Differentiating atrial tachycardias with centrifugal activation: Lessons from high-resolution mapping.

BACKGROUND: Centrifugal activation is not always the origin of a focal atrial tachycardia (AT) ("true-focal"), but passive activation from the other structures ("pseudo-focal"). OBJECTIVE: We aimed to establish a method to differentiate true-focal from pseudo-focal. METHODS: In 49 centrifugal activations in 35 patients with AT, 12-lead electrocardiogram, activation map, atrial global activation histogram (GAH), and local electrograms were analyzed. GAH demonstrates the relation between the activation area and timing through the cycle length, displayed with a normalized value, ranging from 0

Inhibition of Small-Conductance Calcium-Activated Potassium Current (I K,Ca) Leads to Differential Atrial Electrophysiological Effects in a Horse Model of Persistent Atrial Fibrillation.

BACKGROUND: Small-conductance Ca2+-activated K+ (KCa2) channels have been proposed as a possible atrial-selective target to pharmacologically terminate atrial fibrillation (AF) and to maintain sinus rhythm. However, it has been hypothesized that the importance of the KCa2 current-and thereby the efficacy of small-conductance Ca2+-activated K+ current (I K,Ca) inhibition-might be negatively related to AF duration and the extent of AF-induced remodeling. EXPERIMENTAL APPROACH AND METHODS: To address the hypothesis of the efficacy of I K,Ca inhibition being dependent on AF duration, the anti-arr

Adenosine sensitive left ventricular summit ventricular tachycardia in a pre-adolescent: case report.

BACKGROUND: Sustained forms of tachycardia especially from the left ventricular summit are rare. Adenosine sensitive outflow tachycardias, especially from the left ventricular summit, are rarer still. These arrhythmias may be exercise or stress induced as they are facilitated by catecholamines and characteristically terminate with adenosine, vagal manoeuvres, and beta-blockers. The surface 12-lead electrocardiogram can be used to localize the anatomic site of origin before catheter ablation; however, prediction of the precise origin may still be challenging due to the intimate and complex anat

Atrial slow conduction develops and dynamically expands during premature stimulation in an animal model of persistent atrial fibrillation.

Slow conduction areas and conduction block in the atria are considered pro-arrhythmic conditions. Studies examining the size and distribution of slow conduction regions in the context of persistent atrial fibrillation (AF) may help to develop improved therapeutic strategies for patients with AF. In this work, we studied the differences of size and number in slow conduction areas between control and persistent AF goats and the influence of propagation direction on the development of these pathological conduction areas. Epicardial atrial electrical activations from the left atrial roof were opti

A combination of two simultaneous tachycardias in the right atrium close to the atrio-ventricular node and within the coronary sinus in a post-operative cor triatriatum patient.

A 71-year-old male was referred to another hospital for dizziness. A bradycardia -tachycardia syndrome and Cor triatriatum were detected, and an operation to resect the membrane in the left atrium and implant a pacemaker epicardially was performed. However, no suitable site could be found on either atria and therefore, a single chamber ventricular pacemaker was implanted. In the electrophysiological study performed in our hospital, we could not detect any atrial potentials in either atria, excluding the region close to the His bundle (HB) and within coronary sinus (CS), in spite of extensive c

Multiscale Coronary Arterial Network Generation and Hemodynamics Using Patient-Specific Fractional Myocardial Blood Volume.

Ischemic heart disease (IHD) is the leading cause of death worldwide. Although 90% of the intramyocardial blood volume resides in the microvasculature, clinical imaging methods cannot visualize the microvascular coronary network in vivo, and non-invasive hemodynamic estimates overlook patient-specific microcirculatory contributions. Herein, we present a multiscale framework to extend the epicardial coronary tree and generate 1D microvascular networks in the myocardium based on ferumoxytol-enhanced magnetic resonance coronary imaging and fractional myocardial blood volume (fMBV) maps. Synthetic

Catheter ablation of atrial flutter in an adult with a univentricular heart, common atrium, and single atrioventricular valve: a case report-'complex things don't always require a complex solution'.

BACKGROUND: The 'univentricular' heart encompasses a variety of congenital cardiac defects characterized by a single functional ventricle and an underdeveloped ventricular chamber. Surgical intervention, typically in infancy or childhood, aims to regulate pulmonary blood flow volume. In adulthood, untreated patients may experience limitations in physical activity and elevated morbidity due to persistent cyanosis and arrhythmias, notably after the Fontan procedure. CASE SUMMARY: A 38-year-old Mexican man with an unrepaired morphologically right single ventricle and a common atrium presented wi

Differential effects of mutations of POPDC proteins on heteromeric interaction and membrane trafficking.

The Popeye domain containing (POPDC) genes encode sarcolemma-localized cAMP effector proteins. Mutations in blood vessel epicardial substance (BVES) also known as POPDC1 and POPDC2 have been associated with limb-girdle muscular dystrophy and cardiac arrhythmia. Muscle biopsies of affected patients display impaired membrane trafficking of both POPDC isoforms. Biopsy material of patients carrying mutations in BVES were immunostained with POPDC antibodies. The interaction of POPDC proteins was investigated by co-precipitation, proximity ligation, bioluminescence resonance energy transfer and bimo

Novel 4DCT Method to Measure Regional Left Ventricular Endocardial Shortening Before and After Transcatheter Mitral Valve Implantation.

BACKGROUND: Regional left ventricular (LV) mechanics in mitral regurgitation (MR) patients, and local changes in function after transcatheter mitral valve implantation (TMVI) have yet to be evaluated. Herein, we introduce a method for creating high resolution maps of endocardial function from 4DCT images, leading to detailed characterization of changes in local LV function. These changes are particularly interesting when evaluating the effect of the Tendyne™ TMVI device in the region of the epicardial pad. METHODS: Regional endocardial shortening from CT (RSCT) was evaluated in Tendyne (Abbot

Safety and Chronic Atrial Lesion Formation With a Large-Tip, Contact-Force-Sensing Multipolar Pulsed Field Ablation Catheter: Effect of Application Number in a Porcine Beating Heart Model.

BACKGROUND: Integration of catheter-tissue contact force with pulsed field ablation (PFA) dosing is essential for achieving safe and durable lesion formation with contact force sensing catheters in ventricular models. However, data on the use of these catheters for atrial ablation remain limited. This study evaluated the procedural safety and 30-day lesion characteristics of atrial PFA delivered with the OMNYPULSE ablation catheter in a porcine model. METHODS: Twelve pigs were randomized to receive either 6 (×6 or Group A) or 12 (×12 or group B) PFA applications per ablation. Following 3-dime

PW1+ cells give rise to cardiac adipocytes during development and myofibroblasts following injury.

BACKGROUND: Pw1, a maternally imprinted gene, is expressed in various stem cell populations, underscoring its crucial roles in tissue development, maintenance, and regeneration. While our recent work has revealed its regulatory function in cardiac fibrosis following ischemic injury, whether PW1+ cells exhibit stem cell properties in the heart remains unclear. METHODS: We utilized genetic lineage tracing with Pw1pCreER/m+;R26-tdT and Pw1p2A-CreER/m+;R26-tdT mouse models. The adipogenic and fibrogenic potential of PW1+ cells was assessed in vitro using sorted tdTomato+ cells subjected to differ

Prompt Identification of the Location of Gap Conduction in the Mitral Isthmus Following Vein of Marshall Ethanol Infusion and Endocardial Ablation.

BACKGROUND: Mitral isthmus (MI) gap conduction is common despite ethanol infusion into the vein of Marshall (EI-VOM) and endocardial ablation of the MI. This study aimed to investigate the characteristics of electrograms of the distal coronary sinus (CSd) to guide the identification of the gap location in the MI. METHODS: A total of 187 patients who underwent EI-VOM and MI ablation were included in the study. After routine completion of EI-VOM and endocardial MI ablation, the characteristics of the electrogram in the CSd during left atrial appendage pacing were analyzed in unblocked MI conduc

Sacubitril/Valsartan Ameliorates Crizotinib-Induced Cardiotoxicity in Mice.

BackgroundLung cancer is one of the major cause of death globally. Crizotinib is a first-line drug used in treating non-small-cell lung cancer (NSCLC). However, the pathophysiological mechanisms underlying its cardiotoxicity are unknown. This study investigated the mechanisms of crizotinib-induced cardiotoxicity and explored whether this toxicity can be prevented by the angiotensin receptor/neprilysin inhibitor sacubitril/valsartan.MethodsMale C57BL/6 mice were randomly divided into three groups: control, crizotinib (40 mg ⋅ kg - 1 ⋅ d - 1 for four weeks), and crizotinib + sacubitril/valsartan

Thermal modulation of epicardial Ca2+ dynamics uncovers molecular mechanisms of Ca2+ alternans.

Ca2+ alternans (Ca-Alts) are alternating beat-to-beat changes in the amplitude of Ca2+ transients that frequently occur during tachycardia, ischemia, or hypothermia that can lead to sudden cardiac death. Ca-Alts appear to result from a variation in the amount of Ca2+ released from the sarcoplasmic reticulum (SR) between two consecutive heartbeats. This variable Ca2+ release has been attributed to the alternation of the action potential duration, delay in the recovery from inactivation of RYR Ca2+ release channel (RYR2), or an incomplete Ca2+ refilling of the SR. In all three cases, the RYR2 mo

Analysis of local ventricular repolarization using unipolar recordings in patients with arrhythmogenic right ventricular cardiomyopathy.

PURPOSE: In arrhythmogenic right ventricular cardiomyopathy (ARVC), abnormal electroanatomic mapping (EAM) areas are proportional to extent of T-wave inversion on 12-lead ECG. We aimed to evaluate local repolarization changes and their relationship to EAM substrate in ARVC. METHODS: Using unipolar recordings, we analyzed the proportion of negative T waves ≥ 1 mV in depth (NegT), NegT area, Q-Tpeak (QTP), Tpeak-Tend (TPE) intervals and their relationship to bipolar (< 1.5 mV ENDO, < 1.0 mV EPI) and unipolar (< 5.5 mV) endocardial (ENDO) and epicardial (EPI) low-voltage area (LVA) in 21 pts. ECG Imaging to Detect the Site of Ventricular Ischemia Using Torso Electrodes: A Computational Study.

Electrocardiography provides some information useful for ischemic diagnosis. However, more recently there has been substantial growth in the area of ECG imaging, which by solving the inverse problem of electrocardiography aims to produce high-resolution mapping of the electrical and magnetic dynamics of the heart. Most inverse studies use the full resolution of the body surface potential (BSP) to reconstruct the epicardial potentials, however using a limited number of torso electrodes to interpolate the BSP is more clinically relevant and has an important effect on the reconstruction which mus

Effects of JTV-519 on stretch-induced manifestations of mechanoelectric feedback.

JTV-519 is a 1,4-benzothiazepine derivative with multichannel effects that inhibits Ca2+ release from the sarcoplasmic reticulum and stabilizes the closed state of the ryanodine receptor, preventing myocardial damage and the induction of arrhythmias during Ca2+ overload. Mechanical stretch increases cellular Na+ inflow, activates the reverse mode of the Na+ /Ca2+ exchanger, and modifies Ca2+ handling and myocardial electrophysiology, favoring arrhythmogenesis. This study aims to determine whether JTV-519 modifies the stretch-induced manifestations of mechanoelectric feedback. The ventricular f
