The effect of hearing loss above 2 kHz on temporal resolution and speech understanding in noise.

OBJECTIVE: This study aimed to evaluate temporal resolution, speech understanding in noise, and daily communication difficulties in individu … between groups were observed in temporal resolution tests, speech recognition thresholds (SRT), and speech intelligibility percentages in -5 dB signal to noise ratio condition of the TMT, as well as in SSQ results. Decision-tree analysis indicated that the Speech Perception subscale of the SSQ and the TMT SRT in noise procedure were considered useful for identifying HF- …

Representation Learning Based Speech Assistive System for Persons With Dysarthria.

An assistive system for persons with vocal impairment due to dysarthria converts dysarthric speech to normal speech or text. Because of the articulatory deficits, dysarthric speech recognition needs a robust learning technique. Representation learning is significant for complex tasks such as dysarthric speech recognition. We focus on robust representation for dysarthric speech recognition that involves recognizing sequential patterns of varying length utterances. We propose a hybrid framework that uses a generative learning based data representation with a discriminative learning based classif

Amantadine-Induced Craniofacial Myoclonus: Distinctive Iatrogenic Dysarthria in Parkinson's Disease.

BACKGROUND: Amantadine is a widely prescribed medication in Parkinson's disease (PD). A distinctive craniofacial distribution of myoclonus w … ll cases, reduction or discontinuation of amantadine markedly attenuated the myoclonus and restored speech intelligibility. LITERATURE REVIEW: Amantadine can induce subcortical segmental or generalized myoclonus. A report in 1996 of "vocal myoclonus" in an amantadine-treated patient with PD was the first observation of a focal distribution of myoclonus, particularly af …

Investigations on Directional Hearing With One-Sided Fitting of an Active Middle Ear Implant or Bone Conduction Hearing Implant.

OBJECTIVE: In patients with conductive or combined unilateral hearing loss, implantable hearing systems can be a treatment option. Due to the overlapping indications of hearing implants, a systematic evaluation of audiologic differences in terms of speech intelligibility and binaural hearing abilities is necessary. Because of the unilateral cochlear stimulation in patients implanted with an active middle ear implant, we expect superior binaural hearing performance compared with patients implanted with a bone-conducting implant that causes bilateral cochlear stimulation. This study focuses espe…

Protocol for the Connected Speech Transcription of Children with Speech Disorders: An Example from Childhood Apraxia of Speech.

OBJECTIVE: While it is known that connected speech has different features to single-word speech, there are currently few recommendations regarding connected speech transcription. This research therefore aimed to develop a clinically feasible protocol for connected speech transcription. The protocol was then used to assist with description of the connected speech of children with childhood apraxia of speech (CAS), as little is known about their connected speech characteristics. PARTICIPANTS AND METHODS: Following a literature review, the Connected Speech Transcription Protocol (CoST-P) was ite

Therapy with hypnosis and home video for speech disorders: A case report.

Speech difficulties in children can impair communication, social development, and academic success, often contributing to anxiety. This case report highlights a novel approach combining therapy with hypnosis, patient-recorded videos, and formative physician feedback to improve a child's speech clarity and confidence. In this report, a nine-year-old girl with longstanding articulation difficulties participated in hypnosis sessions and created cooking demonstration videos. These videos served as tools for self-assessment and for receiving targeted physician feedback on speech pacing and clarity.

Speech production skills of bilingual children using cochlear implants.

Evidence is lacking on the impact of bilingualism on the speech skills of children with cochlear implants (CIs). This study described the speech production of children with CIs acquiring French and one or more additional spoken languages. Four groups of children aged 4-11 were included: bilinguals (n = 15) and monolinguals (n = 14) with CIs and bilinguals (n = 14) and monolinguals (n = 20) with typical hearing. Data were collected about the percentage of consonant correct (PCC) and vowel correct (PVC) produced in French and intelligibility in all languages they spoke. Bilingual and monolingual

Effects of different noises on speech discrimination by the elderly.

Elderly people often complain of difficulty in speech discrimination, especially in noise environments. The effects of 4 competing noises on sentence intelligibility were evaluated: speech noise, cocktail party noise, traffic noise and continuous discourse. A comparison was made between young and old normally hearing subjects and young and old hearing-impaired subjects with similar audiograms. The old people with normal hearing had slightly reduced discrimination compared to the young during competing speech noise and cocktail noise. Discrimination in the hearing-impaired elderly was also sign

Alaryngeal Speech Enhancement for Noisy Environments Using a Pareto Denoising Gated LSTM.

Loss of the larynx significantly alters natural voice production, requiring alternative communication modalities and rehabilitation methods to restore speech intelligibility and improve the quality of life of affected individuals. This paper explores advances in alaryngeal speech enhancement to improve signal quality and reduce background noise, focusing on individuals who have undergone laryngectomy. In this study, speech samples were obtained from 23 Lithuanian males who had undergone laryngectomy with secondary implantation of the tracheoesophageal prosthesis (TEP). Pareto-optimized gated l…

Neoglottic reconstruction following total laryngectomy.

Aspiration of saliva and food and postoperative stenosis of the shunt are not infrequent among laryngectomized patients who have undergone surgery for voice rehabilitation. A new technique for neoglottic reconstruction has been developed to overcome these complications. A neoglottis was created from the upper-tracheal rings to prevent postoperative stenosis and was also made to protrude into the esophagus, in order to prevent aspiration. This procedure has been performed on eight patients, seven of whom are speaking well and have no aspiration. Their speech is superior to esophageal speech whe

Development and evaluation of a new intraoral voice assist device called the voice retriever.

OBJECTIVE: Patients lose their voice after laryngectomy for laryngeal cancer or aspiration prevention surgery for severe dysphagia. To assist such patients, we developed and verified the utility of a novel vocalization method using a device termed the voice retriever (VR), in which the sound source is placed in the mouth. METHODS: We investigated the effectiveness of the VR in patients. The VR consists of a mouthpiece with a built-in speaker and a dedicated application that serves as the sound source. We compared the speech intelligibility and naturalness in normal participants using VR and a…

Rapid exhaustion of auditory neural conduction in a prototypical mitochondrial disease, Friedreich ataxia.

OBJECTIVES: In patients with Friedreich ataxia (FRDA), mitochondrial failure leads to impaired cellular energetics. Since many FRDA patients have impaired hearing in noise, we investigated the objective consequences on standard auditory brainstem-evoked responses (ABRs). METHODS: In 37 FRDA patients, among whom 34 with abnormal standard ABRs, hearing sensitivity, speech-in-noise intelligibility and otoacoustic emissions were controlled. ABR recordings were split into four consecutive segments of the total time frame used for data collection, thus allowing the dynamics of ABR averaging to be o

Treatments for dysarthria in Parkinson's disease.

Dysarthria in Parkinson's disease can be characterised by monotony of pitch and loudness, reduced stress, variable rate, imprecise consonants, and a breathy and harsh voice. Use of levodopa to replenish dopamine concentrations in the striatum seems to improve articulation, voice quality, and pitch variation, although some studies show no change in phonatory parameters. Traditional speech therapy can lead to improvement of dysarthria, and intensive programmes have had substantial beneficial effects on vocal loudness. Unilateral surgical lesions of subcortical structures are variably effective f
