Assessment of condylar fractures associated with mandibular fossa and/or tympanic plate fractures.

Mandibular fractures are frequently diagnosed in trauma centres. Tympanic plate and mandibular fossa fractures can be associated with those fractures and can pass unnoticed if a careful evaluation and adequate imaging exam are not performed. This descriptive study was made to evaluate the incidence of condylar fractures associated with mandibular fossa and/or tympanic plate fractures in a large trauma hospital. Twenty-nine patients were diagnosed with such fractures between July/2019 and July/2020. A total of 81.5% of the patients were diagnosed with condylar fracture associated with tympanic

Iatrogenic radial nerve palsy in the surgical treatment of humerus shaft fracture -anterolateral versus posterior approach: A systematic review and meta-analysis.

BACKGROUND: Although many studies have investigated iatrogenic radial nerve palsy (RNP) in humerus shaft fracture, there is inconsistent evidence on which approach leads to iatrogenic RNP. Moreover, no meta-analysis has directly compared the anterolateral and posterior approaches regarding iatrogenic RNP. METHODS: In this systematic review and meta-analysis, the MEDLINE, Embase, and Cochrane Library databases were searched systematically for studies published before March 30, 2021. We included studies that (1) assessed the RNP in the surgical treatment of humerus shaft fracture and (2) direct

Primary total knee replacement for tibial plateau fractures in older patients: a systematic review of 197 patients.

INTRODUCTION: Tibial plateau fractures are typically treated with osteosynthesis. In older patients, osteosynthesis is associated with some complications, risk of post-traumatic osteoarthritis and long partial, or non-weight bearing during the recovery phase. To avoid these problems, primary total knee replacement (TKR) has become an increasingly common treatment option. The aim of this study was to evaluate all the relevant literature and summarize the current evidence-based knowledge on the treatment of tibial plateau fractures with primary TKR in older patients. MATERIALS AND METHODS: A sy

Stand-alone coracoclavicular suture repair achieves very good results in unstable distal clavicle fractures at a minimum follow-up of 1 year.

BACKGROUND: The treatment of unstable (Neer type IIB and V) extra-articular distal clavicle fractures remains challenging, especially when encountering a small lateral fragment which does not allow for sufficient screw purchase. It was the purpose of this study to present the clinical and radiologic outcome of a consecutive series of patients treated by a stand-alone coracoclavicular stabilization using a so-called cow-hitch technique with a suture anchor. METHODS: Nineteen patients were treated with a specific surgical technique for distal clavicle fractures (11 left, 8 right) with either ru

Migration Measurement of Pins in Postoperative Recovery of the Proximal Femur Fractures Based on 3D Point Cloud Matching.

Background and objectives: Internal fixation is one of the most effective methods for the treatment of proximal femur fractures. The migration of implants after the operation can seriously affect the reduction of treatment and even cause complications. Traditional diagnosis methods can not directly measure the extent of displacement. Methods: Based on the analysis of Hansson pins, this paper proposes a measurement method based on three-dimensional matching, which uses computerized tomography (CT) images of different periods of patients after the operation to analyze the implants' migration in

Patient Pain and Function After Correction of Posterior Malleolar Malunion.

BACKGROUND: The management of an ankle malunion involving the posterior malleolus remains challenging, and only a few published studies described the operative treatment of complex posterior malleolar malunion. A transfibular approach allows for direct visualization of the articular reduction of the posterior malleolus, but the reports of its use for correcting posterior malleolar malunion are rare. This study aims to evaluate the results of the intra-articular osteotomy via a transfibular approach for correcting an ankle malunion involving the posterior malleolus. METHODS: We conducted a ret

Minimally Invasive Intramedullary Screw Versus Plate Fixation for Proximal Phalanx Fractures: A Biomechanical Study.

PURPOSE: To compare the maximum interfragmentary displacement of short oblique proximal phalanx (P1) fractures fixed with an intramedullary headless compression screw (IMHCS) versus a plate-and-screws construct in a cadaveric model that generates finger motion via the flexor and extensor tendons of the fingers. METHODS: We created a 30° oblique cut in 24 P1s of the index, middle, ring, and little fingers for 3 matched pairs of cadaveric hands. Twelve fractures were stabilized with an IMHCS using an antegrade, dorsal articular margin technique at the P1 base. The 12 matched-pair P1 fractures w

Medial malleolus malunion with secondary tibiotalar and syndesmotic incongruence - two case reports and ''box resection'' for its treatment.

INTRODUCTION: Ankle fractures are among the most common injuries treated by orthopaedic surgeons. Malunion is one of the possible complications, and corrective surgery is often used to avoid long-term disability. However, few studies address medial malleolus malunion and the best revision surgery technique is controversial. We describe a new surgical technique, the "box resection". MATERIAL AND METHODS: We present two cases of medial malleolus malunion with secondary talus lateral translation and syndesmotic incongruence. In order to restore a congruent tibiotalar joint, we performed the desc

Determination of the insertion position of implants for the cross-pin fixation of distal femoral physis fractures.

The cross-pin technique for the treatment of distal femoral physis fractures (specifically, Salter-Harris Type I fractures) was investigated using femurs collected from beagle cadavers. The pin was inserted from the medial surface of the femur at an inclination of approximately 30 to 45° relative to the long axis of the femur in the anteroposterior direction; the pin exit was set proximal to the origin of the long digital extensor tendon. Digital and radiographic images of the femur in the anteroposterior and lateral directions were obtained. In both types of images, the insertion angle of the

Ipsilateral Displaced Physeal Fractures of the Proximal and Distal Humerus with Subperiosteal Extrusion in a 6-Year-Old Child: A Case Report.

CASE: We present a case of a 6-year-old male child with ipsilateral proximal and distal physeal separation of humerus with subperiosteal extrusion after manipulation by a bonesetter. Treatment included open reduction and internal fixation with Kirschner wire proximally and distally. At 1.5-year follow-up, the child had a shortening of 0.5 cm, no pain, and/or no instability. CONCLUSION: Manipulation by bonesetters, a commonplace in developing countries, can produce unpredictable complications and compromise outcomes. Although accepted in certain settings, the regional orthopaedic surgeons shou

An Anatomical Landmark for Retromandibular Transparotid Approach to Treat Subcondylar Fractures.

Although the retromandibular transparotid approach (RTA) has been widely used to treat mandibular subcondyle fractures, the transient facial nerve injury (FNI) caused by this approach is not uncommon. The purpose of this paper was to present an anatomical landmark for RTA to treat subcondylar fractures, aiming to minimize FNI caused by the surgery. A total of 25 patients with subcondylar fractures but no FNI underwent RTA surgery. Intraoperatively, the plane of angulus oris was utilized as a reference to open the parotid and masseter tissues for exposing the fracture site. Postoperatively, FNI

Biomechanical optimization of the far cortical locking technique for early healing of distal femur fractures.

This finite element study optimized far cortical locking (FCL) technology for early callus formation in distal femur fracture fixation with a 9-hole plate using FCL screws proximal to, and standard locking screws distal to, the fracture. Analyses were done for 120 possible FCL screw configurations by varying FCL screw distribution and number. A hip joint force of 700 N (i.e. 100% x body weight) was used, which corresponds to a typical 140 N "toe-touch" foot-to-ground force (i.e. 20% x body weight) suggested to patients immediately after surgery. Increased FCL screw distribution (i.e. shorter p

Homemade 3D mirror imaging models utility for surgical treatment of complex fractures of scapula.

The treatment of complex articular scapular fractures requires obtaining an anatomic reduction. The experience of the surgeon and the surgical skills needed are still the most important aspect in this surgical field. 3D printed models at a 1:1 scale provides tactile and visual experience. We used a specular healthy scapula (reverse engineering) as a reliable template for preoperative planning. We pre-contour plates directly over a printed mold, for direct application, without further corrections during the surgical procedure. This reduces improvisation during surgery, the risks of complication

Unsuccessful Intervention of CT-Assisted Osteosynthesis for Stress Fractures of the Hook of the Hamate: A Report of 3 Cases.

CASE: We performed computed tomography (CT)-assisted dorsal approach osteosynthesis for stress fractures of the hook of the hamate using the dorsal approach in 3 high school baseball players in the hybrid operating room. Bony union was observed in all patients on CT. All patients were able to play baseball without pain for at least 6 months after surgery. However, refractures were observed in all patients at a mean 9.7 months after surgery. CONCLUSION: The indications of osteosynthesis for stress fractures of the hook of the hamate in baseball players should be carefully considered.

Locking compression plate fixation of critical-sized bone defects in sheep. Development of a model for veterinary bone tissue engineering.

PURPOSE: To develop a segmental tibial bone defect model for tissue engineering studies in veterinary orthopedics using single locking compression plate (LCP) fixation and cast immobilization. METHODS: A 3-cm bone defect was created in the right tibia of 18 adult Suffolk sheep. A 10-hole, 4.5-mm LCP was applied to the dorsomedial aspect of the bone. Four locking screws were inserted into the proximal and three into the distal bone fragment. Operated limbs were immobilized with casts. Animals were submitted to stall rest, but were allowed to bear weight on the operated limb. During the recover

Talar Neck and Sustentaculum Fracture Fixation.

Talar neck fractures are uncommon fractures that result from high-energy trauma causing bony and soft tissue injury. When combined with sustentaculum fractures, care must be taken to plan the approach and fracture fixation strategy. We present a 57-year-old woman who experienced a motor vehicle accident, sustaining a Hawkins II talar neck fracture with associated sustentaculum tali fracture. We demonstrate the dual-incision approach to the talar neck with medial screw and lateral plate fixation. The sustentaculum fixation was accomplished with cannulated screws through a direct medial approach

Treatment Of Vertical Fracture Of Distal Radius Using Interfragmentary Screws Through Direct Lateral Approach To Distal Radius.

Distal radius fractures are very common injuries. Majority of these fractures are transverse or oblique in nature. Isolated vertical fractures of the distal radius in the sagittal plane are rare injuries. The management of these vertical fractures is therefore different from the usual transverse or oblique fractures of the distal radius. We report on a 53-year-old right-handed gentleman who fell from his bicycle and sustained a vertical fracture of the distal radius. His distal radius fracture was fixed surgically using two interfragmentary screws. A direct lateral approach of the distal radiu
