Identification of internuclear ophthalmoplegia signs in multiple sclerosis patients. Saccade test analysis.

BACKGROUND AND PURPOSE: Patients with multiple sclerosis (MS), due to the diverse nature of the disease, have many types of eye movement disorders, particularly internuclear ophthalmoplegia (INO). The aim of the study was to evaluate the usefulness of a quantitative statistical saccade test analysis for identification of INO in MS patients. METHODS: Sixty MS patients and 50 volunteers underwent clinical bedside ocularmotor examination and electrooculographic evaluation in a tertiary referral university hospital. Simultaneous electro-oculography recordings of each eye separately were made. In

[Gaucher's disease--ocular manifestation and treatment].

Gaucher's disease is the most common lysosomal storage disorder due to a deficiency of glukocerebrosidase activity. It leads to an accumulation of glukosylceramide within the cells of the the reticuloendothelial system. Gaucher's disease is divided into three subtypes based on clinical symptoms. Type I--nonneuronopathic--chronically progressive in adulthood, type II--acute neuronopathic--infantile form lead up to the difficult damage nervous system, and type III--juvenile form--subacute neuronopathic. The aim of this paper is to present the typical ocular symptoms which occured in the disease.

Saccadic instabilities in albinism without nystagmus.

Albinism effects a surprising manipulation of the visual pathway in which some of the normally uncrossed axons of the temporal retina instead cross at the chiasm. An expected consequence of this misrouting is that subjects with albinism will have difficulty in specifying the targets of saccades. Usually albinos have nystagmus so the stability of their saccadic eye movements is not readily accessible, but some albinos do not have nystagmus. In these subjects it was found that they had frequent saccadic intrusions, the sizes of which were correlated with velocities of steady drifts in fixations

Evidence for ocular motor deficits in developmental dyslexia: application of the double-step paradigm.

PURPOSE: Dyslexia is a language-based learning disability characterized by difficulties with reading, spelling, and writing. Persons with dyslexia often have deficits in processing rapid temporal sensory information. There is also evidence of sensorimotor deficits in persons with dyslexia. Whether these deficits include ocular motor problems is still an open question. Some previous studies have shown an increased saccadic latency in dyslexics, whereas others have not reproduced this finding. The purpose of the present study was to investigate saccadic latency in young adults with dyslexia duri

Conjugate eye deviation in acute stroke: incidence, hemispheric asymmetry, and lesion pattern.

BACKGROUND AND PURPOSE: The authors studied clinical features and lesion patterns associated with conjugate eye deviation (CED) in acute stroke. METHODS: Consecutive patients with supratentorial stroke (<6 hours after symptom onset) were prospectively recruited. CED was classified according to the National Institute of Health Stroke Scale (NIHSS) item "gaze palsy" as being forced, partial, or absent. Lesion patterns on diffusion-weighted images (DWI) were analyzed in 46 patients using a normalization procedure for direct comparisons between patients. Perfusion data were analyzed for a subset

A catastrophic-onset longitudinal myelitis accompanied by bilateral internuclear ophthalmoplegia in a patient with systemic lupus erythematosus.

Transverse myelitis (TM) extending from midbrain to the entire spinal cord accompanied by internuclear ophthalmoplegia is extremely rare but cause serious central nervous system complications in patients with systemic lupus erythematosus. We report a case of a 28-yr-old woman with TM extending from the midbrain to the conus medullaris longitudinally and internuclear ophthalmoplegia associated with systemic lupus erythematosus. Her neurological symptoms had an abrupt catastrophic onset and rapidly progressed to respiratory failure within 24 hr. Bilateral internuclear ophthalmoplegia was also fo

[Comments on Cogan's syndrome (congenital oculomotor apraxia) based on a case].

CASE: A 4-month-old girl was seen by a paediatrician for psychomotor delay and defective visual fixation. In order to shift gaze direction, she had to jerk her head in order to drag her eyes in the direction she wanted to look. She also showed non-specific pyramidal impairment. Herpes human virus type 6 (HHV-6) was amplified by PCR. No other findings could explain her troubles with fixation. She was diagnosed with congenital ocular motor apraxia (C.O.M.A.). DISCUSSION: When C.O.M.A. is suspected, other causes of defective fixation and abnormal cephalic movements must be ruled out through the

Role of anterior and occipital white matter lesions for smooth eye tracking in myotonic dystrophy.

BACKGROUND: The degradation of smooth pursuit eye movements (SP) in myotonic dystrophy (MD) has been shown to result from lesions in the central nervous system. Imaging and histopathological studies have shown considerable areas of white matter lesions (WMLs) with periventricular emphasis. OBJECTIVE: To discover which of these WMLs are relevant in SP impairment? METHOD: Horizontal sinusoidal SP, the vestibulo-ocular reflex (VOR), and its suppression by fixation (VOR-S) were investigated in 12 patients with MD, and the data obtained were compared with those from a control group. RESULTS: Par

Quantitative oculographic characterisation of internuclear ophthalmoparesis in multiple sclerosis: the versional dysconjugacy index Z score.

BACKGROUND: There is a poor correlation between multiple sclerosis disease activity, as measured by magnetic resonance imaging, and clinical disability. OBJECTIVE: To establish oculographic criteria for the diagnosis and severity of internuclear ophthalmoparesis (INO), so that future studies can link the severity of ocular dysconjugacy with neuroradiological abnormalities within the dorsomedial brain stem tegmentum. METHODS: The study involved 58 patients with multiple sclerosis and chronic INO and 40 normal subjects. Two dimensional infrared oculography was used to derive the versional dysc

Ocular myasthenia mimicking a one-and-a-half syndrome.

A 52-year-old patient developed an eye movement disorder first resembling a left internuclear ophthalmoplegia and subsequently a "one-and-a-half syndrome" as the presenting symptoms of ocular myasthenia gravis. No accompanying myasthenic features were present except for the fluctuation in the amplitude of dissociated nystagmus. This patient shows that an oculomotor disorder considered a typical pontine lesion may instead be caused by myasthenia gravis, even in the absence of other clinical and electrophysiologic features of neuromuscular deficit.

Congenital ocular motor apraxia with autosomal dominant inheritance.

PURPOSE: To document congenital ocular motor apraxia in five first-degree relatives. METHODS: Case series. Five family members with a history of horizontal head thrusting had neuro-ophthalmologic evaluation. Magnetic resonance imaging of the brain was obtained in the proband. RESULTS: Four siblings (one boy and three girls) had congenital ocular motor apraxia. The father had head thrusting as a child and displayed residual signs of the disorder. Magnetic resonance imaging disclosed no central nervous system abnormalities in the father. CONCLUSION: Congenital ocular motor apraxia can be inhe

Ocular torticollis and facial asymmetry.

OBJECTIVE: Ocular torticollis secondary to congenital superior oblique palsy can be associated with facial asymmetry. The purpose of this study was to determine if other ocular causes of head tilting also carry the same association. DESIGN: The study design was clinical evaluation of patients. PARTICIPANTS: Fifteen patients with marked, moderate, or mild head tilts associated with dissociated vertical deviation (DVD), and 3 patients with nystagmus and a torsional null point. A control group was comprised of 3 patients with congenital superior oblique palsy, 3 with traumatic superior oblique

Congenital ocular aberrant innervation--new concepts.

BACKGROUND: We have the impression that congenital aberrant innervations are more common than previously reported. Many varieties exist and typically involve the sixth nerve. The most common ocular miswirings are Duane's syndrome and Marcus Gunn jaw-winking ptosis. The second most common miswiring involves lateral rectus activation in upgaze causing a "Y" pattern exotropia (pseudo inferior oblique overaction). This commonly is confused with inferior oblique overaction but surgery on the obliques does not cure the condition. Lateral rectus recession and elevation are required. METHODS: We sele

[Perinaud's oculoglandular syndrome as a manifestation of cat-scratch disease].

The first Danish case of Parinaud's oculoglandular syndrome, a manifestation of cat-scratch disease, is reported in a 35-year-old man presenting with an enlarged preauricular lymph node and an ipsilateral conjunctival granuloma. Surgical removal of the granulomatous lesion was followed by rapid healing. The diagnosis was verified by demonstrating a high antibody tire against Rochalimaea (Bartonella) henselae. On subsequent questioning the man gave a history of acquiring a kitten six weeks before his illness. The importance of eye examination in patients presenting with preauricular lymphadenop
