Elevated salivary and synovial fluid beta2-microglobulin in Sjogren's syndrome and rheumatoid arthritis.

Beta2-Microglobulin is normally present in low concentrations in serum and other bodily fluids. By use of a radioimmunoassay, elevated concentrations of beta2--microglobulin were found in saliva and synovial fluid from patients with Sjogren's syndrome and rheumatoid arthritis, autoimmune inflammatory diseases that attack and destroy the salivary glands and articular tissues, respectively. Elevated beta2-microglobulin concentrations decreased in the saliva of two patients who simultaneously showed a clinical response to systemic treatment. Measurement of beta2-microglobulin in inflammatory flui

Pathophysiology of Acute Liver Failure.

Acute liver failure (ALF) is a rare syndrome resulting from an acute insult to the liver in patients without known underlying chronic liver … known. Secondary MOF is often a result of the initial massive proinflammatory response generating a systemic inflammatory response syndrome followed by a compensatory anti-inflammatory response leading to immune cell dysfunction and sepsis. As the liver is a tremendously important metabolic organ involved in energy metabolism, protein synthesis, fat metabolism, and glycemic control, mul …

Influenza SIRS with Minimal Pneumonitis.

Although systemic inflammatory response syndrome (SIRS) is a known complication of severe influenza pneumonia, it has been reported very rarely in patients with minimal parenchymal lung disease. We here report a case of severe SIRS, anasarca, and marked vascular phenomena with minimal or no pneumonitis. This case highlights that viruses, including influenza, may cause vascular dysregulation causing SIRS, even without substantial visceral organ involvement.

Severe acute pancreatitis concurrent with lethal rupture of cerebral aneurysm: A case report and review of the literature.

INTRODUCTION: With high incidence and mortality, severe acute pancreatitis (SAP) is an inflammatory disease of pancreas. When concurrent with systemic inflammatory response syndrome (SIRS), multiple organ failure syndrome (MODS) or pancreatic encephalopathy (PE), it will significantly augment the mortal rate. Herein, we report the first SAP case complicated with fatal rupture of cerebral aneurysm and pre-existing cerebral arteriovenous malformation; meanwhile, numerous examinations indicated the occurrence of SIRS and MODS. CASE PRESENTATION: A 34-year-old male was admitted for these complain…

Role of DAMPs in respiratory virus-induced acute respiratory distress syndrome-with a preliminary reference to SARS-CoV-2 pneumonia.

When surveying the current literature on COVID-19, the "cytokine storm" is considered to be pathogenetically involved in its severe outcomes such as acute respiratory distress syndrome, systemic inflammatory response syndrome, and eventually multiple organ failure. In this review, the similar role of DAMPs is addressed, that is, of those molecules, which operate upstream of the inflammatory pathway by activating those cells, which ultimately release the cytokines. Given the still limited reports on their role in COVID-19, the emerging topic is extended to respiratory viral infections with focu…

Multi-omics biomarkers in post cardiac arrest syndrome: From pathophysiological mechanisms to the potential prognostication.

Post cardiac arrest Syndrome (PCAS) is a systemic ischemia-reperfusion injury (IRI) that occurs after the return of spontaneous circulation (ROSC), characterized by endothelial dysfunction, systemic inflammatory response syndrome (SIRS), and multiple organ failure (MOF). This review focuses on the pathophysiological mechanisms, dynamic change patterns, and prognostic predictive value of three key categories of biomarkers in PCAS: inflammatory factors, endothelial injury biomarkers, and neuronal injury biomarkers. It covers the dual roles of inflammatory mediators such as Interleukin-6 (IL-6), …

Longitudinal Profiling Reveals Divergent Kidney and Brain Immune Responses during Metabolic Syndrome.

Chronic kidney disease (CKD) and cerebral small vessel disease (cSVD) share major cardiometabolic risk factors, including hypertension, diabetes, and obesity. The immune system may play a key role in kidney-brain interactions and disease development. However, whether circulating inflammatory mediators reflect similar immune responses in the kidney and brain remains unclear. We therefore characterized circulating, renal and cortical cerebral responses in a rat model of metabolic syndrome. Lean and Obese ZSF1 rats were studied at 8-9, 22-23, and 34-35 weeks of age. Systemic inflammation was asse

Circular RNAs at the crossroads of acute pancreatitis: bridging molecular mechanisms to diagnostic and therapeutic innovations.

Acute pancreatitis (AP) is a severe inflammatory condition that frequently leads to systemic inflammatory response syndrome and multiple organ failure. Despite the increasing occurrence, targeted therapies remain unavailable. CircRNAs have recently been implicated in AP pathogenesis, regulating biological processes such as apoptosis, pyroptosis, and inflammation through mechanisms including miRNA sponging, protein scaffolding, and translation. Their disease-specific roles make them promising candidates for clinical translation. This review highlights the potential of circRNAs as novel diagnost…

Novel inflammatory markers predict cardiovascular events after percutaneous coronary intervention in patients with acute coronary syndrome.

INTRODUCTION: Inflammation plays a crucial role in the pathophysiology of acute coronary syndrome (ACS) and is associated with the risk of major adverse cardiovascular events (MACE). Novel inflammatory biomarkers such as the Systemic Immune-Inflammation Index (SII), Systemic Inflammatory Response Index (SIRI), and Systemic Immune-Inflammation Response Index (SIIRI) have recently been proposed as potential novel inflammatory biomarkers that may offer a valuable insight into cardiovascular risk prediction. OBJECTIVE: To evaluate the association between inflammatory and immune-inflammation indic

THE NEUROENDOTHELIAL AXIS IN TRAUMATIC BRAIN INJURY: MECHANISMS OF MULTIORGAN DYSFUNCTION, NOVEL THERAPIES, AND FUTURE DIRECTIONS.

Severe traumatic brain injury (TBI) often initiates a systemic inflammatory response syndrome, which can potentially culminate into multiorgan dysfunction. A central player in this cascade is endotheliopathy, caused by perturbations in homeostatic mechanisms governed by endothelial cells due to injury-induced coagulopathy, heightened sympathoadrenal response, complement activation, and proinflammatory cytokine release. Unique to TBI is the potential disruption of the blood-brain barrier, which may expose neuronal antigens to the peripheral immune system and permit neuroinflammatory mediators t…
