Postpartum Continuous Glucose Monitoring to Detect Dysglycemia After Gestational Diabetes.

BACKGROUND: Up to one-third of people with gestational diabetes (GDM) will have persistent dysglycemia, and more than half do not complete the recommended postpartum oral glucose tolerance test (OGTT). This study assessed the use of blinded postpartum continuous glucose monitoring (CGM) to detect dysglycemia by assessing return rates, participant experience, and power to predict OGTT results. METHOD: Blinded CGM was placed on postpartum day 1 to 3 before discharge from the hospital and again at six weeks after pregnancy complicated by GDM and worn at home for up to 10 days. Participants maile…

Exploring Continuous Glucose Monitoring in Gestational Diabetes: A Systematic Review.

(1) Background: Gestational diabetes mellitus (GDM) is a glucose metabolism disorder that typically develops in the second half of pregnancy, transforming a normal pregnancy into a high-risk condition, with both short- and long-term complications for the mother and the fetus. Achieving optimal glycaemic control during pregnancy is essential for preventing these outcomes and could be realized using continuous glucose monitoring systems (CGMSs). This systematic review aims to evaluate the role of the CGMS as a potential diagnostic aid and predictor of maternal and fetal outcomes in GDM. (2) Meth…

Theragnostic utility of continuous glucose monitoring in post-gastrectomy hypoglycemia.

Diagnosing post-gastrectomy hypoglycemia is challenging, often relying on medical history with documented low plasma glucose levels. Here, we present three cases of patients who presented a high probability of post-gastrectomy hypoglycemia diagnosed and managed successfully using "theragnostic" continuous glucose monitoring and alpha-glucosidase inhibitors. In the first week, patients maintained their current lifestyle without medical intervention; in the second week, voglibose 0.2 mg before meals was prescribed. Continuous glucose monitoring data from the first week confirmed the diagnosis wi…

Continuous Glucose Monitoring: An Opportunity for Population-Based Diabetes Management.

The increasing prevalence of prediabetes and diabetes in the United States poses a serious threat to the health of the population. Closely associated with obesity, hypertension, dyslipidemia, and cardiovascular disease, a diagnosis of diabetes or prediabetes carries with it life-altering demands. Critically important among these is the requirement for frequent glucose level checks-typically performed with blood obtained from a fingerstick. Innovative real-time continuous glucose monitoring (rtCGM) technology makes it possible for patients and their clinicians to receive continuous glucose leve…

Continuous glucose monitoring and Rituximab treatment in insulin autoimmune syndrome.

Insulin autoimmune syndrome (IAS) or Hirata's disease is a rare cause of hypoglycemia. It is characterized by hyperinsulinemic hypoglycemia, … post prandial hypoglycemia and was diagnosed with insulin autoimmune syndrome. She was managed with continuous glucose monitoring and two doses of Rituximab 10 weeks apart, that resulted in resolution of hypoglycemia. This case report underlies the role of Rituximab as a first line agent for treatment of hypoglycemia in IAS.

Accuracy of Continuous Glucose Monitoring Measurements in Normo-Glycemic Individuals.

BACKGROUND: The validity of continuous glucose monitoring (CGM) is well established in diabetic patients. CGM is also increasingly used for research purposes in normo-glycemic individuals, but the CGM validity in such individuals is unknown. We studied the accuracy of CGM measurements in normo-glycemic individuals by comparing CGM-derived versus venous blood-derived glucose levels and measures of glycemia and glycemic variability. METHODS: In 34 healthy participants (mean age 65.7 years), glucose was simultaneously measured every 10 minutes, via both an Enlite® CGM sensor, and in venous blood…

GLUCOSE CHANGES IN A PATIENT WITH INSULIN AUTOIMMUNE SYNDROME DEMONSTRATED BY CONTINUOUS GLUCOSE MONITORING.

OBJECTIVE: Insulin autoimmune syndrome (IAS) is an unusual cause of hypoglycemia in individuals without underlying diseases. Continuous glucose monitoring (CGM) has rarely been applied for IAS. We present a case of IAS with available 6-day CGM data. METHODS: A 61-year-old Taiwanese man was admitted because of impaired consciousness while driving, caused by a low blood glucose level of 30 mg/dL. He regained consciousness fully after parenteral glucose administration. RESULTS: During the prolonged fasting test, his C-peptide and insulin levels were respectively 11 ng/mL and 169.34 μIU/mL when …

Blood-glucose Profile Evaluation with a Model-based Approach using Continuous Glucose Monitoring Data.

Continuous glucose monitoring (CGM) is widely used in type 1 diabetes management. Although less common in type 2 diabetes (T2D), its application is increasing, especially among patients with T2D on insulin therapy. CGM provides detailed, continuous glucose data that reveal daily glycemic fluctuations and help mitigate hyper- and hypoglycemic episodes. However, missing information on meal size and timing complicates the interpretation of data. To address these challenges, we propose a pharmacometric modeling approach that describes blood glucose profiles in patients with T2D receiving basal ins…

Glucose Profiles in Pregnant Women After a Gastric Bypass : Findings from Continuous Glucose Monitoring.

BACKGROUND: The diagnosis of gestational diabetes mellitus (GDM) usually requires an oral glucose tolerance test, but this procedure seems inappropriate after gastric bypass surgery (Roux-en-Y gastric bypass (RYGB)) due to specific altered glycemic responses. We aimed here at describing continuous glucose monitoring (CGM) profile of pregnant women after RYGB. METHODS: CGM was performed in 35 consecutive pregnant women after RYGB at 26.2 ± 5 weeks of gestation. RESULTS: After RYGB, pregnant women display high postprandial interstitial glucose (IG) peaks and low IG before and 2 h after meals. …

The cost-effectiveness of real-time continuous glucose monitoring versus intermittently scanned continuous glucose monitoring in individuals with insulin-treated Type 2 diabetes mellitus in Canada.

Aim: Continuous glucose monitoring (CGM) supports glycemic control and reduces diabetes complications. CGM systems include intermittently scanned CGM (is-CGM) and real-time CGM (rt-CGM). While rt-CGM may provide better outcomes than is-CGM, it costs more upfront and its cost-effectiveness in Canada has not been established. We assessed the cost-effectiveness of rt-CGM versus is-CGM in people with insulin-treated Type 2 diabetes mellitus (T2DM) from a Canadian healthcare payer perspective. Materials & methods: We used the ECHO-T2DM microsimulation model to estimate incremental lifetime health o…

Quantifying temporal glucose variability in diabetes via continuous glucose monitoring: mathematical methods and clinical application.

BACKGROUND: Continuous glucose monitors (CGMs) collect detailed blood glucose (BG) time series, which carry significant information about the dynamics of BG fluctuations. In contrast, the methods for analysis of CGM data remain those developed for infrequent BG self-monitoring. As a result, important information about the temporal structure of the data is lost during the translation of raw sensor readings into clinically interpretable statistics and images. METHODS: The following mathematical methods are introduced into the field of CGM data interpretation: (1) analysis of BG rate of change;

[Standardization of quality criteria for continuous glucose monitoring systems in Argentina].

Although glycated hemoglobin is the standard method for assessing glycemic control in people with diabetes mellitus (DM), this biomarker does not reflect circadian or daily glycemic variations, which means that episodes of hypoglycemia and/or hyperglycemia -which are associated with acute or chronic microvascular and macrovascular complications of DM- may go unnoticed. Continuous glucose monitoring (CGM) systems provide real-time glucose readings, allowing for the identification of trends and fluctuations throughout the day. These devices facilitate treatment decisions to optimize glucose cont…

Characteristic Glycemic Patterns during Capivasertib Treatment Identified by Continuous Glucose Monitoring.

We herein report a case of hyperglycemia in a patient with recurrent breast cancer treated with the AKT inhibitor capivasertib. Continuous glucose monitoring revealed a distinctive pattern characterized by pronounced daytime postprandial hyperglycemia and nocturnal-to-morning decline, which resolved during the drug-free intervals. Hyperglycemia was unresponsive to metformin treatment but was markedly exacerbated by intravenous glucose infusion. These findings suggest that capivasertib-induced hyperglycemia is caused by an impaired glucose uptake and dysregulated hepatic glucose output associat…
