The outcome of intra-articular distal radius fractures treated with fragment-specific fixation.

PURPOSE: To assess the clinical, radiographic, and functional outcome of treating intra-articular distal radius fractures with fragment-specific fixation. METHODS: A retrospective review of 81 patients with 85 intra-articular distal radius fractures who were treated with fragment-specific fixation was performed. Minimum time to follow-up evaluation was 1 year, with a mean time of 32 months. The immediate postoperative films were compared with those taken at the final follow-up evaluation. Radiographs of the uninjured wrist were also obtained at the final follow-up evaluation for comparison. P…

Treatment of distal radius fractures with intrafocal (kapandji) pinning and supplemental skeletal stabilization.

The technique of intrafocal pinning was originally described by Kapandji. This technique, along with the addition of supplemental external or internal bridging fixation, can be performed quickly with minimal soft tissue damage. This technique is indicated for the treatment of unstable extra-articular or minimally displaced intra-articular fractures of the distal radius. Studies demonstrate that young patients with displaced extra-articular distal radius fractures and minimal comminution can be treated by percutaneous intrafocal pin fixation alone, whereas, patients over 55 years of age and you…

[Simultaneous management of multi-fragment distal radius fractures with palmar and dorsal approach].

Reconstruction of the articular anatomy can be extremely difficult in distal radius fractures with severe comminution of both the dorsal and palmar cortices. In these cases, neither open reduction using an unilateral approach nor closed reduction with ligamentotaxis and fixateur externe will be successful. Reconstruction of both the dorsal and palmar cortical column is necessary. Sixteen of nineteen patients with unstable bicolumnar distal radius fractures treated by a combined plate osteosynthesis were studied retrospectively. Follow-up time averaged 32.5 months (8 to 74). At the time of re-e…

Distal radius fractures: mechanisms of injury and strength prediction by bone mineral assessment.

The strength of the radius depends on the mechanical properties of cancellous and cortical bone. By assessing both compartments quantitatively with bone densitometry, we tried to identify the specificity of each in predicting the load at which the distal radius will fracture. Twenty human cadaver forearms were scanned for bone mineral and geometric properties with quantitative computed tomography and dual x-ray absorptiometry. In both a neutral loading situation and one in which the wrist was extended 45 degrees, the load distribution was determined with pressure-sensitive films, and a fractur

Early open reduction and bone grafting for unstable fractures of the distal radius.

We describe a new technique for open reduction, bone grafting and fixation with a single Kirschner wire of unstable fractures of the distal radius. Of the 83 patients treated by this technique, most had regained volar tilt when seen at an average of 13 months after injury. Malunion was seen in 18 patients due either to poor placement of the graft and Kirschner wire or because of both volar and dorsal comminution. Assessment of hand and wrist function showed an average recovery of 63% of mass grip strength with an excellent return of specialised grip strength and range of movement. The advantag

Scapho-lunate diastasis in fractures of the distal radius. Pathomechanics and treatment options.

The concurrence of scapho-lunate diastasis and distal radial fractures is now well established. External fixation is considered to be one of the best methods of treating distal radial fractures, but in the presence of ligament injury, distraction fixation may lead to carpal instability and chronic wrist pain. We have reviewed existing English literature and analyzed possible mechanisms of injury, and offer some suggestions towards treatment of this combination of injuries.

Radio-scapho-lunate partial wrist arthrodesis following comminuted fractures of the distal radius.

Painful radiocarpal arthritis following comminuted fractures of the distal radius may be treated either by total wrist fusion or by procedures which preserve movement. The authors have reviewed 15 patients with such fractures who have undergone radio-scapho-lunate partial arthrodesis. They report the results with an average follow-up of 23.8 months. Pain was abolished in 7 patients and resolved virtually completely in 4 cases. Restored grip strength averaged 49% of the contralateral side. There was considerable limitation of postoperative range of motion which was restricted to an oblique plan

Ulnar nerve palsy following fractures of the distal radius: clinical and anatomical studies.

We report three cases of progressive ulnar nerve palsy following fractures of the distal radius. One was an open injury and one involved dorsal displacement of the nerve through a dislocated distal radio-ulnar joint. All were associated with dense scar tissue formation around the nerve. Early surgical decompression was successful in all cases. An anatomical study indicated that the relative immunity of the nerve to injury in this region may be due to its greater excursion and mobility compared with the median nerve.

Simultaneous Bennett's and distal radius fractures treated with external fixation. A case report.

Colles' fracture is rarely associated with a carpal or metacarpal fracture. We report a case of ipsilateral, simultaneous Bennett's and distal radius fracture after moderate trauma. Closed reduction and external fixation led to uneventful healing and no residual symptoms.

Predicting a safe screw length for volar plate fixation of distal radius fractures: lunate depth as a marker for distal radius depth.

PURPOSE: We hypothesized that the lunate depth as measured on plain lateral radiographs can be used to predict distal radius depth radially and ulnarly and serve as a useful reference for intraoperative screw placement in volar plate fixation of distal radius fractures. METHODS: Plain radiographs and magnetic resonance imaging (MRI) of the wrists of 30 patients were reviewed. The lunate depth and the maximal depth of the distal radius were determined from plain lateral radiographs. Depth of the distal radius, measured in quartiles, was determined from axial MRI images, and the lunate depth wa…

Measurement of ulnar variance and radial inclination on X-rays of healed distal radius fractures. With the axis of the distal radius or ulna?

Ulnar variance and radial inclination are radiological parameters frequently used to evaluate displacement of distal radius fractures. In most studies measurements are based on the long central axis of the distal radius, although the axis of the distal ulna can also be used. The purpose of this study was to determine which axis is more reliable. Four observers performed measurements on standard anteroposterior digital wrist X-rays of 20 patients taken 1 and 2 months after sustaining an extra-articular distal radius fracture. Intraobserver reliability was similar with both methods. No differenc…

Buckle Up! Formal Restrictions Are Not Required After Pediatric Distal Radius Buckle Fractures.

BACKGROUND: Trials that support brace treatment of pediatric distal radius buckle fractures either explicitly restricted all childhood activities or did not clarify activity protocols. In practice, parents and schools request documented recommendations. This prospective semi-randomized study aimed to determine whether formally prescribed activity restrictions are necessary during treatment of an isolated distal radius buckle fracture in school-aged children. METHODS: Children 5 to 16 years old with an acute, dorsal, distal radius buckle fracture were prospectively enrolled into 3 cohorts: (gr

Artificial Intelligence in Detection of Distal Radius Fractures in Adults: A Scoping Review.

Artificial intelligence has the potential to assist in the interpretation of radiographs. The distal radius is one of the most commonly fractured bones, and accurate and timely diagnosis is required for proper treatment. This review aims to summarize the current literature on the use of artificial intelligence, particularly convolutional neural networks, in diagnosing distal radius fractures. The results indicate that artificial intelligence has shown increasingly improved performance with image interpretation over time, often on par with specialized physicians. However, these models still str…

Distinct Cortical Microstructure in Postmenopausal Women with Distal Radius and Proximal Humerus Fractures.

Distal radius fractures and proximal humerus fractures are common fractures in postmenopausal women with osteoporosis. The question of whether bone microstructural deficits are reflected in specific changes in bone microstructure at peripheral skeletal sites has not yet been investigated. In this retrospective study, treatment-naïve postmenopausal women with distal radius fractures (DRF) or proximal humerus fractures (PHF) were included. Of more than 500 patients screened, 30 patients with DRF and 30 patients with PHF were included in the final analysis following propensity score matching. All…

Digital Eye: Deep Learning for Detecting Physeal Fractures of the Pediatric Distal Radius.

BACKGROUND: Distal radial fractures (DRFs) are some of the most common pediatric injuries, often involving the physis. Diagnostic accuracy can be challenging for physicians unfamiliar with managing pediatric injuries, with misdiagnosis rates reported as high as 46% [1-3]. These patients frequently do not present to a pediatric orthopaedic surgeon or musculoskeletal radiologist initially. Therefore, the development of tools to classify DRFs by physeal involvement could improve diagnostic accuracy and hence enhance patient safety. We propose to fine-tune convolutional neural networks (CNNs), a c

Fixation of the Volar Ulnar Corner in Distal Radius Fractures: A Comparative Study.

BACKGROUND: The purpose of this study was to compare outcomes of distal radius fractures with a volar ulnar corner (VUC) component treated with standard volar plating or by specific VUC fixation. This study investigated outcomes, radiographic measures, and specialty-based preference associated with surgical treatment of VUC injuries using VUC-specific fixation versus nonspecific VUC fixation. METHODS: We retrospectively analyzed outcomes for 39 patients with a distal radius fracture with VUC component at a level-1 trauma center over 10 years, 2011-2021. Patients underwent either VUC-specific …

Novel artificial intelligence model predicts the need for reduction of distal radius fractures.

PURPOSE: Distal radius fractures are among the most common upper extremity injuries. While convolutional neural networks (CNNs) have shown promise in fracture detection, no models have specifically addressed the need for reduction, which is a critical distinction for clinical decision-making. Current AI models detect fractures but fall short in guiding treatment, highlighting an opportunity to enhance triage through AI. METHODS: We retrospectively assembled 495 patients with AP, lateral, and oblique wrist radiographs. To mirror real-world deployment, images were smartphone-style screenshots s…
