Hereditary lipo-muscular atrophy with joint contracture, skin eruptions and hyper-gamma-globulinemia: a new syndrome.

We previously reported two siblings with decreased subcutaneous adipose tissue, muscular atrophy, joint contractures, recurrent skin eruptions, hyper-gamma-globulinemia, and reduced natural killer cell activity. Some of their clinical features are similar to those of partial lipodystrophy, but they are distinct in that muscular atrophy, joint contractures and recurrent skin eruptions are not found in patients with partial lipodystrophy. Thirteen other Japanese patients with similar clinical manifestations have been reported. We propose that such cases should be considered a distinct clinical e…

Muscle involvement in Walker-Warburg syndrome. Clinicopathologic features of four cases.

Walker-Warburg syndrome is a lethal autosomal recessive disorder characterized by anomalies of the central nervous system and the eye. Recently, elevation of muscle enzymes and congenital muscular dystrophy have been found to contribute to the diagnosis of this syndrome. The authors studied two pairs of siblings with Walker-Warburg syndrome, offspring of consanguineous parents from two unrelated families. The patients had hydrocephalus, congenital glaucoma, elevated muscle enzymes, and hypotonicity. The histologic and ultrastructural muscular changes were consistent with congenital muscular dy…

Familial spastic paraplegia with peroneal amyotrophy. A family with hypersensitivity to pyrexia.

We describe 4 siblings with spastic paraparesis and peroneal amyotrophy who were prone to severe pain and painful dysesthesias, tetraparesis and pyramidal signs during pyrexial episodes of variable etiology. These symptoms cleared almost completely in 10-20 days. Nerve conduction velocity was reduced more markedly during the spells of fever. Muscle biopsy specimen was normal. Some transient functional disturbance of membrane equilibrium of the nervous pathways of both central and peripheral nervous systems was probably responsible for the attacks during pyrexial episodes.

[Rieger syndrome as an expression of neural crest dysgenesis].

Malformations of the anterior segment of the eye can be associated with different systemic findings. The regular combination of aplasia of the permanent central and lateral incisors, typical ocular changes, and umbilical dysmorphology is known as the Rieger syndrome. Aplasia of the permanent incisors can serve as a diagnostic symptom. We report on three siblings who had characteristical symptoms of Rieger syndrome. The tissue involved derives from the neural crest cells. Mesenchymal dysgenesis is discussed as the etiology of this disorder.

Sex chromosome aneuploidy and Bardet-Biedl syndrome.

The inheritance of Bardet-Biedl syndrome is thought to be autosomal recessive. Of the approximately 500 case reports in the literature, three patients were found to have sex chromosome aneuploidy. The authors describe two siblings with Bardet-Biedl syndrome, one of whom has a unique sex chromosome aneuploidy with mosaicism, including deletion of the short arm of the X chromosome (45,X/46,X,del(X)(p21)). The possible significance of sex chromosome aneuploidy and the Bardet-Biedl syndrome is discussed.

Asymmetric crying facies with microcephaly and mental retardation. An autosomal dominant syndrome with variable expressivity.

An infant boy with asymmetric crying facies, microcephaly, developmental retardation and failure to thrive is reported. His two siblings died in the newborn period because of complex congenital heart defects. The mother and the maternal grandmother have asymmetric crying facies, microcephaly and normal intelligence. A maternal aunt has severe physical and mental retardation, facial asymmetry, microcephaly, and cleft palate. This family allows an expansion of the spectrum of malformations associated with asymmetric crying facies and suggests autosomal dominant inheritance with variable expressi…

Neurological findings in triosephosphate isomerase deficiency.

Two siblings with hemolytic anemia caused by triosephosphate isomerase deficiency developed a progressive neurological syndrome featuring dystonic movements, tremor, pyramidal tract signs, and evidence of spinal motor neuron involvement. Intelligence was unaffected. The findings in these patients and in 14 previously published cases indicate that neurological manifestations are an integral part of the disorder and suggest that specific structures in the basal ganglia, brainstem, and spinal cord bear the brunt of the pathological process, which does not affect the cerebral cortex.

The family history of uncomplicated congenital hydrocephalus: an epidemiological study based on 270 probands.

A longitudinal study was carried out on the family history of 270 babies with uncomplicated congenital hydrocephalus. They had 453 siblings of whom five (11.1/1000) had congenital hydrocephalus and nine (19.9/1000) neural tube defects. These data, together with those obtained from previously published reports indicate that the aetiology of the two conditions is probably the same in many cases, although there are unusual exceptions in special families even apart from those with the sex linked, recessively inherited hydrocephalus.

Vasovagal syncope in children requiring pacemaker implantation.

Four children presented with episodic loss of consciousness. Two of the children were siblings. Neurologic causes were initially suspected in all but extensive evaluations and EEGs excluded seizures. ECGs in one patient demonstrated first- and second-degree AV block and first-degree AV block in another. The QT and QTc intervals were normal in all. Eyeball pressure in all patients produced profound bradycardia. All patients became asymptomatic after the implantation of pacemakers, although one died 15 months afterward from another cause.

Intra- and interhemispheric processing of visual information in callosal agenesis.

Two siblings with total callosal agenesis were compared with two control groups on a number of tachistoscopic tasks involving unilateral and bilateral presentation of verbal and nonverbal stimuli. Acallosals were able to effect intra- and interhemispheric comparisons and to correctly identify all kinds of stimuli in either visual field. However, they responded more slowly, especially in the bilateral condition where they also made more errors than the controls. The results are interpreted in terms of increased use of extra-callosal commissures.

[Ciliary immotility syndrome without situs inversus in 2 children].

The authors report two cases of immotile cilia syndrome occurring in two children without situs inversus. The two boys, 3 and 7 years old, had bronchiectasis, chronic sinusitis and recurrent upper airway infections. In the siblings, we found Kartagener's syndrome (sister of the first boy, and two sibs of the second). The diagnosis in the 2 cases was performed by study of ciliary motion in bronchial brushing. Ultrastructural examination of biopsies from bronchial mucosa showed specific defects of the axoneme.

Metopic craniostenosis as a consequence of fetal head constraint: two interesting experiments of nature.

Two instances of metopic craniosynostosis provide dramatic experiments of nature which implicate fetal head constraint as one cause of early sutural fusion. The presumed restriction of growth stretch at the metopic suture in one instance was due to a bicornuate uterus in which the fetal head was markedly constrained. The second instance was in one of monozygotic triplets reared in a small mother in which the affected fetal head had been wedged between the hips of the two unaffected siblings.

Phenotype i associated with congenital cataract in Japanese.

A Japanese family with two siblings of phenotype i is presented. Both had a past history of surgical treatment for congenital cataract. In Japan, 18 individuals of phenotype i, including our case, have been found in ten unrelated families. Seventeen of them had congenital cataract. Cataract was not found in any of the 45 phenotype I members in these families. It is briefly discussed why these two linked and quite rare genes were found in combination only in Japanese persons.

Chronic familial hyperphosphatasemia.

Two siblings displaying macrocrania and multiple skeletal deformities, as well as cardiomegaly, had high levels of serum alkaline phosphatase and markedly increased urinary hydroxyproline excretion. The radiological findings of chronic familial hyperphosphatasemia, which are typical of a rare bone-remodeling disease, are presented. Scintigraphy disclosed intense uptake of the radionuclide by the skeletons of both patients. This finding, considered to be related to abnormal collagen metabolism, can be used in the diagnosis and assessment of skeletal involvement in such patients.

Pigmented Xerodermoid: first report of a family.

Pigmented Xerodermoid is a variant of Xeroderma pigmentosum characterized by late manifestation of sun--sensitivity, dyschromic and poikilodermatic skin changes and multiple malignant skin tumors in light exposed areas. In contrast to Xeroderma pigmentosum excision repair system is normal, but DNA synthesis after UV--irradiation is markedly prolonged, suggesting that the postreplicational repair mechanism is disturbed. A family of 89 members with 4 siblings suffering from pigmented Xerodermoid was examined. The pedigree of this family suggests autosomal recessive inheritance in this disease.

Spontaneous Rupture of an Infant's Cornea.

A 2-year-old boy of consanguineous parents presented with spontaneous globe rupture of the right eye that initially raised suspicions of child abuse. Other ocular and extraocular findings, a similar history of corneal rupture following minor trauma in his sibling, and genetic analysis confirmed the diagnosis of brittle cornea syndrome. [J Pediatr Ophthalmol Strabismus. 2022;59(4):e42-e45.].

A connective tissue defect in two rabbits similar to the Ehlers-Danlos syndrome.

A connective tissue disease resembling the human Ehlers-Danlos syndrome is reported in two sibling rabbits about four months old. The clinical signs included skin hyperextensibility and fragility and poor wound healing. There were ultrastructural abnormalities in the structure of the collagen fibrils and in their arrangement in bundles of fibres. Affected rabbits may be a useful laboratory animal model for collagen disorders in human beings.

[Familial occurence of congenital short bowel (author's transl)].

We report the clinical and post-mortem findings of an infant with inborn short bowel. One sibling died because of the same malformation. The parents are cousins. This malformation is combined with malrotation, often with pylorus-stenosis too, and can cause an intestinal obstruction syndrome. All presently known cases in the pertinent literature are summarized. The occurence of familial cases suggest a rezessive autosomal disease.

Abnormal neutrophil chemotaxis in a syndrome of unusual facies, proportionate small stature and sensorineural deafness-mutism.

Two of 5 children in one family presented with unique facies, proportionate small stature and sensorineural deafness-mutism. One of the children who had a history of recurrent infections, was shown to have a defect of leukocyte chemotaxis. Although impairment of chemotaxis could not be demonstrated in the other affected sibling, it is unlikely that the association of a previously undescribed syndrome and a rare disorder of chemotaxis is a chance occurrence.

Bony and vascular anomalies of the head. Their possible relation to anencephaly.

This study links a midfacial anomaly in an 8-year-old boy, with underlying bony skull defects and abnormalities of the cerebral vasculature, with an earlier spontaneous miscarriage and the subsequent birth of a frankly anencephalic sibling. We believe that the midfacial and vascular anomalies in the second child could represent a forme fruste of anencephaly, which should alert the physician to monitor subsequent pregnancies for the complete anencephalic phenotype.
