A comparison of anterior ring fixation constructs in Young-Burgess lateral compression type 2 and 3 (LC2, LC3; AO/OTA 61-B2/B3) pelvic ring injuries: does fixation matter?

OBJECTIVE: To compare outcomes of lateral compression LC2 and LC3 pelvic ring injuries when posterior ring fixation is applied and different anterior ring constructs are used. METHODS: A retrospective analysis from two Academic Level I Trauma Centers of all operatively treated LC2 and LC3 (AO/OTA 61-B2/B3) pelvic ring injuries from January 2019-January 2024. A comparison of anterior ring constructs was made: no fixation vs. fixation; indirect fixation (external fixators and InFix)) vs. internal fixation; long vs. short percutaneous screws. Long percutaneous screws were defined as either bicor

Intramedullary Screw Fixation of Distal Metacarpal Fractures: A Biomechanical Study.

PURPOSE: Intramedullary headless compression screw (IHCS) fixation of metacarpal (MC) fractures is popular. A concern for distal fractures is the potential loss of fragment/head fixation and with intra-articular incursion of hardware. We evaluated the biomechanical stability of IHCS fixation of progressively distal MC osteotomies using a cadaveric simulated active motion model. METHODS: We included 12 matched-pair fresh-frozen cadaveric hands (median age, 73.5 years; 75% men), excluding matched-pair fingers with deformities/severe arthritis at the metacarpophalangeal joint or proximal interph

Biomechanical evaluation of different medial column fixation patterns for valgus pilon fractures.

BACKGROUND: The purpose of this study was to perform a biomechanical analysis to compare different medial column fixation patterns for valgus pilon fractures in a case-based model. METHODS: Based on the fracture mapping, 48 valgus pilon fracture models were produced and assigned into four groups with different medial column fixation patterns: no fixation (NF), K-wires (KW), intramedullary screws (IS), and locking compression plate (LCP). Each group contained wedge-in and wedge-out subgroups. After fixing each specimen on the machine, gradually increased axial compressive loads were applied wi

Finite element analysis of the Union Plate in treating elderly acetabular fracture patients.

BACKGROUND: Elderly acetabular fractures are one of the more difficult types of fractures to resolve. For patients at this age, the more common type of fracture is comminuted. How to better fix this type of fracture has always been an issue of concern. This study was performed to observe the mechanical properties of different internal fixation methods used in treating elderly acetabular fracture patients. METHODS: A model of a comminuted acetabular fracture in osteoporosis was established, consisting of an anterior column-posterior hemitransverse fracture with disruption of the quadrilateral

Biodegradable versus titanium plates and screws for paediatric facial skeleton fractures.

OBJECTIVE: The use of biodegradable materials represents a new option in the treatment of paediatric facial skeleton fractures. The benefits of a resorbable system include reductions in time for long-term stability, diminished immobilisation period, and elimination of painful procedures for implant removal. The resorption of the material did not influence bone repair and growth. Bioresorbable plates and screws get completely excreted through physiological routes. MATERIAL AND METHODS: The age of the patients ranges from that of pre-schoolers till 18 years. The mean age of boys and girls was 1

Scaphoid screw fixation perpendicular to the fracture plane: Comparing volar and dorsal approaches.

INTRODUCTION: To percutaneously fixate a midwaist scaphoid fracture, both volar and dorsal approaches are considered valid options although they may have different screw insertion angles relative to the scaphoid fracture plane influencing fixation stability. In this virtual simulation study, we investigated the accessibility of placing a screw perpendicularly to the fracture plane in transverse and horizontal oblique scaphoid midwaist fracture models and compared standard volar and dorsal approaches. MATERIAL AND METHODS: Computed tomography scans of 38 healthy wrists were used to obtain virt

Closed Reduction and Percutaneous Fixation of Calcaneal Fractures in Children.

Open reduction and internal fixation has been widely used to treat displaced intra-articular calcaneus fractures in children. However, the complications of surgical trauma and the wound created through the extended lateral approach cannot be ignored. This study analyzed the outcomes of displaced intra-articular calcaneal fractures in children treated with closed reduction and percutaneous fixation. Medical records of pediatric patients who had displaced intra-articular calcaneus fractures and underwent closed reduction and percutaneous fixation at the study institution between January 2008 and

Establishing a central zone in scaphoid surgery: a computational approach.

PURPOSE: Scaphoid fractures are commonly fixed with headless cannulated screws positioned centrally in the scaphoid. Judgement of central placement of the screw may be difficult. We generated a central zone using computer analysis of 3D reconstructions of computed tomography (CT) images. As long as the screw axis is completely contained within this central zone, the screw would be considered as centrally placed. METHODS: Thirty cases of 3D CT reconstructions of normal scaphoids in a computerised operation planning and simulation system (Vxwork software) were obtained. The central zone was est

[Growth and deformity after in situ fixation of slipped capital femoral epiphysis].

BACKGROUND: For mild to moderate slipped capital femoral epiphysis (SCFE) in situ fixation is the current treatment standard. However, concerning the implant selection (screw versus k-wires) as well as the prophylactic stabilisation of the non-affected hip, controversies still exist. The aim of this study was to analyse femoral residual growth and femoral deformities after in situ fixation of SCFE either with k-wires or screws. PATIENTS AND METHODS: We conducted a retrospective analysis of the radiographs of adolescents treated for SCFE in our department between 01/2003 and 02/2011. To evalua

A new volar vascularization technique using the superficial palmar branch of the radial artery for the collapsed scaphoid nonunion.

Achieving union to prevent scaphoid nonunion advanced collapse wrist in the scaphoid nonunion is a challenging clinical problem. Much of the difficulty relates to the tenuous blood supply to the scaphoid. One unsolved reconstructive problem is the collapsed scaphoid that requires an intercalated wedge graft with proximal pole avascular necrosis. We offer a simple technique that only requires preservation of the superficial palmar branch of the radial artery that is typically ligated during the volar approach to the scaphoid. This technique can also be used during any open volar approach to the

Carpal derangement and associated carpal valgus in a dog.

This case report describes derangement of the numbered carpal bones resulting in a valgal growth deformity in the right carpus of a seven-month-old dog. Radiographic assessment of the right carpus revealed abnormalities in the size and shape of the numbered carpal bones and carpal valgus. Surgical correction of the growth deformity was planned by partial carpal arthrodesis; however medial collateral laxity associated with the carpal valgus necessitated a pancarpal arthrodesis to achieve correct limb alignment.

Use of 3.5-mm acetabular reconstruction plates for internal fixation of flail chest injuries.

The technique of using 3.5-mm acetabular reconstruction plates for multiple rib fractures with cerclage wires has been successful. We modified it by securing the reconstruction plates to the fractured ribs with cortical screws using the standard AO-ASIF (Arbeitsgemeindschaft fur Osteosynthesefragen [Association for the Study of Internal Fixation]) group technique. Our method for the internal fixation of the unstable chest wall is described. It is relatively simple and provides uniform results.

Internal fixation of acute stable scaphoid fractures in the athlete.

Our study evaluated the results of surgical repair of acute carpal scaphoid fractures in athletes and the time required for the athletes to return to play. Although playing casts are a nonsurgical option, they reduce the effectiveness of the athlete in sports that require maximal manual dexterity; thus, the management of scaphoid fractures is challenging when early return to sports is desired. Twelve athletes with 12 acute midthird scaphoid fractures were treated with Herbert screw fixation. All patients were in-season athletes in sports that precluded the use of a playing cast. Return to spor

Evaluation of an interfragmentary compression system for the repair of equine femoral capital physeal fractures.

Femoral neck and proximal epiphyseal lengths were measured in 37 femurs from 19 cadaver foals that were 1 day to 12 months old to determine the applicability of a human interfragmentary compression system to equine femoral capital physeal fractures. Because components of the implant system are available only in fixed sizes, its use was possible in foals older than 5 weeks of age, but not in younger foals. The 135 degree angle plate conformed best to the equine femur. Femoral capital physeal fractures were created surgically and repaired with the implant system in three foals. Fracture stabilit

Nonunion of slightly displaced fractures of the lateral humeral condyle in children: an update.

Inadequate treatment continues to be the leading cause of nonunion of slightly displaced fractures of pediatric lateral humeral condyles, despite references to prevention in the orthopaedic literature. In most cases, the elbow was established nonunion can be salvaged by early stabilization and bone grafting, provided that the fragment is in acceptable position and the growth plate of the condylar fragment is open. Procrastination may allow the physis of the condylar fragment to close prematurely, preemptying the golden opportunity to salvage the child's elbow. The author has collected 23 simil
