[Technique and diagnostic value of musculoskeletal ultrasonography in rheumatology. Part 6: ultrasonography of the wrist/hand].

Sonography of the hands is especially helpful in the diagnosis of early arthritis. Sonography allows for a very sensitive detection of small … c evaluation of the fingers: 1) volar longitudinal, 2) volar transverse scan in extension along the finger joints to detect effusion and synovial proliferation, tenosynovitis, irregularities of the bone surface (osteophytes, erosions), 3) dorsal longitudinal scans in extension and flexion >70 degrees along the CMC I, MCP, PIP and DIP joints to evaluate effusio …

Bony mallet finger: a novel surgical approach to the terminal tendon insertion using a dual parallel incision - A case series study.

A bony mallet finger refers to an avulsion fracture at the dorsal base of the distal phalanx. When the fracture fragment involves more than one-third of the articular surface and is accompanied by volar subluxation of the DIP joint, the joint's stability is compromised. In this setting, surgical treatment is often necessary. Open reduction and hook plate fixation provide the most rigid fixation and have become increasingly popular. However, surgical approaches - including Y-, H-, S-, and U-shaped incisions - are frequently associated with complications such as skin necrosis, nail deformity, an

Multiple bilateral trigger fingers in a child with neurofibromatosis type I following an acute viral infection: A case report.

INTRODUCTION: Triggering of the fingers in children is an uncommon entity. A review of the literature on the topic did not reveal any reports of multiple triggering in patients with neurofibromatosis. CASE REPORT: Our patient is a known case of neurofibromatosis type I (NF 1). At the age of 4 years, the child required chemotherapy to treat symptomatic bilateral optic glioma. Chemotherapy was complicated by severe viral upper respiratory tract infection with concurrent multi-joint synovitis and mild triggering of the fingers. A second course of chemotherapy was required 2 years later. This was

Adult-onset idiopathic progressive acro-osteolysis with proximal symphalangism.

We experienced a 57-year-old female with adult-onset non-congenital idiopathic acro-osteolysis combined with proximal symphalangism. At the age of 36, she developed severe pain and swelling of the toe base of both feet and underwent Clayton surgery. However, the size of her toes diminished progressively over the 5-year period after surgery. At the age of 41, she suffered pain and swelling of the proximal interphalangeal (PIP) joints of fingers of both hands. These PIP joints became rigid and inflexible. Subsequently, she noticed shortening of the little finger of both hands, followed later by

Ring avulsion injuries: classification and prognosis.

Fifty-five cases of ring avulsion injury were reviewed to examine how extent of injury and surgical management correlated with results. After injury, three patients had adequate circulation. Of 52 patients with inadequate circulation, 8 had primary amputation; salvage was attempted in 44. Of these 44, 9 fingers were amputated secondarily, 19 were successfully revascularized, and 16 were successfully replanted. Return-to-work time averaged between 4.5 and 10.3 weeks. Cold symptoms occurred in 65% of salvaged fingers. Range of motion was usually normal at the metacarpophalangeal joint; most pati

Hangboard Training as a Preventive Strategy for Upper Extremity Pathologies in Rock Climbers.

CONTEXT: Rock climbing places significant mechanical demands on the fingers, wrists, elbows, and shoulders. The repetitive use of isometric gripping, extreme joint positions, and dynamic movements contributes to increasing upper extremity overuse injuries as the sport rises in popularity. Many of these injuries stem from inadequate finger and forearm strength, leading to compensation with proximal musculature and secondary stabilizers. Hangboarding has become a well-established regimen for improving finger flexor strength, but its role in injury prevention remains poorly defined. EVIDENCE ACQ

Monodigital Double Cross-Finger Flaps-A Novel Approach for Coverage of Large Volar Defects.

INTRODUCTION: A single cross-finger flap can only address digital defects of a limited size, and larger defects often require distant flaps such as abdominal or groin flaps that are cumbersome to perform and sometimes bulky. Two cross-finger flaps harvested from the same donor digit could offer a simple and effective solution for many such defects. In this study, we describe the technique of monodigital double cross-finger flaps and its clinical applicability, with evaluation of donor and recipient site morbidity. METHODS AND MATERIALS: A total of 10 subjects with large volar defects were inc

A biomechanical study of distal interphalangeal joint subluxation after mallet fracture injury.

PURPOSE: There is no consensus in the literature regarding the size of a mallet fracture fragment that may lead to subluxation of the distal interphalangeal (DIP) joint. The purpose of this study was to determine the relationship between the size of the dorsal articular fragment and DIP joint subluxation in a cadaveric mallet fracture model. METHODS: Twenty-nine fresh-frozen fingers without evidence of DIP joint osteophytes were dissected to the metacarpal base. The mean age of the 17 donors at the time of death was 69 years (range, 46 to 89 years). Obliquely oriented fractures through the do

Hand injuries in competition climbers.

All 67 of the competitors at the first British Open climbing competition were examined for signs of previous or present hand injury. The most important clinical findings were that 26 per cent of the climbers had signs of previous injury to the A2 pulley of the ring finger, and that fixed flexion deformity of the proximal interphalangeal joints of the fingers was present in 24 per cent.

Management of Proximal Interphalangeal Joint Fracture Dislocations.

Fracture dislocations of the proximal interphalangeal (PIP) joint of the finger are often caused by axial load applied to a flexed joint. The most common injury pattern is a dorsal fracture dislocation with a volar lip fracture of the middle phalanx. Damage to the soft-tissue stabilizers of the PIP joint contributes to the deformity seen with these fracture patterns. Unfortunately, these injuries are commonly written off and left untreated. A late-presenting PIP joint fracture dislocation has a poor chance of regaining normal range of motion. The provider must be suspicious of these injuries.

The biomechanical model of the long finger extensor mechanism and its parametric identification.

The extensor mechanism of the finger is a structure transmitting the forces from several muscles to the finger joints. Force transmission in the extensor mechanism is usually modeled by equations with constant coefficients which are determined experimentally only for finger extension posture. However, the coefficient values change with finger flexion because of the extensor mechanism deformation. This induces inaccurate results for any other finger postures. We proposed a biomechanical model of the extensor mechanism represented as elastic strings. The model includes the main tendons and ligam…

A Novel Approach for Reconstruction of Finger Neurocutaneous Defect: A Sensory Reverse Dorsal Digital Artery Flap from the Neighboring Digit.

OBJECTIVE: Providing soft tissue coverage for finger neurocutaneous defects presents aesthetic and sensory challenges. A common source for reconstruction of soft tissue defects of the fingers is the same finger. However, when the donor areas are damaged by concomitant injuries, this option is not available. The present study aims to reconstruct finger neurocutaneous defects using a sensory reverse dorsal digital artery flap from the neighboring digit and to evaluate the efficacy of this technique. METHODS: The study included 16 patients, with an average age of 34.9 years (range, 20-53 years)

Patterns of Involvement of the Hand Joints in Classical Rheumatoid Arthritis.

BACKGROUND: Symmetrical involvement of the hand joints is described as characteristic of rheumatoid arthritis (RA). Quantitative data on specific patterns of involvement are lacking. OBJECTIVE: The Brigham Rheumatoid Arthritis Sequential Study was created for observational studies of patients with RA and afforded a unique opportunity to answer these questions. METHODS: Of 1598 subjects in the Brigham Rheumatoid Arthritis Sequential Study cohort, 535 met the following criteria: (1) disease duration of 7 years or greater, (2) seropositive, and (3) hand radiographs available. Patterns in specif

Small joint fusions of the hand using the Herbert Bone Screw.

The Herbert Bone Screw has a thread at both ends and was originally developed for internal fixation of fractured scaphoid bones. The two main advantages of compression and no head, make it most suitable for internal splinting of the small joints of the hand. The authors have used this device on thirty-nine joints in ten different sites of the hand, and it has proved to be an extremely successful method.

In vivo evidence of lumbricals incursion into the carpal tunnel in healthy hands: An ultrasonographic cross sectional study.

INTRODUCTION: During finger flexion, the tendons of flexor digitorum profundus migrate proximally, along with their attached lumbrical muscles. This incursion was suggested to extend into the Carpal Tunnel. Ultrasonographic imaging can be used to assess in vivo soft tissue behavior and incursion. PURPOSE OF THE STUDY: To clinically quantify the lumbrical muscles incursion in different finger positions. STUDY DESIGN: Cross sectional, observational study. METHODS: The lumbricals of 20 healthy adults with no history of hand injuries were evaluated with neuromuscular ultrasound imaging (n = 160

[Radiosynoviorthesis of the thumb's carpometacarpal joint].

Radiosynoviorthesis (RSO) is an established therapeutic method for the local treatment of pain in aseptic joint inflammation (e.g. arthritis, activated osteoarthritis, synovitis). RSO can be used for the treatment of synovial membrane inflammation of the finger joints such as the thumb's carpometacarpal joint. The beta emitter Erbium-169 (Er-169) is injected into the joint space, which irradiates the inflamed synovialis, thereby leading to fibrosis and obliteration of the pain receptors of the synovial membrane. The chances of success in the treatment of the thumb's carpometacarpal joint by RS…

Emergency reconstruction of a collateral ligament of a metacarpophalangeal joint using Dacron material.

We present a case in which an open wound involving the ulnar collateral ligament of the metacarpophalangeal joint of the little finger was treated by ligament reconstruction using a strip of Dacron material, nerve grafting and coverage by a posterior interosseous artery pedicled flap. At a long term follow-up of 4 years, the joint was stable and had a full range of movement.

Medial three-finger drop in high-energy forearm trauma: An unrecognized peripheral nerve injury. A report of two cases.

CASE: Absent active extension at metacarpophalangeal (MCP) joints of medial three fingers with intact extension of index finger and thumb following high energy forearm trauma due to recurrent branch of posterior interosseous nerve (RBPIN) injury has not been reported yet. The aim is to highlight an unrecognized sequel of a commonly encountered forearm trauma in two patients who sustained fractures around the elbow and forearm. CONCLUSION: In the acute traumatic setting, medial three-finger drop due to RBPIN injury can be missed or misdiagnosed. This has medico-legal and prognostic implication

Outcome of conservative management of spiral/long oblique fractures of the metacarpal shaft of the fingers using a palmar wrist splint and immediate mobilisation of the fingers.

Conservative management of spiral/long oblique fractures of the shaft of the metacarpal almost always results in shortening and hence these fractures have the reputation of ending up with an extension lag and reduced grip strength. In a prospective study, 42 patients with 54 fractures were treated with a palmar wrist splint and immediate mobilisation of all finger joints. All fractures united. Extension lag of fractured fingers was initially seen in all fingers, but eventually recovered. Grip strength progressively increased to reach a mean of 94% of the contralateral hand by 1 year after inju…

The effects of adding splint use to corticosteroid injection for the treatment of trigger finger: A randomized controlled trial.

BACKGROUND: Trigger finger is the most common flexor tendinopathy affecting the general population. We evaluated the effects of adding a static metacarpophalangeal joint splint to corticosteroid injection for the management of trigger finger in the short term. METHODS: We carried out a randomized controlled trial with two parallel arms in Department of Physical Medicine and Rehabilitation at a university hospital. We randomly allocated 60 participants (34 women) with trigger fingers other than the thumb to two groups (both n = 30). The mean (SD) age was 41.5 (7.6) years. All participants rece
