BACKGROUND: A 78-year-old male was admitted to our institute with increasing shortness of breath and decreased exercise tolerance. His increasing symptoms were not relieved with medical management. He had a complex medical history that included aortic valve replacement (AVR). Echocardiography showed a deteriorating aortic bioprosthesis with severe aortic regurgitation.
METHOD: Intraoperative extraction of this prosthesis proved technically challenging and a valve in valve successfully was implanted as a salvage procedure.

Sutureless bioprosthetic valves are typically used in patients with a normal-sized aortic root and annulus because of concerns that the stent portion of the valve will not be properly anchored. This report describes an initial case series of sutureless valve implantation in the setting of a diseased aortic root and ascending aortic aneurysm, and it shows that the use of the Perceval valve (LivaNova PLC, London, UK) is feasible in these complex situations.

We present an uncommon case of intraoperative transient myocardial ischemia related to suspicion of left main stem artery extrinsic compression caused by the radial force of the stent of Perceval® bioprosthesis. No other cases have been described, but some similarities can be found with extrinsic coronary compression in percutaneous pulmonary valves. Surgeons should be aware of this complication in patients with coronary anomalies or with bicuspid anatomy.
