The main aim of this study was to perform a complex biomechanical analysis for a custom-designed temporomandibular joint (TMJ) prosthesis in … ht testing machine. Two methods of fixing the implant to the bone were analyzed-using three or five bone screws. The greatest stress was located on the top of the head of the prosthesis. The stress on the prosthesis with the five-screw configuration was lower than in the prosthesis with the three-screw configuration. The peak load analysis shows that the samp …

The purpose of the study was to compare the stability of the plate osteosyntheses of intra-articular calcaneal fractures using various types of a sustentacular screw insertions. A geometrical model of a calcaneal fracture was created. The fracture was fixed with a plate and screws with a uniform distribution. The individual models differed regarding the position of the sustentacular screw. The screw was inserted using three different variants: Model A: into the tip of sustentaculum tali, Model B: under the sustentaculum tali, and Model C: into the inferior peripherial rim of the sustentacular

Miniscrew-assisted rapid palatal expansion (MARPE) extends non-surgical, predominantly skeletal expansion to skeletally mature patients. Its success depends on the palatal miniscrews achieving sound engagement in a safe site and on the expander and the screws fitting together accurately. Two sequences are described. In a bone-first sequence, the miniscrews are inserted first, commonly through a CAD/CAM (computer-aided design/computer-aided manufacturing) guide that controls trajectory and depth, after which the appliance must be fitted to the screws in the positions they actually occupy. In an

BACKGROUND: Metacarpal shaft fracture is a common fracture in hand trauma injuries. Surgical intervention is indicated when fractures are unstable or involve considerable displacement. Current fixation options include Kirschner wire, bone plates, and intramedullary headless screws. Common complications include joint stiffness, tendon irritation, implant loosening, and cartilage damage.
OBJECTIVE: We propose a modified fixation approach using headless compression screws to treat transverse or short-oblique metacarpal shaft fracture.
MATERIALS AND METHODS: We used a saw blade to model transver

OBJECTIVE: The study aims to evaluate the biomechanical properties of feline femora with craniocaudal screw-hole defects of increasing diameter, subjected to three-point bending and torsion to failure at two different loading rates.
STUDY DESIGN: Eighty femoral pairs were harvested from adult cat cadavers. For each bending and torsional experiment, there were five groups (n = 8 pairs) of increasing craniocaudal screw-hole defects (intact, 1.5 mm, 2.0 mm, 2.4 mm, 2.7mm). Mid-diaphyseal bicortical defects were created with an appropriate pilot drill-hole and tapped accordingly. Left and right f

OBJECTIVE: To compare the stability of a corticopedicular posterior fixation (CPPF) device with traditional pedicle screws for decompression and fusion in adult degenerative lumbar spondylolisthesis.
METHODS: Finite element analysis (FEA) was used in a validated model of grade 1 L4-L5 spondylolisthesis to compare segmental stability after laminectomy alone, laminectomy with pedicle screw fixation, or laminectomy with CPPF device fixation. A 500-N follower load was applied to the model and different functional movements were simulated by applying a 7.5-Nm force in different directions. Outcome

BACKGROUND: Implant-associated infection remains a major complication of orthopedic surgery. The treatment of such infection is complicated by bacterial biofilm formation on the metal surfaces of implants. Biofilm surrounds and protects the bacteria against the organism's endogenous defense system and from external agents such as antibiotics and mechanical debridement. This study aims to evaluate whether freezing nitrogen ethanol composite (FNEC), the combination of liquid nitrogen and 95% ethanol in a 3 to 1 ratio, used frequently in bone tumor surgery, is capable of disinfecting Staphylococc

We experienced failures in attempting to remove a Gamma nail and a long Gamma nail, both of which were used for fixation of a subtrochanteric fracture in two young patients. We suspect that new bone forms in the notch, which is located beneath the screw thread in the lag screw, and causes jamming of the lag screw in young patients after fracture consolidation.

BACKGROUND: The medial malleolar osteotomy is required in some cases of osteochondral lesions, bone tumors, and fractures of the medial talar dome for surgical access. The literature is uncertain about which type of osteotomy and fixation is best. The objective of this study was to compare, in cadaveric specimens, the oblique and chevron medial malleolar osteotomies and their fixation with either 2 or 3 screws, evaluating the possibility of articular displacement. We hypothesize that the chevron osteotomy results in a lower chance of articular displacement at the end of the procedure than the

INTRODUCTION: We have developed the single or double endplates penetrating screw (SEPS/DEPS) technique, which is a novel percutaneous pedicle screw (PPS) insertion technique suitable for osteoporotic vertebral body fracture (OVF) patients with diffuse idiopathic skeletal hyperostosis (DISH). This study aims to compare the effectiveness of this SEPS/DEPS technique with the conventional pedicle screw technique.
METHODS: The screw is inserted upward from the outer caudal side of the pedicle toward the inner cranial side. Vertebrae affected with DISH were inserted with screws using the SEPS/DEPS

BACKGROUND: To evaluate the effectiveness and safety of robot-aided percutaneous triangular osteosynthesis combined with close reduction for vertically unstable sacrum fractures (VUSFs).
METHODS: The data on 21 patients of the VUSF were retrospectively analyzed from November 2016 to January 2021. According to Denis classification, there were 3 cases in zone I, 11 cases in zone II, and 7 case in zone III. The main perioperative indicators were recorded. The maximal angulation and displacement deviations of the screws were analyzed by comparing the planned trajectory with the actual position. P

BACKGROUND: Interference screw is commonly used for graft fixation in anterior cruciate ligament (ACL) reconstruction. However, previous studies had reported that the insertion of interference screws significantly caused graft laceration. The purposes of this study were to (1) quantitatively evaluate the graft laceration from one single insertion of PEEK interference screws; and (2) determine whether different types of sutures reduced the graft laceration after one single insertion of interference screws in ACL reconstruction.
METHODS: The in-vitro ACL reconstruction model was created using p

The drilling bone may potentially cause excessive frictional heat, which can lead to local bone necrosis. This heat generation and local necrosis has been suggested to contribute to the resorption of bone around the placed screws, ending in loss of screw purchase in the bone and inadvertent loosening and/or the bone-implant construct. In vivo studies on this subject have inherent obstacles not the least of which is controlling the variables and real time bone temperature data acquisition. Theoretical models can be generated using computer software and the inclusion of known constants for the m

BACKGROUND: Clinical studies have identified threshold HU values above which the risk of lumbar implant instability increases significantly; however, these threshold values have a very wide range. Furthermore, there is a lack of rationale for HU value thresholds based on biomechanical models to predict fusion failure. The purpose of the study is to provide biomechanical evidence for using an HU threshold to predict lumbar fusion failure.
METHODS: A fused model of the L4-L5 lumbar segment with screws and an interbody cage was developed. The model was then numerically investigated under standar

The purpose of this study was to describe a newly developed procedure for temporary mandibulotomy and subsequent osteosynthesis. Clinical outcomes were evaluated, including complications and the impact on postoperative treatment, particularly postoperative radiotherapy. Twenty-four patients underwent temporary mandibulotomies for the surgical resection of malignancies located in the posterior oral or oropharyngeal region. All were treated with postoperative radiotherapy. An angulated median mandibulotomy was followed by osteosynthesis with three anchor screws directed towards the inferior aspe

PURPOSE: To describe a modification of Wright's technique for C2 translaminar screw fixation.
METHODS: Bilateral crossing C2 laminar screws have recently become popular as an alternative technique for C2 fixation. This technique is particularly useful in patients with anomalous anatomy, as a salvage technique where other modes of fixation have failed or as a primary procedure. However, reported disadvantages of this technique include breach of the dorsal lamina and spinal canal, early hardware failure and difficulty in bone graft placement due to the position of the polyaxial screw heads. Morscher, of Switzerland, has developed an anterior cervical spine plate system (THSP) that does not require screw purchase of the posterior cortex. This design eliminates potential neurologic complications usually associated with the anterior plate system, but maintains the mechanical advantages of internal fixation. The authors reviewed 13 consecutive patients in whom the THSP system was applied. Indications for the use of this device included acute trauma in three patients, trauma of more than 6 weeks' duration in five patients, and spondylosis in five patients. Fifteen plates and 58 screws

So far the operative treatment of bone fractures has resolved itself to immobilisation and bone fragment joining with the help of metal implants such as nails, screws, plates and wires. Once the bone had healed, they had to be removed. This action bears some risk, e.g. extremity disfunction, wound infection. This requires re-hospitalization and increases social cost of the treatment. These reasons gave rise to research on new, biodegradable materials. One of them is "Biofix" manufactures as threads or nails, employed when immobilising bone fragments. Application of "Biofix" allows early rehabi

PURPOSE OF THE STUDY To compare the treatment outcome of scaphoid facture fixation with one versus two Herbert screws (HBS). MATERIAL AND METHODS 72 patients underwent open reduction internal fixation (ORIF) following acute scaphoid fracture, and were followed prospectively by one surgeon. All fractures were Herbert & Fisher classification type B, the most common fracture lines being oblique (n=38) and transverse (n=34). Fractures with similar fracture lines were randomly assigned into two groups; fractures stabilized with one HBS (n=42) and fractures stabilized with two HBS (n=30). A specific
