Development of IDDM after donating kidney to diabetic sibling.

The goal of this study was to describe a patient who developed insulin-dependent diabetes mellitus (IDDM) after donating a kidney to his sibling and to suggest a possible solution to prevent such an occurrence. A 42-yr-old man was found to have islet cell autoantibodies (ICAs) as part of a screening program of first-degree relatives with IDDM. Two years previously, he had donated his kidney to his HLA-identical sibling with long-standing IDDM. Both oral and intravenous glucose tolerance tests demonstrated a gradual loss of insulin secretion and increasing glucose intolerance until the patient

Clinical features of illness in siblings with schizophrenia or schizoaffective disorder.

Evidence implicating genetic or prenatal-perinatal environmental causes in the familial aggregation of schizophrenia led us to study 53 sets of siblings, two or more of whom had chronic psychosis, either schizophrenia or schizoaffective disorder. We looked for similarities in clinical features and concordance of diagnosis within sibships to test for shared familial causes. Clinical variables, including diagnosis, specific symptoms, age at onset, and nongenetic perinatal factors, were studied. Auditory hallucinations, paranoid delusions, thought disorder, negative symptoms, and poor premorbid s…

Reprint: The Sibling Sexual Behaviour Mapping Tool (SSBMT): Supporting practitioner confidence, planning and competency when responding to sexual behaviours between siblings.

OBJECTIVES: This paper outlines the development and initial pilot of the Sibling Sexual Behaviour Mapping Tool (SSBMT). Building on the findings of the National Project on Sibling Sexual Abuse (King-Hill, McCartan, et al., 2023) and work by Yates and Allardyce (2023), the tool was devised with the aim of supporting frontline child-protection social workers during the initial stages of working with families where sibling sexual behaviours (SSB) may be present. METHODS: This paper explores the rationale for the SSBMT design, and preliminary findings from the first phase of resource evaluation.

Antenatal and Neonatal Management of Siblings With Carbonic Anhydrase VA Deficiency.

Carbonic anhydrase VA (CAVA) deficiency (OMIM 114761) is an ultra-rare inborn error of metabolism with fewer than 20 cases described. Affect … measurement of acid-base status and ammonia levels. In contrast to their older brother, these male siblings had unremarkable neonatal periods with no significant clinical or biochemical concerns, demonstrating that in a neonate known to be affected with CAVA deficiency, early intervention can be instituted to minimise the risk of metabolic decompensation …

Homozygous 896delT (c.764del) in Somali-American Siblings With Cystic Fibrosis.

Cystic fibrosis is an autosomal recessive condition historically reported to be predominantly in those of European ancestry; however, more cases of CF are reported in those with non-European ancestry as we improve diagnostic awareness and aptitude. We report two cases of Somali siblings with rare CFTR 896delT (c.764del; p.Ile255ThrfsX6; dbSNP rs2485020167; ClinVar ID 2682475; NM_000492.4:c.764del) homozygous variants who manifested classic CF lung and hepatic disease. They presented with elevated immunoreactive trypsinogen on newborn screen but required exon sequencing to confirm their rare va…

Resilience among bereaved siblings of homicide and traffic accidents in Japan.

Background: Siblings bereaved by homicide or accidents are often referred to as the forgotten bereaved because they face difficulties in accessing adequate support. Globally, research focusing directly on siblings' perspectives remains scarce, and studies specifically addressing siblings' resilience are almost absent in Japan. A deeper understanding of siblings' experiences is required to develop support systems that acknowledge their emotions and strengthen their resilience. Objective: This study aimed to clarify the psychological transformation processes of bereaved siblings in Japan, focusin…

Supporting siblings of children with chronic medical conditions: a systematic review.

OBJECTIVE: Siblings of youth with chronic medical conditions (CMCs) experience a variety of family changes and are at risk for psychosocial problems. To support siblings, many medical specialties agree that evidence-based, sibling-focused interventions could be beneficial. This systematic review sought to understand the current evidence of psychosocial interventions for siblings of youth with any CMC. METHODS: PsycINFO, PubMed, Cumulative Index to Nursing and Allied Health Literature, Web of Science, ERIC, and ProQuest Dissertations & Theses Global were searched for studies that assessed psyc…

A rare cause of kidney disease in two siblings: transaldolase deficiency.

Transaldolase is a key enzyme in the pentose phosphate pathway, an alternative pathway for glucose metabolism. Transaldolase deficiency leads to the accumulation of metabolites such as polyols and seven-carbon sugars, particularly in urine. Transaldolase deficiency is a rare disorder with multisystem involvement, including liver dysfunction, hepatosplenomegaly, anemia, thrombocytopenia, endocrine abnormalities and dysmorphic features. We now report on two siblings with kidney involvement due to transaldolase deficiency. Kidney dysfunction is common in transaldolase deficiency and may progress …

Unusual Disease-Progression in Two Siblings With Xeroderma Pigmentosum Group G.

Protein truncation mutations in the gene for XPG nuclease cause a very severe clinical phenotype. Two siblings have splicing mutations, which result in in-frame deletions and a less severe phenotype.

"His brain works in a different way": siblings' understanding of autism.

Siblings of autistic children are at increased risk of mental health problems. Lack of autism knowledge may contribute to this risk. We examined siblings' autism knowledge using the Sibling Knowledge Interview (SKI) conducted by clinicians before a sibling intervention. The sample comprised 28 siblings (61% boys; 39% girls) aged 8 to 12 years. All had a brother or sister with a clinically confirmed autism diagnosis. Audiotaped recordings of the interviews were transcribed and analyzed thematically. We identified six main themes: (1) definition, including diagnostic label, localization, explana…

A Rare Cause of Acute Pancreatitis: Two Siblings With Werner Syndrome.

Lipodystrophies are rare disorders characterized by loss of body fat resulting in leptin deficiency. Patients are predisposed to metabolic complications such as severe insulin resistance, hypertriglyceridemia, and hepatic steatosis. Werner syndrome (WS) is among the progeroid syndromes in the classification of lipodystrophy. In this case report, we describe two siblings. In the first case, lipodystrophy was suspected when the patient presented with acute pancreatitis and hypertriglyceridemia, and a diagnosis of WS was confirmed. Subsequently, genetic screening of the patient's sister, who had …

New Phenotype in Two Siblings with Familial FLVCR1 Mutation: Neurotrophic Keratopathy.

The feline leukemia virus subgroup C receptor (FLVCR1) gene plays a role in heme, choline, and ethanolamine transport. In biallelic pathogenic FLVCR1 variants, macrocytic anemia may be associated with childhood- or adult-onset neurodegeneration of the retina, spinal cord, and peripheral nervous system. In patients with FLVCR1 variants, optic atrophy and retinitis pigmentosa are previously described ocular findings, but neurotrophic keratopathy has not been reported. In this study, we describe two patients with homozygous novel likely pathogenic variants in terms of their clinical findings, inc
