Benign lesions of the proximal femur usually weaken the supporting framework in the femoral neck. Of seven patients aged 11 through 26 years with the diagnosis of fibrous dysplasia (four), aneurysmal bone cyst (two), and simple bone cyst (one), six were treated with curretage and an autogenous fibula strut graft in conjunction with a sliding hip screw. One patient was treated with only the curretage and strut graft. The functional results were excellent (five), good (one), and fair (one), with no local recurrence. In treating fibrous dysplasia, dysplastic bone should be supplemented with graft

When treating periprosthetic femur fractures, bicortical screw fixation around a medullary-filling femoral stem may present a technical challenge, potentially leading surgeons to opt for unicortical locking screws or cerclage wires. These solutions may not achieve desirable stability, particularly in osteoporotic bone. In this technical note, we describe a simple approach using bicortical small-fragment screws-in-washers through large-fragment plates to facilitate secure bicortical screw passage around the stem. We also present preliminary outcomes from a case series in which this technique wa

Using the Spider plate fixation device, limited wrist arthrodesis was performed on 30 patients over a 2-year period. Developed as an alternative to Kirschner wires and compression bone screws in limited intercarpal fusions, its rigid fixation was expected to yield better results. Five hand surgeons treated 24 men and 6 women with an average age of 45 years. Diagnosis included the following: scaphoid nonunion advanced collapse, scapholunate advanced collapse, scapholunate dissociation, midcarpal arthrosis, and Kienböck disease. The 8-hole regular Spider and 6-hole mini-Spider plates were applie…

The increased use of locking plates to treat difficult fracture scenarios has been advocated in patients that have reduced bone mineral density. One of these difficult fracture patterns, fixation of the distal fragment of a distal fibula fracture, may depend on unicortical and cancellous bony purchase. This study investigated the construct stiffness afforded by using locking and conventional plating schema in a cadaveric model. Overall, the data indicate that a locking plate construct with two distal unicortical screws was mechanically equivalent to standard plating with 3 distal screws. In ad

INTRODUCTION: The S2 alar-iliac (S2AI) screw is a modification of the traditional iliac fixation technique and has surgical and biomechanical benefits. However, there are significant regional neurovascular structures along the path of such screws. Therefore the current anatomic study was performed to better elucidate these relationships.
METHODS: Using fluoroscopy, S2AI screws were placed in 2 adult cadavers through a standard posterior midline exposure. The screw insertion point was placed 10 mm lateral to a line bisecting the S1 and S2 foramina, adjacent to the sacroiliac joint. Using 30- t

BACKGROUND: Anterior cruciate ligament (ACL) ruptures have become increasingly common in pediatric and adolescent athletes. While multiple methods exist, all-epiphyseal ACL reconstruction is a popular technique in the skeletally immature patient. Given the high rate of reruptures in this population and the increasing number of commercially available fixation devices, biomechanical testing is crucial to understand the performance of these devices in pediatric epiphyseal bone. To our knowledge, there has not been a biomechanical analysis of ACL fixation devices in skeletally immature bone.

INTRODUCTION: This technical note presents, to the authors' knowledge, the first reported case of a hybrid pedicle-cortical screw system for instrumented fusion in a patient with congenital vertebral column deformity.
CASE: Cortical screws were navigated using stereotactic guidance to extend a prior non-segmented fusion mass, facilitating instrumentation in a circumstance with completely distorted anatomy. This technique provided a safe trajectory with excellent cortical purchase in an anatomically deformed spine.
DISCUSSION: Cortical screw fixation may serve to be helpful in augmenting pedi

When obtaining surgical fixation of lateral malleolus fractures, a cortical lag screw is commonly used to obtain anatomic reduction. Subsequently, a neutralization plate is applied. Slight loss of fracture reduction after plate placement occasionally occurs. Although this is frequently attributed to poor bone quality or suboptimal initial lag screw fixation, a frequently overlooked factor is screw order when applying the neutralization plate. The purpose of this technique tip is to highlight the biomechanical rationale behind this loss of reduction and advocate a specific screw order for later

While ACL reconstruction using single-tunnel and single-bundle techniques generally yields good clinical results, more and more studies are now reporting results that are not entirely satisfactory, as this type of reconstruction only exerts control over forward tibial shifting, not tibial rotation, when activities that exert high functional demands are undertaken. As a result, recent years have seen the appearance of numerous techniques for anatomic ACL reconstruction that reproduce both the anteromedial and posterolateral bundles of the ligament and therefore offer potentially improved rotati

OBJECTIVE: The favorable properties of silicon nitride (Si3N4) ceramics, such as high mean strength level and fracture toughness, suggest biomedical use as an implant material. Minor reservations about the biocompatibility of Si3N4 ceramics were cleared up by previous in vitro and in vivo investigations.
STUDY DESIGN AND SETTING: A Si3N4 prototype minifixation system was manufactured and implanted for osteosynthesis of artificial frontal bone defects in 3 minipigs. After 3 months, histological sections, computed tomography (CT) scans, and magnetic resonance imaging (MRI) scans were obtained.

Acetabular reconstruction cages are indicated for severe combined segmental and cavitary acetabular bone defects. The purpose of this study was to evaluate the implications of screw placement and drill plunge and the potential insult to anatomical structures when implanting acetabular reconstruction cages. A segmental cavitary defect was reamed into the acetabulum and a cage was implanted in each of the 10 hemipelvises. The relative course of the superior gluteal neurovascular bundle was mapped to assess dissection intervals. When cage screws were placed at least 15 mm longer than needed, 13%

This study investigates the optimal placement of locking plate screws for bone fracture stabilization in the humerus, a crucial factor for enhancing healing outcomes and patient comfort. Utilizing Finite Element Method (FEM) modeling, the research aimed to determine the most effective screw configuration for achieving optimal stress distribution in the humerus bone. A computer tomography (CT) scan of the humerus was performed, and the resulting images were used to create a detailed model in SOLIDWORKS 2012. This model was then analyzed using ANSYS Workbench V13 to develop a finite element mode

Interference screw in the fixation of autologous tendon graft to the bone tunnel is widely accepted for the reconstruction of anterior cruciate ligament (ACL), but the regeneration of fibrocartilaginous entheses could hardly be achieved with the traditional interference screw. In the present work, biodegradable high-purity magnesium (HP Mg) showed good cytocompatibility and promoted the expression of bone morphogenetic protein-2 (BMP-2) and vascular endothelial growth factor (VEGF), fibrocartilage markers (Aggrecan, COL2A1 and SOX-9), and glycosaminoglycan (GAG) production in vitro. The HP Mg

Many surgical treatments for chronic low back pain that is refractory to medical treatments focus on spine stabilization. One of the main surgical procedures consists of placing an interbody cage with bone grafts associated with pedicle screws [2, 25, 30]. This technique can be performed using different approaches: a large open posterior approach, tubular approaches (minimal open) or percutaneously (minimally invasive percutaneous or MIP) [5, 28]. One of the main difficulties is to precisely locate the screws into the pedicle avoiding especially infero-medial pedicle breaches. This difficulty
