Role of thrombolytic therapy in axillary-subclavian vein thrombosis.

Axillary-subclavian vein thrombosis in young patients has produced long-term disability because of failure of the thrombosed vein to recanalize. In a review of 10 patients with axillary-subclavian vein thrombosis treated in our institution, four were effort vein thrombosis. All patients were diagnosed by venography. One patient received urokinase with complete resolution of symptoms and complete dissolution of the clot after two days of initial therapy, which was confirmed by venography and duplex imaging. Another patient received streptokinase with partial resolution of symptoms and dissoluti

Low-dose streptokinase thrombolysis of axillary-subclavian vein thrombosis.

Nine patients with 10 episodes of acute axillary-subclavian vein thrombosis were treated with local low-dose streptokinase infusion. All patients except one were heparinized during the procedure with conversion to oral anticoagulation upon completion of the infusion. Recanalization was achieved in 7 patients (70%). Thrombosis recurred in 1, and in 2 patients the procedure was unsuccessful. No major complications were encountered. We concluded that local low-dose streptokinase infusion is a safe, simple, and effective method for treating acute axillary-subclavian vein thrombosis.

Pulmonary embolus after treatment for subclavian-axillary vein thrombosis.

Treatment of effort-induced subclavian-axillary vein thrombosis of the upper extremity is aimed at elimination of the chronic symptoms of the postphlebitic syndrome. Various treatments have been tried, with varied success. Recently, fibrinolytic therapy with streptokinase (Abbokinase) or urokinase (Kabikinase, Streptase) has gained popularity as a treatment option. In the case presented here, complete lysis of effort-induced subclavian-axillary vein thrombosis was achieved with the use of catheter-directed infusion of streptokinase. Subsequently, a pulmonary embolus developed, causing marked m

[Post-thrombotic axillary vein syndrome (Paget-von Schroetter syndrome)].

In acute thrombosis of the subclavian and axillar vein--known in the literature as Paget-von-Schroetter syndrome--skin can also be involved if no treatment is provided. The skin lesions resemble stasis dermatitis seen in chronic venous insufficiency of the lower extremity. The specific role of intima lesions of the subclavian vein caused by microtraumas is discussed because the Paget-von-Schroetter-syndrome is mainly seen in physically hard working men.

Axillary-subclavian vein occlusion in patients with lung neoplasms.

Twelve patients with pulmonary neoplasms treated at the Fox Chase Cancer Center were found to have a syndrome of axillary-subclavian vein occlusion. Ten patients had non-small-cell lung carcinoma, one had small cell carcinoma, and one had mesothelioma. In eight patients, this vascular syndrome developed as a consequence of progressive or metastatic recurrent disease, while in four patients it presaged the appearance of the initial malignant disease. Axillary-subclavian vein occlusion should be easily differentiated from the superior vena cava syndrome. Treatment with anticoagulation therapy in

Ultrasound-guided infraclavicular axillary vein cannulation: a useful alternative to the internal jugular vein.

BACKGROUND: Ultrasound (US) guidance reduces complications and increases accuracy during internal jugular vein (IJV) cannulation. The subclavian vein (SCV) is popular but is less amenable to US guidance. The axillary vein (AxV), a direct continuation of the SCV, is an alternative, but to date, experience with US is limited to small case series. METHODS: Retrospective procedural data were collected on 2586 sequential patients referred for insertion of tunnelled central venous access at a UK tertiary centre from 2004 to 2011. RESULTS: A total of 99.8% of patients tolerated the procedure with l

The New Surface Landmarks for Blind Axillary Vein Puncture.

OBJECTIVE: To compare the efficacy of blind axillary vein puncture utilizing the new surface landmarks for the subclavian method. METHODS: This prospective and randomized study was performed at two cardiology medical centers in East China. Five hundred thirty-eight patients indicated to undergo left-sided pacemaker or implantable cardioverter defibrillator implantation were enrolled, 272 patients under the axillary access and 266 patients under the subclavian approach. A new superficial landmark was used for the axillary venous approach, whereas conventional landmarks were used for the subcla

Axillary vein spasm during cardiac implantable electronic device implantation.

BACKGROUND: The technique of axillary vein (AV) or subclavian vein (SV) puncture has become an important alternative to cephalic vein (CV) cutdown as an approach allowing cardiac lead introduction into the venous system during cardiac implantable electronic device (CIED) implantation procedures. Irrespective of the technique used, the injury associated with lead insertion may induce a reflex venous spasm that can even cause total venous obstruction. In order to assess the incidence of AV spasm during AV puncture, we analysed a total of 735 (382 in females and 353 in males; mean age 75 ± 11 yea

Jugular-axillary vein bypass for salvage of arteriovenous access.

Stenosis or occlusion of the subclavian vein can cause incapacitating upper extremity swelling and venous hypertension in the patient with an arteriovenous (AV) access. A case of subclavian vein occlusion is reported that was treated with internal jugular-axillary vein bypass. This procedure resulted in salvage of the access and rapid resolution of the associated upper extremity swelling. It was concluded that jugular-axillary vein bypass should be considered in patients who have massive upper extremity edema resulting from a functioning AV access and ipsilateral subclavian vein occlusion. Diagnostic and therapeutic approach to axillary-subclavian vein thrombosis.

We report the cases of four patients who complained of post-exertional shoulder and/or arm discomfort, and who were diagnosed with acute or possible impending axillary-subclavian vein thrombosis. One regained full patency of a stenotic and obstructed vein after local streptokinase infusion, first rib surgical resection, and transvenous angioplasty. A second with a patent but narrowed and tented vein was treated with heat and elevation, and was referred for possible surgical correction of thoracic outlet syndrome. The third patient, who presented two weeks after the thrombotic event, experience

Decompression of Axillary Vein: An Essential Adjunct for Advanced Lymphedema.

BACKGROUND: In advanced lymphedema, lymphovenous anastomosis (LVA) can be a solution based on using the pressure gradient between the high-pressure lymphatics and the low-pressure veins. If the vein pressure is high, the effect of surgery will be less optimal. This study evaluated the effect of axillary vein perivascular scar release on LVA. METHODS: This is a retrospective study of 40 upper limb stage 2 and 3 lymphedema patients divided into 2 groups with an average follow-up of 33 months (minimum of at least 12 months): the scar release group ( n = 25) and the control group ( n = 15). All p

Arteriovenous Hybrid Graft with Outflow in the Proximal Axillary Vein.

BACKGROUND: The patency of long-term hemodialysis access in end-stage renal disease patients remains a significant challenge. Often these patients are affected with limited venous outflow options, requiring limb abandonment, and creation of new access in the contralateral arm. Vascular surgeons are familiar with the exposure of the proximal axillary artery via an infraclavicular incision. The axillary vein is easily exposed through this technique. The use of the hybrid Gore graft can make the venous anastomosis easier. A hybrid graft with its venous outflow placed in the proximal axillary vein

Axillary vein perforation by the medial cutaneous nerve of the forearm.

Variation in the venous pattern in the arm is common. In this study, a rare variant of the axillary vein and its association with the median cutaneous nerve of the forearm is described. In the axilla, the medial cutaneous nerve of the forearm penetrated the axillary vein, thereby creating two narrow venous channels at the site of passage. Such variations are important because a large number of diagnostic and therapeutic invasive procedures are carried out on veins. A possible mode of origin and the clinical importance of this variation are discussed.
