Acoustics of clear speech: effect of instruction.

PURPOSE: This study investigated how different instructions for eliciting clear speech affected selected acoustic measures of speech. METHOD: Twelve speakers were audio-recorded reading 18 different sentences from the Assessment of Intelligibility of Dysarthric Speech ( Yorkston & Beukelman, 1984). Sentences were produced in habitual, clear, hearing impaired, and overenunciate conditions. A variety of acoustic measures were obtained. RESULTS: Relative to habitual, the clear, hearing impaired, and overenunciate conditions were associated with different magnitudes of acoustic change for measur

Stapedius tendon reconstruction during stapedotomy: technique and results.

During surgery for otosclerosis, it is common for the surgeon to cut the stapedius tendon. Even without reconstruction of the tendon, the results of this kind of surgery are particularly satisfactory. The stapedial reflex is more important for improving intelligibility of speech in the presence of background noise than for protection against hazardous levels of noise. An intact stapedial reflex improves ones ability to follow speech in the presence of background noise. This report will present a technique to reconstruct the stapedius tendon, along with the results obtained when adding this pro

Simultaneous natural speech and AAC interventions for children with childhood apraxia of speech: lessons from a speech-language pathologist focus group.

In childhood apraxia of speech (CAS), children exhibit varying levels of speech intelligibility depending on the nature of errors in articulation and prosody. Augmentative and alternative communication (AAC) strategies are beneficial, and commonly adopted with children with CAS. This study focused on the decision-making process and strategies adopted by speech-language pathologists (SLPs) when simultaneously implementing interventions that focused on natural speech and AAC. Eight SLPs, with significant clinical experience in CAS and AAC interventions, participated in an online focus group. The…

Predicting the speech reception threshold of cochlear implant listeners using an envelope-correlation based measure.

A modulation-based index is proposed for predicting speech intelligibility by cochlear implant (CI) listeners. The input to the proposed index are speech envelopes extracted using the individual CI user's daily strategy, and as such, this approach incorporates information about the number of active electrodes, shape of the compression function and electrical dynamic range. High correlation (r = 0.96) was achieved with the proposed index when evaluated with speech-reception thresholds (SRTs) obtained by CI users in steady and speech-masker conditions. This outcome suggests that the information …

Speech rate treatments for individuals with dysarthria: a tutorial.

Research confirms the long-standing clinical observation that patients with dysarthria exhibit variability in speech rate. Thus, modifying speech rate has been documented as one of the best treatment options for these patients. In this tutorial, several published rate control interventions for dysarthric speakers are presented. Studies discussed utilized various interventions, including pacing boards, alphabet board supplementation, visual and auditory feedback, cueing and pacing strategies, and delayed auditory feedback. Interventions discussed represent a hierarchy from "rigid" strategies, w

Clinical design and evaluation of the prototype of a new digital audioprosthesis for profound hearing loss: speech recognition aspects.

The design and implementation of the prototype of a digital hearing aid and its computerized fitting interface, followed by a basic preliminary clinical evaluation concerning speech recognition aspects, is reported. The final device is particularly destined to those patients suffering sensorineural hearing losses with recruitment and problems for speech recognition. The prototype is based on the digital signal processor TMS320C30. The host is a personal computer. The primary concept of the processing strategy is the 'integral treatment of acoustic information' within the remaining auditory fie

Effects of intensive voice treatment (the Lee Silverman Voice Treatment [LSVT]) on ataxic dysarthria: a case study.

This study examined the effects of intensive voice treatment (the Lee Silverman Voice Treatment [LSVT]) on ataxic dysarthria in a woman with cerebellar dysfunction secondary to thiamine deficiency. Perceptual and acoustic measures were made on speech samples recorded just before the LSVT program was administered, immediately after it was administered, and at 9 months follow-up. Results indicate short- and long-term improvement in phonatory and articulatory functions, speech intelligibility, and overall communication and job-related activity following LSVT. This study's findings provide initial…

Speech prosthesis retention problems in dysarthria: case report.

Prosthesis retention and adaptation problems in the management of dysarthria due to paralysis of the palatopharyngeal mechanism has been frequently reported in the literature. This report describes treatment of speech prosthesis retention difficulty in a 20-year-old woman who sustained a cranial cerebral trauma in a motor vehicle accident. Residual deficits included moderate-to-severe dysarthria with a significant palatopharyngeal valving disorder, higher level cognitive impairments, and long-term memory deficits. Speech intelligibility was maximized via the fitting of an intraoral speech pros…

Investigating long-term effects of cochlear implantation in single-sided deafness: a best practice model for longitudinal assessment of spatial hearing abilities and tinnitus handicap.

OBJECTIVES: To evaluate methods for measuring long-term benefits of cochlear implantation in a patient with single-sided deafness (SSD) with respect to spatial hearing and to document improved quality of life because of reduced tinnitus. PATIENT: A single adult male with profound right-sided sensorineural hearing loss and normal hearing in the left ear who underwent right-sided cochlear implantation. METHODS: The subject was evaluated at 6, 9, 12, and 18 months after implantation on speech intelligibility with specific target-masker configurations, sound localization accuracy, audiologic per…

Types of articulation errors in individuals with cleft lip and palate.

OBJECTIVE: To identify types of articulation errors in speech camp. MATERIAL AND METHOD: Sixteen children with cleft palate with or without lip and articulatory defects in Mahasarakham Province and surrounding area enrolled for 3-day intensive speech camp that was held in Srinagarind Hospital. Assessments of speech and language problems: articulation, resonance, nasal emission/turbulence, voice, and intelligibility were performed. Type of articulation errors were grouped based on standard guidelines. RESULTS: Velar production was the most common types, both at word (75%) and sentence levels

Impact of obturation of palatal fistulae on speech quality and aerodynamic parameters in patients with cleft palates.

OBJECTIVES: The aim of this prospective study was to measure nasal and oral airflow during speech, before and after obturation. PATIENTS AND METHODS: Included were children aged 3-18 years with nonsyndromic clefts and palatal fistulae. The corpus used was: syllable /pi/; a sentence containing stop consonants and a nasal phoneme; and the description of a picture of a scene. Analysis criteria were: percentage of nasality; value of average flow for the explosion; perceived nasality and intelligibility; and tolerance of the proposed device. RESULTS: Only 5 children were included due to the obser

Cluttering in Children and Adolescents: Speech Motor Development, Neurocognitive Mechanisms, and Allied Health Implications.

Background/Objectives: Cluttering in childhood and adolescence is characterised by unstable speech timing, excessive coarticulation, irregular rate and reduced intelligibility, yet the developmental mechanisms underpinning these behaviours remain partially understood. This review synthesises empirical and conceptual evidence to examine cluttering through the lenses of speech motor development, neurocognitive mechanisms, task demands and allied-health practice. Four research questions guided the review, focusing on motor characteristics, developmental and neurocognitive mechanisms, task depende

Disentangling a Functional Speech Disorder in the Context of another Neurological Disease: A Case Report.

This clinical case describes the communication profile of a functional speech disorder. A 48-year-old woman was admitted to the emergency se … prosody. Three days later, another assessment was carried out, showing a significant improvement in speech intelligibility. There was a marked variability in speech features defects found over the days, as well as in their severity. These behaviors appear simultaneously with a speech pattern that is not justified by any of the neurological lesions found.
