; further, many available dynamic external fixation systems are costly and cumbersome. We reviewed our experience using an external fixator made from a 1-mL syringe and K-wires and report

OBJECTIVES: The incidence of osteosynthesis is increased by volar anatomic plate used for treatment of distal radius fractures and this increases the incidence of wrist extensor tendon irritation, a postoperative complication of the aforementioned surgical technique. The purpose of this study; was to evaluate the intraoperative skyline view which is commonly used to prevent dorsal cortex penetration of distal screws during the surgical treatment of intra-articular distal radius fractures with comminuted dorsal cortex with CT (computed tomography) scanning to determine its effectiveness. Background: Posterior sagging (PS) of the proximal fragment after cephalomedullary nailing (CMN) of intertrochanteric fractures leads to loss of anteromedial cortical support. Methods: We retrospectively reviewed 118 patients with AO/OTA 31-A2 intertrochanteric fractures (2007 classification). After an institutional protocol change, standard-length constructs with one distal screw were used until March 2019 (n = 65) and long constructs (≥300 mm) with two distal screws from June 2019 (n = 53); the cohorts were consecutive, not concurrent. Results: PS occurred in 23/118 patients (19.5%): 21/65 (
