β1-Adrenoreceptor Polymorphisms and Blood Pressure: 49S Variant Increases Plasma Renin But Not Blood Pressure in Hypertensive Patients.

BACKGROUND: Activation of beta-1 adrenoreceptor (β1-AR) in the kidney releases renin that plays a major role in the maintenance of blood pressure. Genetic variation in β1-AR could therefore alter the physiological and clinical effects of this hormone. We tested this hypothesis in patients from a primary care cohort being screened for primary hyperaldosteronism (n = 467). METHODS: Demographic and hemodynamic data were measured and plasma renin was determined by a standard immunoassay. Subjects were genotyped for the 2 common single-nucleotide polymorphisms Arg389Gly (rs1801253) and Ser49Gly (r…

Unanswered Questions in the Genetic Basis of Primary Aldosteronism.

Over the past six years, the genetic basis of a significant fraction of primary aldosteronism (PA) cases has been solved. Breakthrough discoveries include the role of somatic variants in the KCNJ5, CACNA1D, ATP1A1, and ATP2B3 genes as causes of aldosterone-producing adenomas (APAs), and the recognition of three novel hyperaldosteronism syndromes with germline variants in the KCNJ5, CACNA1D, and CACNA1H genes. The description of somatic variants in CACNA1D and ATP1A1 in aldosterone-producing cell clusters (APCCs) suggests that these clusters are precursors of some aldosterone-producing adenomas…

Detecting and treating primary aldosteronism: primary aldosteronism.

Primary aldosteronism (PA) is the most common cause of mineralocorticoid hypertension. Different studies, using the plasma aldosterone concentration to plasma renin activity ratio (PAC/PRA) for the screening of patients with hypertension, have shown a marked increase in the detection rate of PA. Idiopathic bilateral adrenal hyperplasia (IHA) and aldosterone-producing adrenal adenoma (APA), are the leading causes of primary aldosteronism. Glucocorticoid-remediable aldosteronism (GRA), also called familial hyperaldosteronism type I, familial hyperaldosteronism type II and carcinomas are rare cau…

Hypothyroidism affects the expression of electrogenic amiloride-sensitive sodium transport in rat colon.

BACKGROUND & AIMS: Both hyperthyroidism and hypothyroidism as well as hyperaldosteronism have been associated with changes of epithelial transport. The aim of this study was to evaluate the role of thyroid hormones in the regulation of electrogenic amiloride-sensitive sodium transport by aldosterone in the distal colon of immature and adult rats. METHODS: The changes in amiloride-sensitive short-circuit current (Isc) and Na+, K(+)-adenosine triphosphatase (ATPase) activity were measured in suckling, weanling, and adult euthyroid and methimazole-induced hypothyroid rats. RESULTS: The developm…

Endogenous prostacyclin synthesis is decreased during activation of the renin-angiotensin system in man.

Prostacyclin has been implicated as a mediator of renin release, whereas angiotensin II evokes prostaglandin I2 (PGI2) release from both vas … mal subjects when sodium intake was decreased from 150 to 10 meq/day. In five patients with primary hyperaldosteronism, PGI-M fell from 199 +/- 34 ng g creatinine-1 preoperatively to 120 +/- 26 pg/mg creatinine-1 after removal of the adenoma. In such patients, the reduction in PGI-M was associated with a significant increase in PRA. Thus, in both normal subjects and …

Abnormal membrane sodium transport in Liddle's syndrome.

We have documented the presence of abnormal sodium transport in Liddle's syndrome by measuring sodium concentration, sodium influx, and fractional sodium outflux in vitro in erythrocytes from normal subjects, two patients with Liddle's syndrome, and one patient with primary hyperaldosteronism. Sodium influx and fractional sodium outflux, but not sodium concentration, were significantly increased in patients with Liddle's syndrome. Sodium outflux in a patient with primary hyperaldosteronism did not differ significantly from normal. These alterations of sodium transport in erythrocytes from pati…

A mosaic mutation in the CLCNKB gene causing Bartter syndrome: A case report.

BACKGROUND: Type III Bartter syndrome (BS) is an autosomal recessive disease caused by mutations in the CLCNKB (chloride voltage-gated channel Kb) gene that encodes CLC-Kb. CLC-Kb is mainly located in the thick ascending limb of Henle's loop and regulates chloride efflux from tubular epithelial cells to the interstitium. Type III BS is characterized by metabolic alkalosis, renal salt wasting, hyperreninemia, and hyperaldosteronism with normal blood pressure. CASE PRESENTATION: We reported the case of a 3-day-old girl whose initial symptom we diagnosed as jaundice, but we accidentally found met…

Molecular Basis of Primary Aldosteronism and Adrenal Cushing Syndrome.

This review reports the main molecular alterations leading to development of benign cortisol- and/or aldosterone-secreting adrenal tumors. C … activating mutations are the main alterations in unilateral cortisol-producing adenomas. In primary hyperaldosteronism (PA), familial forms were identified in 1% to 5% of cases: familial hyperaldosteronism type I (FH-I) due to a chimeric CYP11B1/CYP11B2 hybrid gene, FH-II due to CLCN-2 germline mutations, FH-III due to KCNJ5 germline mutations, FH-IV due to CACNA1H …

Role of KCNJ5 in familial and sporadic primary aldosteronism.

Primary aldosteronism is characterised by the dysregulation of aldosterone production and comprises both sporadic forms, caused by an aldosterone-producing adenoma or bilateral adrenal hyperplasia, and familial forms (familial hyperaldosteronism types I, II and III). The two principal physiological regulators of aldosterone synthesis are angiotensin II and serum K(+), which reverse the high resting K(+) conductance and hyperpolarized membrane potential of adrenal glomerulosa cells. The resulting membrane depolarization causes the opening of voltage-gated Ca(2+) channels and an increase in intr…

Severe hypokalaemic paralysis and rhabdomyolysis due to ingestion of liquorice.

Chronic ingestion of liquorice induces a syndrome with findings similar to those in primary hyperaldosteronism. We describe a patient who, with a plasma K+ of 1.8 mmol/l, showed a paralysis and severe rhabdomyolysis after the habitual consumption of natural liquorice. Liquorice has become widely available as a flavouring agent in foods and drugs. It is important for physicians to keep liquorice consumption in mind as a cause for hypokalaemic paralysis and rhabdomyolysis.

[Conn's adenoma as a cause of hypertension].

The authors describe two observations of primary hyperaldosteronism, caused by a cortical adrenal adenoma (Conn's adenoma). Both patients were treated by unilateral adrenalectomy from lumbotomy. (In the second patient subtotal resection). The problem is discussed from the aspect of morphological diagnosis and location with emphasis on the primary importance of CT. The surgical operation is aimed and simple without the necessity of peroperative confirmation of the morphological background of the syndrome.

Hypomagnesemia associated with gentamicin therapy.

Two patients developed symptomatic hypomagnesemia most likely secondary to gentamicin therapy. Both patients were on regular doses of the antibiotic and had hypokalemia and hypocalcemia secondary to hypomagnesemia. Mechanisms postulated for the occurrence of hypomagnesemia secondary to gentamicin therapy include drug-induced hyperaldosteronism and tubular toxicity. Patients on gentamicin therapy should have serum magnesium, calcium, and potassium levels monitored routinely.

Nuances of Adrenal Metabolic Workup.

Adrenal function testing is recommended in all adrenal nodules 1 cm or larger with evaluation for pheochromocytoma, hyperaldosteronism, cortisol production, and androgen production as 40% of adrenal nodules are functional. Additional testing can be performed in patients with suspicious nodules by imaging. A multidisciplinary approach to evaluation and treatment of adrenal nodules is mandatory to consider the nuances involved in laboratory screening, confirmatory testing, and localization procedures.

[Diagnosis and treatment of the idiopathic edema syndrome].

Idiopathic edema is a little studied syndrome occurring in middle-aged women. Edemas are not associated with cardiac, liver or kidney pathology and are frequently caused by psychic trauma, infection or pregnancy. The disease pathogenesis is stipulated by hyperaldosteronism, antidiuretic hormone excess, estrogen-progesterone unbalance, initial disorder of the central hypothalamic regulation, etc. The treatment is mainly symptomatic; the disease course is non-malignant.

Progress on Genetic Basis of Primary Aldosteronism.

Primary aldosteronism (PA) is a heterogeneous group of disorders caused by the autonomous overproduction of aldosterone with simultaneous suppression of plasma renin activity (PRA). It is considered to be the most common endocrine cause of secondary arterial hypertension (HT) and is associated with a high rate of cardiovascular complications. PA is most often caused by a bilateral adrenal hyperplasia (BAH) or aldosterone-producing adenoma (APA); rarer causes of PA include genetic disorders of steroidogenesis (familial hyperaldosteronism (FA) type I, II, III and IV), aldosterone-producing adren…
