Agenesis of the left internal carotid artery associated with right anterior cerebral artery A1 segment bifurcation aneurysm: a case report.

Agenesis of the internal carotid artery occurs in less than 0.01% of the population. Intracranial aneurysms accompany agenesis in 25-43% of the patients. We present a unique patient with a saccular aneurysm of the anterior communicating artery associated with agenesis of contralateral internal carotid artery.

[Chronic mesenteric ischemia with consecutive ischemic colitis. Suggestions for diagnosis and therapy].

The incidence of acute mesenteric ischaemia has decreased over the last few years. However, cases of chronic mesenteric ischaemia have grown in number, as this disease is the most frequent disorder of the large intestine in the elderly. The typical clinical presentation of ischaemic colitis develops gradually and only becomes recognisable in the late stage of the disease. We present a 51-year-old woman with a history of unexplained abdominal pain. Multislice CT demonstrated an irregular stenosis of the truncus coeliacus and superior mesenteric artery. A Riolan's anastomosis was present and sho

Vascular involvement in diabetic subjects with ischemic foot ulcer: a new morphologic categorization of disease severity.

OBJECTIVES: Arteriographic lesions of diabetic subjects with critical limb ischemia (CLI) and ischemic foot ulcer were reviewed retrospectively, to provide new criteria for stratification of these patients on the basis of their vascular involvement. PATIENTS: In 417 consecutive CLI diabetic subjects with ischemic foot ulcer undergoing lower limb angiography, lesions were defined as stenosis or occlusion, localization, and length (<5 cm, 5-10 cm, >10 cm). In a subgroup of 389 subjects, foot arteries also were evaluated. Patients then were categorized into 7 classes of progressive vascular invo

Cerebral oxygen metabolism and neuronal integrity in patients with impaired vasoreactivity attributable to occlusive carotid artery disease.

BACKGROUND AND PURPOSE: It is still unclear that impaired cerebrovascular reactivity (CVR) to acetazolamide is comparable to elevated oxygen extraction fraction (OEF) on positron emission tomography (PET) in patients with occlusive carotid diseases. Therefore, in this study, the authors aimed to clarify whether OEF is elevated in all patients with reduced cerebral blood flow (CBF) and CVR (type 3) on single photon emission computed tomography (SPECT), and, if not, to specify the underlying pathophysiology of type 3 but normal OEF. METHODS: This study included 46 patients who had decreased CBF

Accuracy of duplex sonography before carotid endarterectomy--a comparison with angiography.

OBJECTIVES: The aim of this prospective study was to contribute to the evaluation of the reliability of Duplex sonography (DS) before carotid endarterectomy (CEA). DESIGN: The study was performed prospectively in a university hospital setting. METHODS: Eighty-one consecutive patients aged 49-83 years were examined with DS and carotid angiography (CAG) before CEA. The results of the DS were judged as either confident, or CAG was assessed to be necessary preoperatively. The results from the DS and the CAG were then compared. RESULTS: DS was judged as confident in 148 of the 162 arteries exami

Intra-arterial streptokinase in acute ischaemic stroke. A pilot study.

AIMS: To assess the safety and efficacy of the intra-arterial administration of streptokinase within 24 hours of acute ischaemic stroke. METHODS: Patients who presented to the Austin Hospital casualty department between 3 and 22 hours after an acute stroke were considered for the study. Eligible patients had pretreatment non-contrast computed tomographic scans of the brain to exclude haemorrhage. Streptokinase (250,000 units) was administered directly into the common carotid artery or the cervical portion of the internal carotid artery. MAIN OUTCOME MEASURES: The incidence of symptomatic and

Complementary use of computed tomography and magnetic resonance imaging in assessing skull base lesions.

Twenty-six patients underwent computed tomography (CT) and magnetic resonance imaging (MRI) of skull base lesions at the Cleveland Clinic Foundation. CT provided improved bone detail, documenting invasion of the lamina papyracea, orbital floor, fovea ethmoidalis, cribriform plate, pterygoid plates, hard palate, and skull base. MRI defined invasion of the orbit, dura, brain, and cavernous sinus. Improved soft-tissue-tumor interface was evident on MRI. MRI was superior to CT in determining carotid artery involvement. MRI distinguished between tumor and retained secretions in the paranasal sinuse

3-dimensional digital reconstruction of the murine coronary system for the evaluation of chronic allograft vasculopathy.

BACKGROUND: Chronic allograft vasculopathy (CAV) is a major mechanism of graft failure of transplanted organs in humans. Morphometric analysis of coronary arteries enables the quantitation of CAV in mouse models of heart transplantation. However, conventional histological procedures using single 2-dimensional sections limit the accuracy of CAV quantification. The aim of this study is to improve the accuracy of CAV quantification by reconstructing the murine coronary system in 3-dimensions (3D) and using virtual reconstruction and volumetric analysis to precisely assess neointimal thickness. Utility of CT angiography in the identification and characterization of supraclinoid internal carotid artery blister aneurysms.

BACKGROUND AND PURPOSE: Blister aneurysms of the supraclinoid ICA represent a rare but potentially catastrophic cause of SAH, often presenting both diagnostic and therapeutic dilemmas. We explore the utility of CTA in the identification and characterization of ICA blister aneurysms. MATERIALS AND METHODS: We performed a retrospective review of catheter cerebral angiograms obtained at our institution over a 12-month period for evaluation of SAH, identifying 6 cases of ICA blister aneurysms. All patients underwent CTA and DSA for evaluation of SAH. The reports from the CTA and DSA studies were

Gain-of-function STAT1 mutations are associated with intracranial aneurysms.

Chronic mucocutaneous candidiasis, characterized by persistent or recurrent fungal infections, represents the clinical hallmark in gain-of-function (GOF) signal transducer and activator of transcription 1 (STAT1) mutation carriers. Several cases of intracranial aneurysms have been reported in patients with GOF STAT1 mutation but the paucity of reported cases likely suggested this association still as serendipity. In order to endorse this association, we link the development of intracranial aneurysms with STAT1 GOF mutation by presenting the two different cases of a patient and her mother, and

Endovascular management of vascular complications in pancreatic transplants.

Vascular complications after pancreatic transplantation carry a high rate of graft loss. Endovascular management of these complications is confined to stent placement for iliac artery inflow disease and embolization for arteriovenous fistulae (AVFs), pseudoaneurysms, or active bleeding. The current study describes the endovascular management of pancreatic transplant venous thrombosis (N = 1), arterial stenosis (N = 5), thrombosis (N = 3), pseudoaneurysms (N = 1), and AVF (N = 2). In addition, embolization of nonfunctioning grafts is described as an endovascular alternative to pancreatectomy.

[Usefulness of basic tests in patients with pulmonary embolism].

There are common, specific noninvasive and invasive tests for the diagnosis of pulmonary embolism (PE). Arterial blood gases, lung radiography and electrocardiography belong to the first group and should be performed in patients with suspected PE. Among the second group, D-dimer determination is essential. Ventilation-perfusion scintigraphy was formerly the most frequently used test but has been relegated to a second-line position by the development of multislice computed tomography (CT) angiography, which provides the possibility of simultaneously studying the lower limbs to detect deep venou

The Resistance Index Used to Evaluate Plantar Arch Anatomy in Patients with Chronic Limb-Threatening Ischemia.

BACKGROUND: Evaluation of the anatomy of the plantar arch (PA) is critical to the lower limb (LL) arterial revascularization. PA visualization in severely ill patients is often inadequate, even with digital subtraction angiography (DSA). Considering this, it is important to know if the resistance index (RI) obtained by Doppler ultrasonography (DU) is useful to evaluate the PA anatomy. OBJECTIVE: To evaluate whether the RI of the distal-leg arteries correlates with the angiographic anatomy of the PA in patients with LL peripheral arterial disease. METHODS: The study population consisted of 12

Jugular bulb and skull base pathologies: proposal for a novel classification system for jugular bulb positions and microsurgical implications.

OBJECTIVE There is no definitive or consensus classification system for the jugular bulb position that can be uniformly communicated between a radiologist, neurootologist, and neurosurgeon. A high-riding jugular bulb (HRJB) has been variably defined as a jugular bulb that rises to or above the level of the basal turn of the cochlea, within 2 mm of the internal auditory canal (IAC), or to the level of the superior tympanic annulus. Overall, there is a seeming lack of consensus, especially when MRI and/or CT are used for jugular bulb evaluation without a dedicated imaging study of the venous ana

Velocimetry based on multiphase CCTA images for reliable coronary flow velocity quantification across stenosis severities and ischemia detection.

BACKGROUND: Coronary flow velocity (CFV) serves as a critical functional feature for evaluating coronary stenosis severity (CSS) and detecting ischemia. With its noninvasive nature and rapid imaging, coronary CT angiography (CCTA) is expected to replace alternatives-computational fluid dynamics (CFD), magnetic resonance imaging (MRI), and digital subtraction angiography (DSA)-for hemodynamic evaluation and CFV quantification. Nevertheless, transluminal attenuation gradient-based velocimetry has been controversial, and its lack of dynamic flow information leads to appreciable quantified deviati
