BACKGROUND: Sleep supports neurophysiological maturation in preterm infants, yet the impact of developmental care interventions on sleep remains poorly quantified. Skin-to-skin contact (SSC) improves physiological stability, but its effects on sleep structure measured with automated methods are unclear. We hypothesized that SSC increases sleep duration and reduces sleep fragmentation.
METHODS: In this prospective observational study (September 2024-May 2025), clinically stable preterm infants born at 29 + 0 to 33 + 6 weeks' gestation were enrolled in a level III Neonatal Intensive Care Unit.

BACKGROUND: Seizure detection is challenging outside the clinical environment due to the lack of comfortable, reliable, and practical long-term neurophysiological monitoring devices. We developed a novel, discreet, unobstructive in-ear sensing system that enables long-term electroencephalography (EEG) recording. This is the first study we are aware of that systematically compares the seizure detection utility of in-ear EEG with that of simultaneously recorded intracranial EEG. In addition, we present a similar comparison between simultaneously recorded in-ear EEG and scalp EEG.
METHODS: In th…

Performance monitoring (PM) has been suggested as a promising cognitive factor to explain individual differences in the association between early maltreatment and depression, yet evidence remains limited. From a monitoring vs. executive perspective, this study thus comprehensively examined whether and how the neurophysiological (ERN and its time-frequency decomposition components) and behavioral (posterror accuracy) indicators of PM interacted with childhood maltreatment on young adult depressive symptoms. A total of 192 Chinese university students (Mage = 21.08 ± 1.91 years; 59.4% girls) comp

OBJECTIVE: To evaluate the reliability of recording cortical somatosensory evoked potentials (SEPs) in asphyxiated newborns using the 4-electrode setup applied in routine long-term amplitude-integrated EEG (aEEG) brain monitoring and to assess the number of averages needed for reliably detecting the cortical responses.
METHODS: We evaluated median nerve SEPs in 50 asphyxiated full-term newborns. The SEP interpretation (present or absent) from the original recordings with 21-electrodes and approximately 600 trials served as the reference. This was compared to SEP classification (absent, presen

Conventional monitoring relies on autonomic reflexes but fails to directly measure cortical hypnosis, creating a critical information gap regarding true brain state. The objective of this article was to evaluate the cardiorespiratory and cortical responses using frontal EEG and parasympathetic tone activity (PTA) in dogs under isoflurane anesthesia and distinct analgesic protocols. Twenty-one adult dogs (n = 7 per group) undergoing tibial plateau leveling osteotomies were assessed at 12 procedural time points using heart rate, blood pressure, Patient State Index (PSI), suppression ratio (SR),

The Chiari malformation (CM) is a syndrome embodied in heterogeneous groups of malformations, spanning from the more benign and known, the CM1, to more complex syndromes such as the rare association with the tethered cord, as spinal lipomas, and the CM2, associated to open spina bifida. The clinical picture may be well expressed and detected at birth or even during intrauterine life, as for CM2, but in the other cases they may run a rather subtle clinical course. The diagnosis of these syndromes is driven by clinical examination and MRI, and it usually requires a multidisciplinary approach in

A woman in her 20s presented with progressive weakness of her left arm and leg, slurred speech and swallowing difficulties. The clinical presentation and neurophysiological tests were consistent with motor neuron disease. She was referred to the regional ventilation unit for respiratory muscle function testing. This confirmed restrictive spirometry and borderline sniff nasal inspiratory pressure and cough peak expiratory flow. Three years later, she presented with an unplanned pregnancy and expressed the wish to continue the pregnancy to term. She was monitored throughout pregnancy with interv

BACKGROUND DATA: Although the use of Cotrel-Dubousset (CD) instrumentation has improved results of operative treatment of scoliotic deformities, this technique may be associated with increased risk of neurologic injury.
CASE HISTORIES: Two cases of neurologic complications immediately after insertion of lumbar laminar hooks during CD instrumentation for correction of scoliosis are reported.
METHODS: Between 1986 and 1992, the authors performed 220 CD instrumentations for various spinal deformities. All patients were monitored with posterior tibial nerve somatosensory cortical evoked potentia

OBJECTIVE: Severe scoliosis, defined by a Cobb angle exceeding 80°, requires surgical correction and is associated with a high risk of postoperative neurological complications. These complications may be reduced by halo gravity traction (HGT) and neuromonitoring. This study evaluated the outcomes of preoperative and intraoperative multimodal neuromonitoring by somatosensory evoked potential (SSEP) and motor evoked potential (MEP) in patients undergoing HGT before and during spinal surgery for severe idiopathic scoliosis. The secondary objective was to investigate any changes in SSEP associated

OBJECTIVE: Complex motor responses (CMRs) can be recorded from lower cranial nerves after laryngeal stimulation. Long latency responses (LLRs) are observed in the soft palate and tongue. This study aimed to describe these responses, evaluate their intraoperative reproducibility, and assess correlations between signal loss and postoperative outcomes.
METHODS: The laryngeal adductor reflex (LAR) was elicited while LLRs were recorded bilaterally from the soft palate and tongue. Permanent signal loss was defined as an amplitude reduction greater than 50% affecting two LLRs. Postoperative outcomes

PURPOSE: The linked quadripolar montage for transcranial electric motor-evoked potentials (qTceMEP) consists of four stimulation electrodes: two linked anodes and two linked cathodes. This montage has grown in popularity because it can result in a larger compound muscle action potential amplitude compared with the conventional bipolar montage. Despite the increasing number of centers adopting qTceMEP, no studies have investigated patient safety to date. The aim of this study is to evaluate the safety profile of qTceMEP.
METHODS: A total of 3,806 spine surgeries performed at a single instituti

INTRODUCTION: Postoperative delirium is common and associated with poor postoperative outcomes. However, the predictive power of intraoperative electroencephalogram (EEG) features for postoperative delirium has not yet been well studied.
METHODS: Intraoperative EEG data from 261 patients who underwent major cardiac surgery were analyzed. Cases were identified using the Confusion Assessment Method. Predictive analytics for delirium outcome were performed using (1) only clinical data, (2) only EEG data, and (3) a combined list of important features from the first two stages.
RESULTS: Eleven pe

BACKGROUND AND OBJECTIVES: The transpsoas approach for lateral lumbar interbody fusion (LLIF) places the surgical retractor near the lumbar plexus, particularly at the L4-5 level. To mitigate risk to these nerves, surgeons rely on intraoperative neuromonitoring using stimulation to approximate the distance between the plexus and retractor. The aim of this study was to review a single-institution series of lateral-position LLIF procedures to determine whether the stimulation threshold was associated with plexus injury.
METHODS: Single-level LLIF procedures performed in the lateral decubitus po

PURPOSE: Utility of pre-operative MRI for patients undergoing scoliosis correction has expanded to include an MRI classification for identifying patients at increased risk of experiencing intra-operative neuromonitoring (IONM) alerts based on the shape of the spinal cord and circumferential presence of CSF at the apex of the thoracic curve. In the present study, the authors explore the utility of this new MRI classification and multiple X-ray radiographic parameters in identifying the AIS sub-population at high risk of IONM alerts.
METHODS: AIS patients < 18 years old who underwent posterior

Brain multimodality monitoring measuring brain tissue oxygen pressure, cerebral blood flow, and cerebral near-infrared spectroscopy may help optimize the neurocritical care of patients with aneurysmal subarachnoid hemorrhage and delayed cerebral ischemia. We retrospectively looked for complications associated with the placement of the probes and checked the reliability of the different tools used for multimodality monitoring. In addition, we screened for therapeutic measures derived in cases of pathological values in multimodality monitoring in 26 patients with acute aneurysmal subarachnoid he

BACKGROUND: The paucity of easy-to-use, reliable objective neuromuscular monitors is an obstacle to universal adoption of routine neuromuscular monitoring. Electromyography (EMG) has been proposed as the optimal neuromuscular monitoring technology since it addresses several acceleromyography limitations. This clinical study compared simultaneous neuromuscular responses recorded from induction of neuromuscular block until recovery using the acceleromyography-based TOF-Watch SX and EMG-based TetraGraph.
METHODS: Fifty consenting patients participated. The acceleromyography and EMG devices analy
