Fixation with poly-L-lactide screws in hip osteotomies.

UNLABELLED: The purpose of our study was to investigate the bone union rates and complication rates, including breakage of screws, infection, and osteolysis, in hip osteotomies using poly-L-lactic screws. We prospectively followed up 111 hips in 100 patients using conventional radiography, in which 81 rotational acetabular osteotomies, 26 Chiari pelvic osteotomies, and four Sugioka's transtrochanteric osteotomies were performed. The mean age of the patients at the time of surgery was 34 years, and the mean followup was 5.4 years. Eighty-one osteotomized acetabula and 98 osteotomized greater tr

Maintaining the space between the mandibular ramus segments during bilateral sagittal split osteotomy does not influence the stability.

BACKGROUND/PURPOSE: Three-dimensional computer-assisted orthognathic surgery allows to simulate the space between the mandibular ramus segments, i.e. intersegmental gap, for the correction of facial asymmetry. The purposes of the study were to estimate the screws- and mandible bone-related changes from the early postoperative period to the period after the debonding and to measure the association between the intersegmental gap volume and the screws- and mandible bone-related changes. METHODS: This cone-beam computed tomography (CBCT)-assisted retrospective study assessed the stability of the

Primary stability of interference screw fixation. Influence of screw diameter and insertion torque.

Interference screw fixation is frequently used in anterior cruciate ligament reconstruction with the bone-patellar tendon-bone graft. There have been no data available about the influence of screw diameter and insertion torque on primary stability. We designed a cadaveric study using paired knees to analyze the comparative effectiveness of 20-mm long screws of different diameters (9 mm versus 7 mm) and the significance of torque during screw insertion. Testing to failure was performed in a tensiometer with a crosshead speed of 200 mm/min. After slippage of a bone peg in the 7-mm screw specimen

In-silico study of type 'B' condylar head fractures and evaluating the influence of two positional screw distance in two-screw osteosynthesis construct.

Clinical fixation screws are common in clinical practices to fix mandibular condyle fractures. Evidence suggests significance of 'working length' that is, distance between proximal and distal fixation screws in proximity to the fracture in orthopaedic implant design. In pursuit of stable implant-bone construct, this study aims to investigate the biomechanical performance of each configuration considered in the study and provide an optimal working length between the screws for clinical reference. Finite element models of virtually designed broken condyle as type 'B' were simulated and analysed

Femoral interference screw placement through the tibial tunnel: a radiographic evaluation of interference screw divergence angles after endoscopic anterior cruciate ligament reconstruction.

Fifty patients who underwent single-incision arthroscopically assisted anterior cruciate ligament (ACL) reconstruction using bone-patellar tendon-bone autograft were reviewed. All procedures were performed using a technique for placement of the femoral interference screw that reduced the divergence between the femoral interference screw and the femoral bone plug. This technique includes securing the femoral interference screw by placing the screw driver through the tibial tunnel, anterior to the tibial bone plug. Using this technique, 8% of our patients had significant femoral bone plug and in

Expandable pedicle screw may have better fixation than normal pedicle screw: preclinical investigation on instrumented L4-L5 vertebrae based on various physiological movements.

Lumbar spine fracture is typically treated by means of screw fixation, the primary aim of which is to reduce fracture by achieving bony union such that the spinal anatomy is restored. Pedicle screw fixation has certain advantages over conventional vertebral screws, e.g. 3-column fixation and improved surgical alignment. However, expandable pedicle screws have been reported to impart better anchorage as compared to conventional pedicle screws, both in case of healthy and osteoporotic bone. The clinical studies notwithstanding, there is a paucity of preclinical investigations on expandable pedic

Independent Locking Screws for Articular Fragment Fixation in the Elbow.

Articular fractures of the elbow require direct reduction and interfragmentary fixation to restore anatomic stability. The fragments are often small, with limited bone stock, hence implants are often placed through the articular surface to gain maximal purchase. Traditional techniques involve the use of countersunk cortical screws or cannulated headless compression screws. We describe a technique in which small locking screws are used independently from a plate as a substitute for headless or other traditional screws. This method of fixation negates the need for countersinking the screw head,

Stacked Biocomposite Screws in a Single-Stage Revision Anterior Cruciate Ligament Reconstruction Has Acceptable Fixation Strength in a Porcine Cadaveric Model.

BACKGROUND: Stacked screws is a commonly used technique in single-stage revision anterior cruciate ligament (ACL) reconstruction in the setting of bone loss, but there are limited data to support its use. HYPOTHESIS: Two configurations of a biocomposite stacked screws construct have similar fixation strength and linear stiffness as a primary ACL reconstruction construct in a biomechanical model. STUDY DESIGN: Controlled laboratory study. METHODS: A total of 30 porcine legs were divided into 3 groups. Group 1 underwent primary ACL reconstruction with a patellar tendon graft fixed into the fe

Tibial implants: biomechanical and histomorphometric studies of hydroxyapatite-coated and uncoated stainless steel and titanium screws in long-term ovariectomized sheep.

This study was designed to evaluate osteointegration of HA-coated and uncoated titanium and stainless steel screws in the cortical bone of long-term (24 months) ovariectomized sheep (OVX group), in comparison with Sham-aged sheep (control group). The screws were tested biomechanically (extraction torque) and histomorphometrically (affinity index: Al) 12 weeks after their implantation in tibial diaphyses. Tibial cortical bone parameters showed significant differences between the groups, showing a reduction of the selected parameters in the OVX group. ANOVA showed significant effects for both ma

A Novel and Easy Approach for Contouring Surgery in Patients With Craniofacial Fibrous Dysplasia.

Although the bone-shaving procedure is the optimal alternative treatment of craniofacial fibrous dysplasia affecting the maxilla and zygoma, it is difficult to determine the appropriate amount of bone to be removed. The authors therefore developed a novel method for bone shaving using three-dimensional printing technology on computed tomography (CT) scans and screws as a guide. In our method, we implanted screws of predetermined lengths into surgical sites and then performed a burring within the visual field. By using screws, the exact amount of bone could be removed, as determined preoperativ

Free subchondral screws in tibial plateau fractures: do they hinder a total knee arthroplasty? Strategies to prevent complications.

Open reduction and internal fixation of extended lateral column tibial plateau fractures through a tibial condyle osteotomy and limited arthrotomy with the use of free subchondral locking screws is a straightforward and safe technique. However, these free subchondral screws are enclosed in the subchondral bone and therefore virtually impossible to remove after bone healing. The question arises whether these free subchondral screws might hinder a future total knee arthroplasty. In order to refute this, we retrospectively reviewed all surgically managed tibial plateau fractures in our tertiary c

What is the appropriate axial position in cannulated screw fixation for femoral neck fractures? A finite element analysis.

BACKGROUND: Cannulated screw fixation is a common surgical treatment for femoral neck fractures; however, there is limited information on the optimal axial position of the screws. Herein, we aimed to investigate the impact of axial screw position on surgical stability in femoral neck fracture models fixed with three cannulated screws. METHODS: Eighteen finite element models (FEMs) replicating Pauwels type II femoral neck fractures were constructed and tested using nine normal and nine osteoporotic bone models. Each FEM simulated combinations of three different screw positions (anterior, centr

Design and analysis of biodegradable buttress threaded screws for fracture fixation in orthopedics: a finite element analysis.

Screws made up of non-biodegradable materials (Ti-alloy, etc.) have been used since long for temporary joining/fixation in applications involving skeleton damage or bone fracture. These screws need to be removed after complete healing as their sustained presence results in many complications, such as - micro-fracturing, stress shielding, etc. The removal of these screws is a little difficult too as it may result in the healed bone getting broken/damaged again. These problems can be overcome by employing metallic implants (plate, screws, etc.) made up of biodegradable metallic materials (Mg-all

A Simple and Reliable Method of Narrowing Genioplasty Using Biodegradable Screws.

The T-osteotomy technique is widely performed to improve lower face aesthetics. During this narrowing genioplasty procedure, metal fixtures are required to rigidly fix the bone segments. Specifically, the use of biodegradable osteofixations has become a viable surgical option as more patients now have the desire to surreptitiously undergo aesthetic surgery. The present report describes a narrowing genioplasty procedure with the use of biodegradable screws only. When the T-osteotomy technique was performed to narrow the chin, the bone segments were first temporarily fixed with a 4-hole titanium

A Novel Pedicle Screw Design with Variable Thread Geometry: Biomechanical Cadaveric Study with Finite Element Analysis.

BACKGROUND: Pedicle screw fixation provides one of the most stable spinal constructs. Their designs together with osseous characteristics have been known to influence the screw-bone interplay during surgical maneuvers and thereafter the fusion process. Various technical modifications to enhance screw performance have been suggested. This study evaluated the pull-out strength and axial stiffness of a novel pedicle screw design with variable thread geometry and pitch. METHODS: The newly designed triple threaded pedicle screw is tapered, and has unique out-turned flanges to hold the cancellous b

Pedicle Screw Fixation Under Nonaxial Loads: A Cadaveric Study.

STUDY DESIGN: An experimental study of pedicle screw fixation in human cadaveric vertebrae. OBJECTIVE: The aim of this study was to experimentally characterize pedicle screw fixation under nonaxial loading and to analyze the effect of the surgeons' screw and placement choices on the fixation risk of failure. SUMMARY OF BACKGROUND DATA: Pedicle screw fixation performance is traditionally characterized with axial pullout tests, which do not fully represent the various tridimensional loads sustained by the screws during correction maneuvers of severe spinal deformities. Previous studies have an

Qualitative Comparison of Stress Distribution of Different Fixation Techniques of Sagittal Split Ramus Osteotomy (SSRO) in Mandibular Setback Surgery in Asymmetric Cases Using Three-Dimensional Finite Element Analysis (FEA).

BACKGROUND: Bilateral sagittal split osteotomy (BSSO) is the most common procedure in orthognathic Surgey of mandible also in asymmetries. However, the methods of fixation are different. Few studies worked on asymmetric cases especially with the use of finite element analysis (FEA). We aimed to evaluate stress distribution of two different Fixation techniques in mandibular setback surgery in asymmetric cases using FEA. METHODS: A 3-dimensional model of asymmetric mandible was obtained. SSRO with modified osteotomy was simulated unilaterally and another side osteotomized as common. Then differ
