Actinic granuloma is a self-limiting chronic disorder with elastolytic granulomas, mainly of the sun exposed skin and unknown pathogenesis. We report a case of a middle aged woman with multiple annular erythematous lesions. Histopathology revealed degenerated elastic fibres with histiocyte aggregates surrounded by multiple giant cells suggestive of elastolytic granulomas. She also had associated dyslipidemia and hypothyroidism. Actinic granuloma is relatively rare in the Indian population and needs to be differentiated from conditions like sarcoidosis and granuloma annulare.

We describe an isolated extracardiac unruptured acquired aneurysm in the right coronary sinus of Valsalva, which was seen in a 55-year-old woman with Marfan's syndrome. The patient underwent aortic root replacement using a reimplantation technique. Pathologic examination revealed absence of the medial elastic fiber of the aortic wall of the normal sinus of Valsalva. This result supports the preference of entire root replacement instead of patch repair of the affected sinus for the isolated aneurysm in 1 sinus of Valsalva in a patient with Marfan's syndrome.

Elastosis perforans serpiginosa is a rare, primary perforating dermatosis, frequently associated with certain genetic diseases and characterized by the transepidermal extrusion of elastic fibers. The present case report describes this dermatosis in a 19-year old female patient with Down's syndrome, who presented with asymptomatic erythematous, keratotic papules in an arciform pattern, located on her right forearm and knee, which had been present for five years. Following histopathological confirmation, treatment with cryotherapy was initiated, resulting in partial remission of the lesions.

The vessels of the basis cerebri were histologically examined in 45 patients who had died from arterial aneurysmal rupture. Angiography had made at the clinic detected cerebral vasospasm of varying severity and extent. All autopsies showed ischemic encephalomalacia in specimens. The findings indicated that arteriostenosis angiographically identified as vasospasm, by causing ischemic events was induced by morphological changes in the arterial wall. The arterial structural changes found were characterized by subsequent changes in the intima, elastic membrane, and medial layer.

Fractures of the mandibular condyle are frequently seen. There is no consensus on the most appropriate treatment modality. Closed reduction with help of arch bars and guiding elastics is in most cases sufficient. Open reduction and fixation with a plate osteosynthesis or a specially designed lag screw is occasionally performed. In a histochemical and electron microscopic study of 45 cases of keratoconus, oxytalan fibres, a member of the elastic fibre family, were found in the extracellular tissues of the cornea. Oxytalan fibres were identified beneath the epithelium over the thinned central region of the conical cornea as well as in areas of stromal scarring. In a number of instances the apparently thickened Descemet's membrane was shown to include an oxytalan-containing retro-corneal layer. Oxytalan fibres were found to be immunostained by antibodies raised against collagen type VI.

A method of obtaining in vivo information about the component elements of the accommodation mechanism is described. Changes in axial lens thickness during accommodation are measured continuously by ultrasonographic biometry. The measurements are fitted with a dynamic biomechanical model of the mechanism of accommodation. A previously fit 20-year-old man presented with a large haemothorax following a stab wound to the left chest. Pre-operative airway assessment indicated that tracheal intubation would be routine. On induction of anaesthesia, visualisation of the larynx proved impossible because of soft tissue swelling. Successful intubation was eventually achieved with the aid of a gum elastic bougie. At operation, the patient's common carotid artery was found to have been perforated close to its origin on the aorta. The patient made an uneventful recovery.

The effects of terbutaline, propranolol, carbachol, and atropine on the static urethral closure function were investigated in 20 healthy women. Terbutaline caused a statistically reduction of the urethral pressure in the high pressure zone, but not at the bladder neck or in the distal urethra. Electron microscopic and X-ray microanalytic studies were performed on four cases of argyria; one generalized and three localized. Deposition of electron dense granules was predominantly found on elastic fibers and around basal laminas of secretory portions of eccrine glands, although the amount of deposition was much less in the case of generalized argyria. In all four cases, X-ray microanalysis revealed that the depositions consisted mainly of silver, selenium, and sulfur. The importance of selenium in the detoxification of heavy metals was discussed.

Pathological data from 122 cases of gynecomastia are studied. According to the importance of epithelial hyperplasia and of specialised stroma, 3 types are described. Every type is compared with the age of patients, the duration of the illness and occasional etiologic circumstances. It is concluded that these 3 types are not evolutive stages, that type II is the most frequent at any age and that there is no relation between etiology and histological type, except between oestrogenic therapy and type I. The alterations of connective tissue, especially breakdown of elastic fibers, are important.

This is a report of a 6-year-old child who died suddenly while at play and without any prior known illness. Autopsy revealed an extensive, fibrotic myocardial infarct involving the apex, diaphragmatic wall of the left ventricle and interventricular septum. The left anterior descending and the right coronary arteries presented with large, bulging calcified aneurysms. The aneurysms showed marked fibrous intimal thickening, atrophy of the media and extensive fragmentation of elastic elements. No evidence of active inflammation was present.

Septal cartilage deviations which are usually present in deformities of the cartilaginous nasal septum are corrected by scarification on the concave side of the curvature. Histological studies indicated that this procedure was best adapted to the structure of the elastic fibres constituting the cartilaginous nasal septum. Follow-up examinations in 33 patients who having undergone surgery showed nasal breathing to be unobstructed bilaterally in all cases; in 29 patients an aesthetically satisfactory nasal shape was restored and full re-alignment of the cartilaginous nasal septum was obtained.
