Survival studies after endoscopic transgastric oophorectomy and tubectomy in a porcine model.

BACKGROUND: Advances in endoscopic equipment have enabled endoluminal techniques to supplant select surgical procedures. Access to extraluminal structures holds the potential to revolutionize flexible endoscopy but, to date, has been limited. The aim of this study was to demonstrate the feasibility of endoscopic transgastric organ resection (oophorectomy and tubectomy) with 2-week survival. METHODS: Female Yorkshire pigs underwent general anesthesia, and a sterile esophageal overtube was placed. Antibacterial gastric lavage was performed, and a sterile gastroscope was passed through the overt

Transoral obesity surgery: endoluminal gastroplasty with an endoscopic suture device.

BACKGROUND AND STUDY AIMS: Surgical creation of a small gastric pouch with a restricted outlet is a well-established option for the treatment of morbid obesity. This report describes initial experience with endoscopic transoral stomach partition using a newly designed suturing apparatus. MATERIALS AND METHODS: A fresh porcine stomach was placed in the Erlangen model. A prototype suture device, incorporating a curved needle and an intracorporeal tightening mechanism, was used in this procedure. A long fishing line was first introduced into the stomach, with the two ends left outside. The sutur

Human antral damage induced by alcohol is potentiated by enprostil.

Enprostil, a synthetic analogue of prostaglandin E2, has been shown to protect the human gastroduodenal mucosa from aspirin injury. This study was designed to determine if enprostil protected against alcohol damage. A double-blind, randomized, cross-over study was performed on eight healthy adult men. After an overnight fast, a gastroscope was inserted and the antral mucosa was sprayed with a 10-mL test solution containing either enprostil (70 micrograms) or its vehicle (control). After 15 minutes, mucosal injury was scored (0-5), and the mucosa was sprayed with 100 mL 80% ethanol. Mucosal inj

Design, Modeling, and Validation of a Pneumatically Actuated Distal Segment for Endoscopy.

To reduce the risk of patient cross-contamination associated with reusable endoscopes, health care systems are increasingly turning to single-use devices. While this shift improves hygiene, it raises significant environmental and economic concerns due to increased medical waste and long-term costs. In response, this study proposes a semi-disposable pneumatic endoscope concept designed to support the transition toward greener endoscopy. It is composed of a reusable handle and a disposable, pneumatically actuated insertion tube. The study focuses on the design, modeling, and prototyping of the f

Case Report: Resectability-guided management of an indeterminate periduodenal spindle cell tumor initially suspected to be a gastrointestinal stromal tumor.

Duodenal mesenchymal spindle cell tumors are uncommon and may overlap radiologically with gastrointestinal stromal tumors (GISTs) and retroperitoneal lesions. We report a 55-year-old woman with abdominal discomfort, nausea, and vomiting. Contrast-enhanced computed tomography demonstrated an approximately 6.0 × 3.8 cm mass posterior to the pancreatic head that was interpreted as a probable duodenal stromal tumor. Gastroscopic biopsy was non-representative. Given the symptoms, tumor size, uncertain origin, and vascular proximity, surgical exploration was undertaken. The tumor was firm and densel

Endoscopic intermuscular dissection for rectum-sparing resection of a refractory rectal lesion.

BACKGROUND AND AIMS: Endoscopic intermuscular dissection (EID) is an emerging technique that enables deeper mural resection beyond the submucosa, offering a rectum-sparing approach for fibrotic or superficially invasive rectal lesions. We describe the use of EID after multiple failed EMRs in a patient with a rectal laterally spreading tumor (LST). METHODS: A 79-year-old man with a 3-cm rectal LST located 3 cm from the anal verge underwent EID after 4 previous unsuccessful resection attempts. Preprocedural evaluation with narrow-band imaging and indigo carmine chromoendoscopy suggested a fibro

Removal of an impacted apricot pit from a scarring duodenal stenosis using the endoscopic retrograde cholangiopancreatography (ERCP) stone extraction technique.

A 66-year-old man presented with repeated vomiting for 5 days. Initial gastroscopy showed gastric retention while computed tomography (CT) revealed a 1.8*1.1 cm, oval-shaped, high-density object in the duodenum. Considering his past medical history of a surgical repair for duodenal ulcer perforation 20 years ago, a diagnosis of foreign body (FB) impaction causing gastric outlet obstruction was established. After gastric lavage, a second gastroscopy was performed. A brownish round FB impacted upon scarring stenoses at the junction of the 1st and 2nd part of duodenum was visualized after advance

Over-The-Scope Clip for the Immediate Management of Stapfer Type-1 Duodenal Perforation during Endoscopic Retrograde Cholangiopancreatography.

Iatrogenic Stapfer type-1 duodenal perforations during endoscopic retrograde cholangiopancreatography (ERCP) typically necessitate surgical management and carry significant morbidity and mortality risk. Here, we present a case of a large duodenal perforation during ERCP managed endoscopically with an over-the-scope clip (OTSC) and describe the subsequent post-procedural management. An 80-year-old woman presented to the emergency department with acute cholangitis. Abdominal ultrasound scan revealed a dilated biliary tree with echogenic material in the common hepatic and intrahepatic ducts. The

Decreased xCT activity in patients associated with Helicobacter pylori infection.

Objective: In animals, Helicobacter pylori (Hp)-induced gastric injury is accompanied by a decrease in the activity of the cysteine/glutamate transporter (xCT), which regulates extracellular glutamate levels. However, the impact of xCT activity in patients with Hp infection remains unclear. This study aims to investigate variations of xCT activity in the gastric mucosa of patients with Hp infection and to provide a clinical basis for identifying targets related to Hp infection. Methods: Our study included a total of 67 patients with gastritis, which consisted of 44 Hp-negative and 23 Hp-positi

Risk factors and clinical correlates of neoplastic transformation in gastric hyperplastic polyps in Chinese patients.

Gastric hyperplastic polyps (GHPs) have a potential risk of neoplastic transformation, but the responsible mechanisms have not yet been established. We conducted a study involving 55 patients (33 female) who had undergone endoscopic or surgical resection of GHPs. We compared 16 patients who had GHPs showing neoplastic transformation with 39 patients who had non-neoplastic GHPs. We analyzed differences in serology, gastroscopic manifestations and pathology between the two groups in order to establish risk factors that may be associated with neoplastic transformation. The mean age of the cohort
