[Cochlear implants].

Cochlear implants aim at the rehabilitation of profound bilateral deafness. The cochlear implant is a prosthesis made out of surgically implanted cochlear electrodes connected to an external vocal processor. The external acoustic signals are converted into electrical signals coded by the vocal processor. They are then sent out, by a transcutaneous mode, to an internal receptor. This receptor transmits the information to the intracochlear electrodes. Initially, the cochlear implantation was recommended to patients totally deaf following a trauma, a degenerative disease of the inner ear, a menin

A semiology-driven framework for explorations of temporal lobe epilepsy.

OBJECTIVE: Temporal lobectomy is effective for drug-resistant temporal lobe epilepsy (TLE), but broad resections risk neuropsychological morbidity. Stereo-electroencephalography (SEEG) supports hypothesis-driven exploration of seizure networks, where semiology-together with imaging, scalp EEG, and neuropsychology-anchors implantation strategies without serving as a sole classifier. We aimed to define common SEEG implantation patterns in TLE from a stereotaxic functional-anatomic perspective and test their reproducibility. METHODS: We analyzed 72 patients (60 main, 12 validation) with drug-res

Elimination of lead dislodgement by the use of tined transvenous electrodes.

Pacemaker lead dislodgement has accounted for a large proportion of the postoperative complications seen after transvenous pacemaker insertion. Ninety-two patients underwent implantation of a tined transvenous electrode over a three-year period without a single dislodgement. Excellent thresholds were obtained and no difficulties related to electrode insertion were encountered. Tined transvenous pacemaker leads are preferred for routine use at this time.

Hearing Preservation and Efficacy Outcomes in CI632 Recipients with Sloping High-Frequency Hearing Loss: Multicenter Trial Results.

INTRODUCTION: Adults with substantial residual low-frequency (LF) hearing are increasingly considered for cochlear implantation. This study evaluated hearing preservation and speech performance outcomes using a slim perimodiolar electrode (CI632) in adults under expanded indications. METHODS: In a prospective, open-label, single-arm feasibility trial at three US tertiary centers, ten adults with LF thresholds ≤50 dB HL at 500 Hz, sloping high-frequency loss, and aided CNC scores ≤60% in the ear to be implanted received the CI632 device. Participants were followed for 12 months postactivation.

Ventricular pacing from the coronary sinus of a patient with a Fontan circulation.

Permanent pacing of the left ventricle was achieved by transvenous implantation of a steroid-eluting electrode into the coronary sinus in a patient who had undergone a Fontan operation. This approach to endocardial pacemaker implantation is potentially of considerable value in patients who do not have transvenous access to the right ventricle.

Medical devices of the head, neck, and spine.

There are many medical devices used for head, neck, and spinal diseases and injuries, and new devices are constantly being introduced. Many of the newest devices are variations on a previous theme. Knowing the specific name of a device is not important. It is important to recognize the presence of a device and to have an understanding of its function as well as to be able to recognize the complications associated with its use. The article discusses the most common and important devices of the head, neck, and spine, including cerebrospinal fluid shunts and the Codman Hakim programmable valve; s

Hearing habilitation with auditory brainstem implantation in two children with cochlear nerve aplasia.

Patients with aplasia and hypoplasia of the cochlear nerve have no chance of having their hearing restored by stimulating the periphery of the auditory system using the traditional cochlear implant. A possible approach to auditory rehabilitation may be direct electrical stimulation of the cochlear nuclei with an auditory brainstem implant (ABI). Recently, two children, aged 4 and 3 years, respectively, with bilateral severe cochlear malformations and cochlear nerve aplasia received an ABI. The present paper reports the technique and the preliminary results of this experience. The classic retro

Tachyarrhythmias associated with programmable automatic atrial antitachycardia pacemakers.

A multiprogrammable automatic antitachycardia pacemaker (Cybertach-60), connected to a tined "J" wire electrode, was positioned in the right atrial appendage in three patients with recurrent supraventricular tachycardia (SVT). Mechanisms of SVT included atrioventricular (AV) nodal reentry (two patients) and reciprocating tachycardia using an accessory pathway (one patient). Indications for implantation included frequent symptomatic episodes, multiple drug failures, and reproducible termination of the arrhythmia by the automatic pacemaker during electrophysiologic studies. Following discharge,

Electrophysiological Characteristics of Epidural Spinal Signals in Preclinical Models of Spinal Cord Stimulation.

OBJECTIVES: Epidural stimulation of the spinal cord evokes distinct electrophysiological responses that can be recorded epidurally. Here, we characterized evoked compound action potentials (ECAPs), doublets (secondary or tertiary ECAPs, likely of different physiological origin than primary ECAPs), evoked synaptic activity potentials (ESAPs), and electromyographic (EMG) signals in preclinical models. Our objective was to clarify the features and distinct physiological origins of these signals, in order to advance mechanistic studies and support clinical applications of spinal cord stimulation (

Effects of acute hippocampal stimulation in the nonhuman primate penicillin model of temporal lobe seizures.

Asynchronous distributed multielectrode stimulation (ADMES) is a novel approach to deep brain stimulation for medication resistant temporal lobe epilepsy that has shown promise in rodent and in vitro seizure models. To further evaluate its effects on a pre-clinical model, we characterized the effect of unilateral ADMES in an NHP model of temporal lobe seizures induced by intra-hippocampal injection of penicillin (PCN). Four non-human primates were used for this study in two contemporaneous cohorts. One cohort (n = 3 hemispheres) was implanted with the Medtronic RC + S stimulation (GIN cohort)

An Invasive Method for the Activation of the Mouse Dentate Gyrus by High-frequency Stimulation.

Electrical high-frequency stimulation (HFS), using implanted electrodes targeting various brain regions, has been proven as an effective treatment for various neurological and psychiatric disorders. HFS in the deep region of the brain, also named deep-brain stimulation (DBS), is becoming increasingly important in clinical trials. Recent progress in the field of high-frequency DBS (HF-DBS) surgery has begun to spread the possibility of utilizing this invasive technique to other situations, such as treatment for major depression disorder (MDD), obsessive-compulsive disorder (OCD), and so on. Deployable electrode arrays for brain interfaces: structural reconfiguration strategies for long-term stability and high-fidelity recording-a review.

Objective.Neural electrode arrays, as essential tools for recording and stimulating neural tissues, significantly impact therapeutic strategies for neurological disorders through deep brain stimulation, responsive neurostimulation, and brain-computer interfaces. Despite considerable advancements, the efficiency and longevity of neural electrode arrays are compromised by brain micromotion, induced by physiological activities such as cardiac pulsation and respiration. The mechanical mismatch between rigid electrode arrays and soft neural tissue generates persistent stresses at the electrode-tiss

The use of active fixation electrodes for permanent endocardial pacing via a persistent left superior vena cava.

Two patients underwent permanent endocardial pacing for complete atrioventricular block. In each case a persistent left superior vena cava was either suspected or known to be present. An active fixation electrode was passed down the left superior vena cava and the tip positioned in the apex of the right ventricle. Stable ventricular pacing was achieved for the follow-up period of approximately six months. With the availability of such active fixation electrodes the presence of a persistent left superior vena cava no longer mandates insertion of an endocardial electrode via the right superior v

Cochlear implantation in a patient with osteogenesis imperfecta.

Osteogenesis imperfecta (OI) is a connective tissue disorder characterized by a deficit in the synthesis of type I collagen. Hearing loss affects 42-58% of OI patients and progresses to deafness in 35-60% of these patients. For OI patients, cochlear implantation (CI) is the only promising treatment option. However, literature on CI in patients with OI is relatively rare. After CI, speech perception is generally good. However, among patients with severe demineralization of the cochlea, most patients are reported to have complications of facial nerve stimulation (FNS), preventing some patients f

Threshold-distance functions predict speech recognition with cochlear implants.

Cochlear implantation shows unexplained outcome variability. Among the key factors affecting outcomes measured by speech understanding is the functional state of the auditory nerve and the amount of its degeneration due to hearing loss. In the present study we actively varied the distance of the stimulating electrodes from the modiolus (the spiral ganglion), quantified it using fluoroscopy and cone-beam computed tomography and related it to electrically-evoked compound action potentials (eCAPs) in human subjects. Stimulation was monopolar. The distance from modiolus could explain up to 91% of
