Paced QRS morphology and lead location in left bundle branch area pacing assessed by photon-counting cardiac computed tomography.

BACKGROUND: The relationship between the intra-septal lead position in left bundle branch area pacing (LBBAP) and paced QRS morphology remains incompletely understood. We used ultra high-resolution photon-counting computed tomography (PC-CT) to evaluate lead-tip location. OBJECTIVE: We retrospectively analyzed 20 patients who underwent contrast-enhanced PC-CT after LBBAP implantations (9 with left bundle branch pacing [LBBP], 9 with left ventricular septal pacing [LVSP], and 2 with deep septal pacing [DSP]). METHODS: Distances from the lead-tip to the LV and right ventricular (RV) endocardiu

Conduction system pacing for cardiac resynchronization therapy: a systematic review.

Biventricular pacing (BVP) remains the standard method for delivering cardiac resynchronization therapy (CRT) in patients with heart failure and left bundle branch block (LBBB). Despite its established role, BVP is a non-physiological pacing approach and is limited by factors such as anatomical constraints of the coronary sinus and a substantial proportion of non-responders. Conduction system pacing (CSP), including His bundle pacing and left bundle branch area pacing, has emerged as a more physiological alternative aimed at optimizing CRT outcomes. Early studies suggest that CSP may achieve s

Pacing system analyzers to guide conduction system pacing implantation procedures: A comparison study of intracardiac and surface signals with an electrophysiology recording system.

BACKGROUND: Left bundle branch area pacing (LBBAP) needs confirmation of left conduction system capture assessed by testing of different electrical parameters. Guidelines recommend the use of an electrophysiology recording system (EP-RS) to guide conduction system pacing procedures. However, some experienced centers perform LBBAP procedures without an EP-RS. OBJECTIVE: We aimed to assess whether LBBAP criteria can be measured by the signals provided by the pacing system analyzers (PSAs) as a surrogate for the EP-RS to simplify and universalize the technique. METHODS: This was an observationa

[Value of old and new electrocardiography criteria for differential diagnosis between ventricular tachycardia and supraventricular tachycardia with bundle branch block].

In 240 tachycardias with wide QRS-complex (> or = 120 ms), the value of the "old" ECG criteria published in 1978 by Wellens and coworkers was compared to the "new" ECG criteria published in 1991 by Brugada and coworkers for the differential diagnosis of tachycardias with wide QRS-complex. As main result of the study, it could be demonstrated that with the old as well as with the new ECG criteria a sensitivity of more than 90% for the correct diagnosis ventricular tachycardia can be reached. The specificity of the old and new ECG criteria for the diagnosis ventricular tachycardia is 70% and 72%

Difficulties in the use of electrocardiographic criteria for the differential diagnosis of left bundle branch block pattern tachycardia in patients with a structurally normal heart.

In patients with left bundle branch block pattern tachycardia, electrocardiographic criteria, based on leads V1, V2 and V6, have been shown to be effective for the diagnosis of ventricular tachycardia in patients with a previous myocardial infarct. To test these criteria on a wider population, we studied 53 consecutive patients with left bundle branch block pattern tachycardia. Seventeen patients had supraventricular tachycardia and 36 had ventricular tachycardia, 18 with a previous myocardial infarction, two with cardiomyopathy, and 16 with a normal heart. The sensitivity for the diagnosis of

Functional bundle branch block: discordant response of right and left bundle branches to changes in heart rate.

Faster heart rates shorten refractoriness more in some tissues than in others. This study investigates whether faster heart rates shorten relative refractoriness more in the right than left bundle branch in humans. Premature atrial stimulation at 2 or more basic cycle lengths was performed in 314 patients with no evidence of atrioventricular conduction system disease. In 10 patients, both functional right and left bundle branch block (BBB) developed with premature atrial stimulation. Functional right BBB occurred at the longer basic cycle length, and functional left BBB at the shorter cycle le

Exposure of concealed right bundle branch block in Wolff-Parkinson-White type B by pacing from the vicinity of the A-V node.

In two infants with Wolff-Parkinson-White type B, right bundle branch block was concealed during sinus rhythm and pacing from close to the atrial entrance of the right-sided accessory pathway. However, pacing from the vicinity of the A-V node, the A-V node itself, and the His bundle exposed the right bundle branch block by producing exclusive ventricular activation through the normal, A-V nodal His-Purkinje pathway. In addition, pacing from close to the A-V node also resulted in fusion beats characterized by absence of delta waves with (pseudo) normal QRS complexes and short H-V intervals. Fal

Pathologic correlations in three cases of bilateral bundle branch disease with unusual electrophysiologic manifestations in two cases.

Examination of the conduction system in three patients with bifascicular block who had electrophysiologic studies forms the basis for this report. Patients 1 and 2 had left bundle branch block and Patient 3 right bundle branch block and left axis deviation. The H-V interval was prolonged in each case (70, 65 and 60 msec, respectively). Serial section examination of the conduction system revealed sclerodegenerative involvement of both bundle branches in all cases. In Case 1, atrial extrastimulus testing converted left to right bundle branch block; in Case 2, it delineated a sinus echo zone with

[Left bifascicular block and percutaneous mitral valvuloplasty].

In some patients, the inflation of balloons through the mitral orifice during percutaneous mitral valvuloplasties may impair intraventricular conduction. In some cases, this appears to correspond to a block of the anterior and middle network within the left branch of the bundle of His. This left "biblock" is characterized by extreme left axial deviation of AQRS, of about -60 to -70 degrees, with a QS aspect on D2 and D3, RS on V1 and R on V6 with no major prolongation of ventricular activation time. If this hypothesis is correct, it would support the "trifascicular" concept of the left branch …

Decremental atriofascicular accessory pathway with bidirectional conduction: delineation of atrial and ventricular insertion by radiofrequency current application.

A 17-year-old girl with a corrected complex congenital heart disease and recurrent episodes of supraventricular tachycardia was referred for catheter ablation. Electrophysiologic studies revealed the presence of an accessory pathway (AP) with bidirectional conduction and decremental properties. We demonstrated a course parallel to the node-His AV conduction system. Transient abolition of the bidirectional conduction through the AP was obtained by radiofrequency application to the ventricular insertion located in the distal right bundle branch and to the atrial insertion, located in the mid-ant

Unusual ventricular activation produced by temporary transvenous cardiac pacing: electrovectorcardiographic findings.

Complete heart block (CHB) results from dysfunction of the cardiac conduction system, which results in complete electrical dissociation. The ventricular escape rhythm can have its origin anywhere from the atrioventricular node to the bundle branch-Purkinje system. CHB typically results in bradycardia, hypotension, fatigue, hemodynamic instability, syncope, or even Stokes-Adams syndrome. Escape rhythm originating above the bifurcation of the His bundle (HB) produces narrow QRSs with relatively rapid heart rate (HR) (except in cases of His system disease). We present a middle-aged man with an HR

Implantable cardioverter defibrillator does not cure the heart.

A man with non-ischemic cardiomyopathy, EF 22%, permanent AF and ICD was admitted for elective device replacement. The need for the optimization of the ventricular rate and avoidance of right ventricular pacing made it necessary to up-grade the existing pacing system using direct His bundle pacing and dual chamber ICD. This enabled the regularization of ventricular rate, avoiding the RV pacing and optimize the beta-blocker dose. The one month follow-up already showed reduction in left ventricle diameter, improvement in ejection fraction, NYHA class decrease to II. The His bundle pacing enabled

Optimizing conduction system pacing lead placement utilizing the image overlay technique.

The His-bundle has several locations from which conduction system pacing can be achieved. Some locations offer better sensing, thresholds and paced QRS durations. Existing techniques to aid repositioning of an already deployed, but sub-optimally placed lead, include either simple memory of the initial lead position combined with its observation on an x-ray review screen or utilizing an additional vascular access and pacing lead with the first lead serving as a real-time marker (Two-lead technique). We describe a novel, readily available, cost-efficient, imaging-based approach to assist in the

Idiopathic ventricular tachycardia. An intracardiac electrical, hemodynamic and angiographic assessment of six patients.

Six patients with idiopathic paroxysmal ventricular tachycardia were investigated by external electrocardiography, intracardiac electrography and pacing, exercise testing, cardiac catheterization and coronary angiography. All had normal hemodynamics and coronary arteries. His bundle electrography proved ventricular origin in five; one had no paroxysmal ventricular tachycardia during His bundle electrography. Treatment with diphenylhydantoin, 4.2 to 8.0 mg/kg/day, and propranolol, 0.8 to 2.7 mg/kg/day, appeared effective, well tolerated therapy in three, and procainamide and propranolol in one

Bundle branch reentrant ventricular tachycardia.

Bundle branch reentrant (BBR) tachycardia is an uncommon form of ventricular tachycardia (VT) incorporating both bundle branches into the reentry circuit. The arrhythmia is usually seen in patients with an acquired heart disease and significant conduction system impairment, although patients with structurally normal heart have been described. Surface ECG in sinus rhythm (SR) characteristically shows intraventricular conduction defects. Patients typically present with presyncope, syncope or sudden death because of VT with fast rates frequently above 200 beats per minute. The QRS morphology duri

[Bundle-branch block depending on the heart rate].

Twenty nine patients are described with transitory bundle branch block due to medicamentosus treatment of 386 patients with paroxysmal tachycardia. The block lasted from 1 to 48 hours and faded without any drug therapy. Three of the patients followed up for about two years died suddenly (very likely rhythm death), 8 had no further paroxysmal tachycardia, 18 patients had from 8 to 12 paroxysms during that period and in only 6 (20.7%) -- a second transitory bundle branch block recorded. The transitory bundle branch block, depending on heart rate could be explained by the latently existing lesion

Radiofrequency ablation of anteroseptal accessory pathway--a challenge to the electrophysiologist.

INTRODUCTION: Anteroseptal accessory pathways (APs) are located in the apex of the triangle of Koch's connecting the atrial and ventricular septum in the region of the His bundle. Ablation of anteroseptal pathway locations remains a challenge to the electrophysiologist due to a very high risk of transiet or permanent atrioventricular (AV) block. CASE REPORT: A male, 18-year-old, patient was hospitalized due to radiofrequency (RF) ablation of APs. He was an active football player with frequent palpitations during efforts accompanied by dyspnea and lightheadedness, but without syncope. Electroc

Transient entrainment of bundle-branch reentry by atrial and ventricular stimulation: elucidation of the tachycardia mechanism through analysis of the surface ECG.

BACKGROUND: Different responses to entrainment have been reported in relation to the pacing site of a variety of tachycardias. However, transient entrainment of bundle-branch reentrant tachycardia (BBRT) has not been investigated systematically. METHODS AND RESULTS: We attempted entrainment of 13 BBRTs in 9 patients by pacing first the right ventricle and then the right atrium. The initial pacing cycle length (CL) was 10 ms faster than the tachycardia CL. Subsequent pacing sequences were performed with 5- to 10-ms CL decrements until tachycardia termination or loss of postatropine 1:1 AV cond
