Pathophysiology of early failure of autologous aortic heart valves (ATCV).

BACKGROUND: Degeneration remains the major drawback of bioprostheses. Among various concepts to mitigate degeneration, the use of autologous pericardium for intraoperative construction of aortic valves (ATCV) was revived recently. Based on in-vivo studies the problem of tissue failure was claimed to be oversome by short immersion in glutaraldehyde. METHODS: Two often ATCV implanted 1994-1996 had to be replaced because of valvular insufficiency due to leaflet shrinkage or tearing. Pathophysiology of failure was evaluated by light microscopy and immune histology, scanning electron microscopy (S

[A case report of emergency Bentall re-operation].

A 51-year-old male of Marfan syndrome with annuloarotic ectasia underwent the Bentall operation. One and a half months later, he suddenly fell into the left ventricular failure associated with lung edema. Echocardiogram revealed total occlusion of the graft and aortic valve detachment was suspected. An emergency operation was performed. The aortic valve was completely detached due to infectious endocarditis. As anastomosed portions of the proximal coronary arteries and distal aorta were intact, these rims of the old graft were reserved. A new composite graft was anastomosed distally to the abo

Internal repair of left ventricular pseudoaneurysm late after mitral valve replacement.

Late left ventricular pseudoaneurysm is a rare complication after mitral valve replacement. Most investigators have recommended surgical repair to treat left ventricular pseudoaneurysm, since untreated left ventricular pseudoaneurysm have a high risk of rupture. Here, we report a case of a 57-year old man with left ventricular pseudoaneurysm. He had two prior mitral valve replacements 16 and 19 years ago, as well as mitral and aortic valve endocarditis causing mitral valve perivalvular leak and perforation of the aortic valve. The mitral and aortic valves were replaced with bovine pericardial

Third-Generation Transcatheter Aortic Heart Valve with Reverse Parachute Sealing Cuff in Patients with Aortic Valve Disease.

BACKGROUND: The Navitor (Abbott Inc, IL, USA) transcatheter heart valve is a novel third-generation self-expanding bioprosthesis with specific features to mitigate paravalvular regurgitation (PVR). Owing to its novelty, there is a paucity of data on its application in clinical practice. METHODS: Consecutive cohort analysis of the use of the Navitor system in an as-treated clinical setting at a quaternary heart hospital. RESULTS: Sixty consecutive non-clinical trial patients treated with Navitor were identified. All patients underwent a successful procedure. The mean age was 79.3 years (±SD 7

Quantitative assessment of aortic regurgitation following transcatheter aortic valve replacement.

Introduction: Transcatheter aortic valve replacement (TAVR) is expanding to lower risk and younger patients with severe symptomatic aortic valve disease. Despite clinical and technological improvements, post-procedural aortic regurgitation (AR) remains a limitation of TAVR, particularly when compared to surgical aortic valve replacement. Although several methods for AR quantification after TAVR are currently available, its exact graduation in everyday clinical practice remains challenging.Areas covered: This review describes the currently available evaluation methods of AR after TAVR, with a s

Evaluation of coronary heights after Bio-Bentall using Piehler technique.

The valve-in-valve procedure is an alternative to the redo surgery for structural valve deterioration following the Bio-Bentall. However, the risk of coronary obstruction and the feasibility of this approach remain unclear. Using computed tomography, we compared the aortic root geometry of 14 consecutive patients (13 true aortic aneurysms) with Bio-Bentall between April 2011 and April 2024 preoperatively and postoperatively. The Piehler technique was used in all coronary artery reconstructions. During the follow-up period, no reconstructive coronary events or valve-related reoperations were ob

Comparison of mechanical versus bioprosthetic valve replacement in mitral stenosis surgery.

BACKGROUND: Mitral stenosis is a common valvular heart disease, and the choice of valve material during surgical treatment significantly impacts patient prognosis. Currently, data on mid-term outcomes following valve replacement in Chinese patients with mitral stenosis are limited. This study aimed to compare the clinical efficacy and mid-term prognosis between mechanical and bioprosthetic valves in mitral stenosis surgery. METHODS: We retrospectively analyzed 200 patients who underwent mitral valve replacement at our cardiac surgery department between January 2015 and December 2020. Patients

Potential role of closure time with adenosine diphosphate in the diagnosis of mitral prosthetic paravalvular leak and prognosis after repair.

INTRODUCTION AND OBJECTIVES: Mitral paravalvular leak (PVL) is associated with hemolysis and heart failure. It has also been linked to shear stress-induced von Willebrand factor abnormalities that prolong closure time with adenosine diphosphate (CT-ADP). The aim of this study was to assess whether CT-ADP serves as a biomarker for mitral PVL diagnosis, follow-up, and prognosis after repair. METHODS: In this prospective study, 89 patients (27 with moderate-to-severe mitral PVL referred for closure, group 1; 31 with prosthetic mitral valves without PVL, group 2; and 31 controls without valvular

Clinical impact of residual gradient and 3D orifice area after transcatheter mitral valve-in-valve implantation.

AIMS: The mitral valve-in-valve (MVIV) procedure has emerged as an important therapy in failing surgical bioprosthetic valves. We aimed to evaluate factors associated with outcome following MVIV intervention, specifically the impact of 30-day MVIV gradient. We also explored the value of intraprocedural MVIV three-dimensional anatomic orifice area (3D-AOA) by transoesophageal echocardiography in a subset of patients (n = 68). METHODS AND RESULTS: Consecutive MVIV patients from a single institution with 30-day transthoracic echocardiography (TTE) were included (N = 100). Clinical and echocardio

Long-term outcomes following aortic valve replacement in bioprosthetic vs mechanical valves.

BACKGROUND: Aortic valve disease(AVD) accounts for 33 % of valvular heart disease(VHD) but causes over 60 % of VHD mortality. For surgical AVR, mechanical valves are recommended for patients <50 years old and bioprosthetic valves for those >70 years old. OBJECTIVES: To investigate the long-term differences following AV replacement(AVR) comparing bioprosthetic and mechanical valves in patients aged 50-70. METHODS: 4,927 patients underwent AVR, 744 of which were propensity-matched 2:1 for bioprosthetic and mechanical valves. Outcomes included mortality, morbidity, and rates of reoperation. RE
