Forty five patients aged 40 years or younger were treated with coronary intervention in our institution between 1983 and 1994. This young Japanese population had a strong predisposition to risk factors but did not have extensive disease. These patients underwent 50 elective interventional procedures for angina pectoris or old myocardial infarction and 6 direct balloon angioplasty procedures for acute myocardial infarction. The initial successful result was obtained in 41 of the 45 patients (91%). The mean follow-up was 43 +/- 35 months. Angiographic follow-up was available in 31 of the 41 elig

The lack of consistent definitions and nomenclature across clinical trials of novel devices, drugs, or biologics poses a significant barrier to accrual of knowledge in and across peripheral artery disease therapies and technologies. Recognizing this problem, the Peripheral Academic Research Consortium, together with the U.S. Food and Drug Administration and the Japanese Pharmaceuticals and Medical Devices Agency, has developed a series of pragmatic consensus definitions for patients being treated for peripheral artery disease affecting the lower extremities. These consensus definitions include

PURPOSE: To evaluate real-world outcomes of endovascular treatment for femoropopliteal lesions involving the popliteal artery and, in a post-hoc exploratory analysis, assess whether anatomical location (P3 vs. P1/P2 segments) was associated with clinical decision-making and prognosis.
MATERIALS AND METHODS: We conducted a retrospective, post-hoc analysis of 946 patients from the prospective PROMISING registry. For this exploratory analysis, patients were stratified according to distal (P3) versus proximal/mid (P1/P2) popliteal artery involvement. Predictors of stent implantation and clinical

Despite limited clinical evidence, the use of drug-coated balloons (DCB) in percutaneous coronary intervention for de novo coronary lesions is expanding and increasing. ALLIANCE is a prospective, multicenter, all-comer registry designed to evaluate the efficacy and safety of the paclitaxel-DCB in an expanded real-world clinical setting. A calculated sample size of 1500 patients was expected to provide 95% power to detect non-inferiority against a performance goal of 7.5% target lesion failure (TLF) at 1 year with a 3.0% absolute non-inferiority margin. The impact of chronic kidney disease (CKD) stage on outcomes after percutaneous coronary intervention (PCI) for left main (LM) coronary artery disease remains unclear. We investigated the impact of CKD stage on long-term outcomes after contemporary LM-PCI. We analyzed 756 consecutive patients who underwent LM-PCI for de novo lesion under intracoronary imaging-guidance across 19 hospitals. Patients were categorized by CKD stage: I/II (n = 360), IIIa (n = 212), IIIb (n = 105), IV (n = 40), and V (n = 39). Three-year major adverse cardiovascular and cerebrovascular events (MACCE), including all-c

OBJECTIVE: Coronary artery perforation (CAP) is a rare but potentially catastrophic complication of percutaneous coronary intervention (PCI). We describe a prespecified candidate recognition pattern - the perforation triad - that may help operators identify an evolving perforation during inflation.
METHODS: In this single-center retrospective matched case-control study, 9865 PCI procedures performed between 2020 and 2025 were screened. Forty-one balloon- or stent-related Ellis II-III CAP cases (0.42%) were matched 1: 1 by age, sex, American College of Cardiology/American Heart Association les

INTRODUCTION AND OBJECTIVES: Implantation of drug-eluting stents (DES) is currently the standard of care in chronic total occlusion percutaneous coronary intervention (CTO-PCI). However, drug-coated balloons (DCB) have emerged as a promising alternative, providing antiproliferative drug delivery while avoiding permanent scaffolding. This meta-analysis compared the effectiveness of DCB and hybrid strategies vs DES in CTO-PCI.
METHODS: Studies comparing DCB with and without hybrid strategies and DES in CTO-PCI were identified through PubMed/MEDLINE and Web of Science up to February 2025. The pr

BACKGROUND: Contemporary guidelines lack support for the performance of complex percutaneous coronary intervention (PCI) in hospitals with no on-site cardiac surgery (NOSCS). We compared safety and efficacy of complex PCI performed at sites with and without on-site cardiac surgery.
METHODS: Data from the Victorian Cardiac Outcomes Registry from 2014 and 2022 was retrospectively analysed. Complex PCI was defined as PCI to unprotected left main, bifurcations, rotational atherectomy or intravascular lithotripsy use, severe left ventricular systolic dysfunction, chronic total occlusion, or vein g

It is sometimes difficult to identify the culprit lesion and treatment strategy in patients with acute coronary syndrome who have complex coronary lesions and jeopardized left internal mammary artery graft. This report describes a heart team approach for a non-ST-segment elevation myocardial infarction case with complex coronary vasculature. A 73-year-old man presented to the emergency department with crescendo angina. He had a history of total aortic arch replacement with concomitant coronary artery bypass graft using left internal mammary artery. Emergent coronary angiography demonstrated se

The durability of chronic total occlusion (CTO) percutaneous coronary intervention (PCI) is uncertain. We aimed to compare the long-term outcomes of CTO PCI with those of complex non-CTO PCI. We built a single-center registry including all patients who underwent CTO and complex non-CTO PCI between 2012 and 2017. Complex non-CTO PCI was defined as: 3 vessels treated, ≥3 stents implanted, total stent length >60 mm, saphenous vein graft intervention, 2-stent bifurcation intervention, left main PCI, protected PCI, or rotational/laser atherectomy. The primary endpoint was target-lesion failure, a c

For the revascularization of patients with clinical and anatomical complexities, several technical skills are often required. However, the prognostic effect of complex percutaneous coronary intervention (C-PCI) on the clinical outcomes is not well known. The aim of this study was to investigate the relationship between the C-PCI and mid-term clinical outcomes. We assessed 1062 patients who underwent PCI with newer-generation drug-eluting stent and stratified the patients according to whether they had complex PCI (C-PCI, n = 358) or non-complex PCI (non-C-PCI, n = 704). C-PCI was defined as a p

OBJECTIVES: This review aims to evaluate the adverse outcomes for patients after treatment with covered stents.
BACKGROUND: Coronary perforation is a potentially fatal complication of percutaneous coronary revascularization which may be treated using covered stents. Studies have evaluated long-term outcomes among patients who received these devices, but hitherto no literature review has taken place.
METHODS: We conducted a systematic review of adverse outcomes for patients after treatment with covered stents. Data from studies were pooled and outcomes were compared according to stent type.

BACKGROUND: Risk stratification after percutaneous coronary intervention (PCI) is mainly based on demographics and clinical presentation (stable coronary artery disease [CAD] vs. acute coronary syndromes [ACS]). We investigated the impact of PCI extent and complexity on 2-year clinical outcomes after successful implantation of drug-eluting stents (DES) and whether this effect is influenced by clinical presentation and/or high platelet reactivity (HPR) on clopidogrel.
METHODS AND RESULTS: Patients from the prospective, multicenter Assessment of Dual Antiplatelet Therapy with Drug-Eluting Stent

BACKGROUND: Enabling strategies (ESs) are increasingly used during percutaneous coronary intervention for chronic total occlusive disease (CTO-PCI), enhancing procedural success. Using the British Cardiovascular Society dataset, we examined changes in the use of ESs and procedural/clinical outcomes for CTO-PCI.
METHODS AND RESULTS: ESs were defined as intravascular ultrasound, rotational/laser atherectomy, dual arterial access, use of microcatheters, penetration catheters or CrossBoss, and procedures categorized by number of ESs used. Data were analysed on all elective CTO-PCI procedures perf

OBJECTIVES: Outcomes with use of lesion-modification strategies in the drug-eluting stent era have received limited study.
METHODS: We conducted a meta-analysis of 22 studies published between 2004-2016 reporting outcomes after use of rotational atherectomy, cutting-balloon, and scoring-balloon angioplasty.
RESULTS: In observational trials, acute luminal gain was higher after lesion modification as compared with control (standardized mean difference, 0.23 mm; 95% confidence interval [CI], 0.01-0.44; P=.04), with no difference in acute gain in randomized studies. Compared with control, lesion

Coronary artery disease (CAD) rates are higher in Asian-Indians than in Caucasians. CAD occurs at an earlier age in this group, with about a quarter of all myocardial infarctions occurring under the age of 40. Previous reports have suggested smaller coronary artery size in Asian-Indians as a major cause for increased CAD in this population. This study sought to evaluate the size of normal "atheroma-free" segments of the epicardial coronary arteries in Asian-Indians and Caucasians aged ≤40 years undergoing coronary artery intervention in other diseased segments. A total of 69 consecutive patien

OBJECTIVE: Although drug-eluting balloons (DEBs) have shown promising results treating de novo (DN) atherosclerotic lesions and appear to have been widely adopted in Europe, their long-term efficacy in the broad spectrum of femoropopliteal restenosis (RE) remains to be proven. The purpose of the study was to assess the efficacy of paclitaxel-DEBs in restenotic (stented and nonstented) vs DN stenotic femoropopliteal arteries.
METHODS: The study prospectively enrolled 100 patients undergoing femoropopliteal endovascular intervention by DEB for RE or DN stenosis. Patients who received additive a
