Needle placement with transtympanic electrocochleography.

Electrocochleography (ECoG) is an objective, electrophysiologic test useful in the clinical diagnosis of endolymphatic hydrops, or Meniere's disease. The purpose of this study was to determine if the position of the needle, using transtympanic methodology, gives a variable SP/AP (summating potential/action potential) response. SP/AP ratios were obtained during routine tympanoplasty procedures. After the tympanic membrane remnant was removed using a lateral graft technique, precise needle placement was obtained at the medial and lateral round window niches, as well as on the promontory. SP/AP r…

Menière's disease: evolution of a definition.

In 1861 Prosper Menière separated patients with episodic vertigo, hearing loss and tinnitus from a group previously described as having apoplectiform cerebral congestion. He suggested the cause was disease within the semicircular canals (Menière, 1861). Over the years it became apparent that within this group there were a number of patients with characteristic signs and symptoms and in 1938 a pathological correlate was found in the form of endolymphatic hydrops. Descriptions such as Menière's 'disease', Menière's 'syndrome' and Menière's 'symptom complex' led to a confusing array of terms for …

[A proposition for mutual genesis of hearing loss, recruitment and ear murmurs in the case of acute inner ear alterations (author's transl)].

Pathological changes on the stereocilia of cochlear hair cells observed by Bredberg et al. (1972) after noise exposure and in the early stages of antibiotic intoxication are suggested to cause a mechanical decoupling between the tectorial membrane and the hair cells. During acute episodes of endolymphatic hydrops a similar decoupling is suggested to occur, although for different reasons. Harris (1968) calculated the noise generated at the input to the hair cells on account of the Brownian motion of air particles in front of the tympanic membrane. For tight coupling, he obtained a level of (--)…

Cochlear conductive hearing loss in patients with Meniere's disease.

OBJECTIVES: Physiological studies with experimental models of endolymphatic hydrops in Meniere's disease demonstrate some pressure changes at the level of round and oval window. Interestingly, conductive type hearing loss was observed in some patients during Meniere episodes. A close auditory follow-up of these patients may provide a better understanding of hydrodynamic changes of the hydrops and micromechanics of the inner ear. PATIENTS AND METHODS: Eighty-four patients (mean age 34.3 years; range 17 to 77 years) with Meniere's disease were enrolled in the study. Duration of Meniere symptoms…

[Serum antibodies to type II collagen and immune complex in cases of Menière's disease].

In a previous study, endolymphatic hydrops was induced in 36% of guinea pigs immunized with type II collagen and a dominant negative SP was recorded from these hydropic animals. These results suggested that the immune response to type II collagen may have a role in the etiopathology of some cases of Menière's disease. In this study, we evaluated the levels of serum antibody to type II collagen and three other immune indexes in patients with Menière's disease, especially those who showed negative SP dominance on electrocochleography. Twenty-nine Menière's cases, 22 normal volunteers serving as …

Significance of endolymphatic sac surgery with and/or without simultaneous cochlea implant surgery in respect of vertigo control and speech perception in patients with Menière's disease.

PURPOSE: The focus on treating patients with Menière's Disease (MD) lies on the reduction of vertigo attacks and the preservation of sensory function. Endolympathic hydrops is considered as an epiphenomenon in MD, which can potentially be altered by endolymphatic sac surgery (ESS). Purpose of the study was to investigate the influences on vertigo control through manipulation of the perilymphatic system with or without ESS. METHODS: Retrospective data analysis of 86 consecutive patients with MD according to current diagnostic criteria after endolymphatic sac surgery alone (ESSalone; n = 45), c

Expression of histamine receptors in the human endolymphatic sac: the molecular rationale for betahistine use in Menieres disease.

The human endolymphatic sac (ES) is situated in a duplicature of the dura in the posterior cranial fossa and constitutes a part of the inner ear. The sac possesses immunological capacities and is responsible for a major part of the trans-epithelial ion transport occurring within the inner ear, via molecular mechanisms similar to that of the kidney collecting duct epithelia. Dysfunction of the trans-epithelial ion transport has been hypothesized as the reason for the endolymphatic hydrops occurring in Menieres diseases. Thus, candidate drug selection for medical treatment of Menieres disease ha…

Normal Serum Levels of Otolin-1 in Patients with Meniere Disease in Remission.

Introduction  Degenerative changes in the otolithic organs have been theorized to be caused by the mechanical obstruction to endolymphatic flow, possibly resulting in endolymphatic hydrops (ELH). Otolin-1 is an otoconial matrix protein that crosses the blood labyrinth barrier and has been found in the serum of healthy and diseased patients. Objective  To measure the serum levels of Otolin-1 in Meniere disease (MD) patients and compared them with the healthy individuals. Methods  This pilot, cross-sectional study was performed at our tertiary care referral center to compare the serum Otolin-1 l…

Imaging endolymphatic space of the inner ear in vestibular migraine.

BACKGROUND: Vestibular migraine (VM), the most frequent episodic vertigo, is difficult to distinguish from Ménière's disease (MD) because reliable biomarkers are missing. The classical proof of MD was an endolymphatic hydrops (EH). However, a few intravenous gadolinium-enhanced MRI studies of the inner ear (iMRI) also revealed an EH in VM. The major questions were the frequency and distribution characteristics of EH in VM for diagnostic use. METHODS: In a prospective case-control study of 200 participants, 75 patients with VM (49 females; mean age 46 years) and 75 with MD (36 females; mean ag…

Evaluation of the vestibular aqueduct in Ménière's disease using multiplanar reconstruction images of CT.

OBJECTIVE: Previous studies, in which the vestibular aqueduct (VA) was determined using axial CT, have indicated that the VA sizes in patients with Ménière's disease were smaller than those in the subjects of the control group and that 25-35% of ears with Ménière's disease were "non-visible type." In this study, in addition to obtaining the axial size, we measured the VA size along the vertical plane by using vertical multiplanar reconstruction (MPR) images. METHODS: The VA size of both ears of patients with unilateral Ménière's disease (n=34) and of subjects of the control group (n=30) was m

A comparison of the furosemide and glycerol tests for Meniere's disease. With special reference to the bilateral lesion.

The feasibility of furosemide test for the detection of endolymphatic hydrops has previously been discussed (Authors, 1973, 1975). The glycerol test also has been reported as being effective for the same purpose but only in Meniere patients with fluctuating hearing loss (Klockhoff & Lindblom, 1966). In 48 patients with Meniere's disease, both the furosemide test (F-test) and the glycerol test (G-test) were performed on 51 ears including 3 cases of bilateral involvement. The average value of urine volume in the F-test was significantly greater than that for the G-test. The decrease in tinnitus …

Clinical application of MIIRMR as a salvage method in gadolinium-enhanced MRI after intra-tympanic injection.

BACKGROUND: Three-dimensional fluid-attenuated inversion-recovery (3 D-FLAIR) and real inversion-recovery (3 D-real IR) sequences are used to detect endolymphatic hydrops (EH), but medium inversion-time inversion-recovery imaging with magnitude reconstruction (MIIRMR) may be more sensitive. AIMS: We investigated the inner-ear visualisation success rate and EH detection rates of 3 D-FLAIR and 3 D-real IR, and whether salvage MIIRMR could improve EH detection. MATERIALS AND METHODS: Fifty-one patients (102 ears) with episodic or chronic vestibular syndrome were injected intra-tympanically with…
