In Part I, "Developmental Case Study", this evaluation traces anecdotally the increasing application of microcomputer technology in a research program examining the uses of a robot arm in the rehabilitation of the high-spinal-cord-injured person. This program, supported by the Veterans Administration since 1974, built upon still earlier VA-supported work at Johns Hopkins on powered upper-limb prostheses. The Johns Hopkins University powered shoulder prosthesis served as the basis for the Robotic Arm/Worktable System, whose evolution is described in applications ranging from handling simple rea…

BACKGROUND: Bone cement implantation syndrome comprises the occurrence of hypoxaemia, hypotension, unexpected loss of consciousness, or cardiac arrest occurring around the time of cementation, prosthesis insertion, or reduction of the joint in arthroplasty. As the yearly number of arthroplasties will increase, the number of complications including bone cement implantation syndrome is also expected to increase. Therefore, a good understanding of this syndrome is important. This scoping review aims to provide a comprehensive overview of the current knowledge of bone cement implantation syndrome.

BACKGROUND: Cervical disc replacement (CDR) is an increasingly used alternative to fusion for symptomatic cervical disc disease. While more studies have suggested favorability of CDR over fusion procedures, limited data exist regarding implant fatigability. Here, the authors present a unique and previously unreported failure of the M6-C prosthesis causing spinal cord injury.
OBSERVATIONS: A 49-year-old female with history of cervical degenerative disease and prior C4-7 M6-C arthroplasty presented 9 years later after a minor fall from standing. She endorsed bilateral hand numbness ascending to

BACKGROUND: Glenoid loosening is the primary reason for failure after a total shoulder arthroplasty (TSA), but the failure mechanism is not yet known. This study determined how the load transfer and stress distribution are affected by the introduction of a glenoid implant.
METHODS: We developed a finite-element model of a scapula with and without a virtually implanted modern glenoid prosthesis design. Two load magnitudes were considered: normal and high. Loading locations were simulated at the center and at 4 eccentric positions on the glenoid. A metal-backed implant was also simulated to und

OBJECTIVE: To describe the authors' surgical technique and to evaluate the final functional outcome of surgical treatment of the "terrible triad of the elbow".
METHODS: Eight patients identified with "terrible triad" injury patterns, including posterior elbow dislocation, radial head fracture and coronoid fracture, were available for a minimum of 11 months follow-up. Evaluation of functional outcome was based on Mayo elbow performance, Broberg-Morrey scores, and the Disabilities of the Arm, Shoulder and Hand (DASH) questionnaire. Complications were also recorded.
RESULTS: Five elbows redislo

Recent advances in the design of prosthetic arms have helped upper limb amputees achieve greater levels of function. However, control of upper limb prostheses is limited by the lack of sensory feedback to the user. Targeted reinnervation, a novel surgical technique for amputees, offers the potential for returning this lost sensation. During targeted reinnervation surgery, truncated nerves are directed to reinnervate new muscle and skin sites. Contractions of reinnervated muscles generate electrical signals that are used to control prosthetic arms. In addition, stimulation of reinnervated skin

UNLABELLED: Metastatic bone disease of the humerus may require surgery for treatment of an impending or existing pathologic fracture or for alleviating disabling pain. Prompt restoration of function is a main goal of surgery, although published results do not reveal if that goal is being met. We retrospectively reviewed range of motion and function of 59 patients operated on from 1986-2003 for those indications. After resection, tumors around the humeral head and condyles (n = 20) were reconstructed with a prosthesis, and tumors at the humeral diaphysis (n = 39) were reconstructed with cemente

AIMS: Radiostereometric analysis (RSA) allows an extremely accurate measurement of early micromotion of components following arthroplasty.
PATIENTS AND METHODS: In this study, RSA was used to measure the migration of 11 partially cemented fluted pegged glenoid components in patients with osteoarthritis who underwent total shoulder arthroplasty using an improved surgical technique (seven men, four women, mean age 68). Patients were evaluated clinically using the American Shoulder and Elbow Surgeons (ASES) and Constant-Murley scores and by CT scans two years post-operatively.

PURPOSE: Aseptic glenoid component loosening remains a common problem in total shoulder arthroplasty (TSA). This study presents long-term prospective follow-up after implantation of a glenoid component using the "cancellous compaction technique" and its effect on clinical outcome and presence and progression of radiolucent lines (RLLs).
METHOD: Thirty-nine TSAs were performed for primary osteoarthritis by one surgeon using the same technique. For the glenoid side, a keeled, polyethylene, convex-backed component was implanted using the "cancellous compaction technique" consisting of minimal re

Between April 1991 and March 1997, 182 bipolar shoulder replacements were implanted in 174 patients (8 bilateral) for painful conditions of the shoulder. The study group comprises 108 patients who were followed for an average of 2.9 years (range, 2-6 years). Diagnoses included osteoarthritis (51), rotator cuff arthropathy (27), avascular necrosis (3), revisions (8), rheumatoid arthritis (2), and fractures--both old and recent (17). A satisfactory rating (University of California at Los Angeles shoulder rating score greater than or equal to 28 points of 35) was achieved by 72% of the patients (

(1) Background: Modular megaprosthetic reconstruction using a proximal humerus replacement has emerged as a commonly chosen approach after bone tumor resection. However, the long-term risk for revision surgery is relatively high. One factor that might be associated with mechanical failures is periprosthetic osteolysis around the stem, also known as stress shielding. The frequency, potential risk factors, and the effect on implant survival are unknown. (2) Methods: A retrospective single-center study of 65 patients with sarcoma who underwent resection of the proximal humerus and subsequent reco

BACKGROUND: Previous studies have indicated an increased risk of periprosthetic joint infection (PJI) in patients treated with reverse shoulder arthroplasty (RSA) compared with patients treated with anatomic total shoulder arthroplasty. The reason for this is unclear but may be related to a high prevalence of previous rotator cuff repair in patients who are treated with RSA. The purpose of this study was to determine previous non-arthroplasty surgery as a risk factor for revision owing to PJI after RSA for cuff tear arthropathy, massive irreparable rotator cuff tears, or osteoarthritis.

BACKGROUND: The purpose of this study was 2-fold: (1) to quantify type E2 bone loss orientation and its association with rotator cuff fatty infiltration and (2) to examine reverse baseplate designs used to manage type E2 glenoids.
METHODS: Computed tomography scans of 40 patients with type E2 glenoids were examined for pathoanatomic features and erosion orientation. The rotator cuff fatty infiltration grade was compared with the erosion orientation angle. To compare reconstructive options in light of the pathoanatomic findings, virtual implantation of 4 glenoid baseplate designs (standard, ha

BACKGROUND: 4-part proximal humerus fractures are complex injuries that are often associated with comminution of the greater tuberosity. The purpose of this study is to evaluate the functional outcomes of 4-part humerus fractures that are treated with reverse total shoulder arthroplasty (rTSA) and correlate these outcomes with the healing status of the greater tuberosity.
MATERIAL AND METHODS: A retrospective observational study was performed including 65 patients who underwent rTSA following a 4-part proximal humerus fracture. These patients were categorized into 3 groups according to the he

Many surgeons continue to face the clinical dilemma of interpreting a positive aspiration or unexpected positive Cutibacterium acnes (C. acnes) culture. There are factors that complicate the interpretation of positive cultures including variations in both frequency of false positive cultures and virulence properties. As indices of virulence, hemolytic strains, from previously confirmed clinically infected shoulders, were compared with non-hemolytic isolates determined to be contaminants, by RNA-sequencing (RNA-Seq). Six C. acnes isolates from patients who underwent revision total shoulder arth

HYPOTHESIS/BACKGROUND: Patients who are treated with reverse shoulder arthroplasty in the setting of proximal humeral bone loss present a technical challenge for humeral component fixation. The purpose of this study was to determine the effect of proximal humeral bone loss on fixation of reverse shoulder humeral implants.
MATERIALS AND METHODS: Three reverse humeral designs (two modular and one monobloc) were cemented into twenty-four Sawbones humeri prepared to simulate intact and proximal humeral bone loss. Torque was applied to the humerus for 1,000 cycles in increments of 2.5 N-m to 25 N-
