[Clinical management of severe ocular surface disease].

Severe ocular surface diseases, such as Stevens-Johnson syndrome, ocular cicatricial pemphigoid or severe ocular burns may result in a significant loss of corneal stem cells, eventually leading to vision impairment or even corneal blindness. In case of unilateral involvement, limbal autografting, by means of transplanting limbal stem cells from the healthy fellow eye, has proved to be an effective procedure for restoring the integrity of the ocular surface. Limbal allografts may be performed in patients with bilateral disease, however, systemic immunosuppression is mandatory in these cases, wi

No clear benefit of patient-specific instruments over conventional instrumentation in unicompartmental knee arthroplasty: A secondary analysis of a multicentre randomised trial with 5-year follow-up.

PURPOSE: The objective of the secondary analysis of this randomised controlled trial, conducted at two medical centres, was to compare the revision-free survivorship and patient-reported outcome measures (PROMs) of patient-specific instruments (PSI) versus conventional instrumentation (CI) used in unicompartmental knee arthroplasty (UKA) over a mid-term follow-up period of at least 5-years. The hypothesis was that both groups would show similar outcomes. METHODS: A total of 120 patients from two separate teaching hospitals were recruited and assigned at random to receive either PSI or CI for

Clinical and radiological outcomes of biportal endoscopic posterior cervical foraminotomy: a single-center retrospective cohort study with 1-year follow-up.

STUDY DESIGN: Retrospective single-center cohort study. PURPOSE: To evaluate 1-year clinical and radiological outcomes of biportal endoscopic posterior cervical foraminotomy (BE-PCF) in cervical spondylotic radiculopathy (CSR). OVERVIEW OF LITERATURE: CSR is commonly managed with anterior cervical discectomy and fusion, cervical disc replacement, or posterior cervical foraminotomy. However, these approaches may lead to fusion-related complications, prosthesis degeneration, or posterior tension band disruption. Minimally invasive techniques, such as biportal endoscopic spine surgery, aim to r

Smartphone-based digital templating using presentation software enhances accuracy in equalizing leg length and femoral offset during bipolar hemiarthroplasty for femoral neck fracture.

BACKGROUND: Restoring femoral offset (FO) and equalizing leg lengths are essential for optimal outcomes and preventing leg length discrepancy (LLD) in bipolar hemiarthroplasty (BHA) for femoral neck fractures (FNF) in elderly patients. However, evidence on the benefits of digital templating remains limited. This study evaluates the accuracy of a smartphone-based digital templating method for equalizing leg length and FO in BHA compared to the conventional intraoperative method. METHODS: A retrospective cohort study was conducted on 210 patients aged ≥ 60 years with FNF who underwent BHA betwe

The Use of Medial Pivot Prostheses for the Treatment of Moderate and Severe Deformities.

BACKGROUND: Total knee arthroplasty is the standard treatment for advanced knee osteoarthritis. Medial pivot (MP) prostheses aim to better replicate native knee biomechanics, but their efficacy in moderate to severe deformities remains unclear compared to posterior-stabilized (PS) designs or more constrained implants. METHODS: A retrospective cohort study was conducted, analyzing patients who had primary knee osteoarthritis and > 10° varus deformity who underwent total knee arthroplasty with either an MP or PS prosthesis between 2017 and 2020. A total of 164 patients (101 MP and 63 PS) met th

Outcomes of radial-sided wrist hemiarthroplasty: A systematic review.

PURPOSE: Wrist hemiarthroplasty (WHA) has gained popularity as a motion-sparing alternative to either total wrist arthroplasty or wrist arthrodesis for a variety of pathologies, yet limited data currently supports its use. This systematic review was performed to evaluate the clinical outcomes and complications following WHA, regardless of indication. METHODS: A systematic review was performed using PubMed, Cochrane, and Embase databases to identify relevant studies published through January 2023. Articles describing outcomes following WHA with a distal radius prosthesis, for any indication, w

Using mobile eye tracking to measure cognitive load through gaze behavior during walking in lower limb prosthesis users: A preliminary assessment.

BACKGROUND: Lower limb amputation does not affect only physical and psychological functioning but the use of a prosthetic device can also lead to increased cognitive demands. Measuring cognitive load objectively is challenging, and therefore, most studies use questionnaires that are easy to apply but can suffer from subjective bias. Motivated by this, the present study investigated whether a mobile eye tracker can be used to objectively measure cognitive load by monitoring gaze behavior during a set of motor tasks. METHODS: Five prosthetic users and eight able-bodied controls participated in

Octogenarians Are the New Sexagenarians: Cruciate-Retaining Total Knee Arthroplasty Is Not Inferior to Posterior-Stabilized Arthroplasty in Octogenarian Patients.

Purpose: The primary goal of this study was to compare survivorship and functional results in individuals aged 80 and over who underwent total knee arthroplasty (TKA) with cruciate-retaining (CR) or posterior-stabilized (PS) implants. Methods: We prospectively analyzed the clinical records of two consecutive cohorts for a total of 96 implants in patients aged 80 years or over. The first cohort consisted of 59 consecutive cemented PS cases, while the second cohort comprised 37 consecutive cemented CR cases. The decision to either perform a PS or CR arthroplasty was taken based on preoperative m

Segmental Motion of the Cervical Spine After Total Disc Replacement Using ActivC Versus Discectomy and Fusion Using Stand-alone Cage.

OBJECTIVE: Anterior cervical discectomy and fusion (ACDF) has been a widely accepted procedure for the treatment of cervical disc diseases. However, due to several reports regarding postfusion exacerbation of adjacent segments, a motion-preserving prosthesis was developed. In the present retrospective analysis, total disc replacement (TDR) using ActivC (Aesculap AG, Tuttlingen, Germany) was compared with ACDF using a stand-alone cage. METHODS: Among patients diagnosed with cervical disc diseases, those who received either ACDF or TDR at a single level between C3 and C7 from January 2010 to De
