Duane syndrome in association with congenital disorder of glycosylation type Ig (ALG12-CDG).

Congenital disorders of glycosylation type I (CDG-I) are a group of autosomal recessive genetic multisystem disorders that arise from defective glycoprotein biosynthesis. Although ocular abnormalities have been described in patients with CDG-I, few ocular abnormalities have been associated with ALG12-CDG (CDG-Ig), a rare subtype of CDG-I. We report a case of Duane syndrome, a congenital strabismus syndrome, in a 17-year-old young woman with ALG12-CDG.

[Advances in research of synergistic divergence].

Synergistic divergence is a rare condition characterized by complete absence of adduction that is associated with simultaneous abduction of the affected eye on attempted adduction. This causes the affected eye to move further into abduction on gaze to the normal side and results in extreme divergence of both eyes. It is sometimes referred to as exotropic Duane syndrome with synergistic divergence (type 4 Duane syndrome) or congenital fibrosis syndrome with synergistic divergence. Diseases combined with synergistic divergence fall within the category of congenital cranial dysinnervation disorde

Type 4 Duane syndrome.

PURPOSE: To identify cases of synergistic divergence whose characteristics suggest that this entity is a form of Duane syndrome. METHODS: The records of all patients with a Duane syndrome diagnosis, including standardized eye position photographs, from the E-Consultation program of Cybersight, Orbis International were analyzed. RESULTS: A total of 350 Duane syndrome cases were identified. Of these, 19 (5%) had features consistent with synergistic divergence, or type 4 Duane syndrome. Of the 19, 16 (84%) were male, 15 (79%) had palpebral fissure narrowing, all had anomalous head posture, and

A patient with de Morsier and Duane syndromes.

de Morsier syndrome, or septo-optic dysplasia, is a developmental malformation complex characterized by optic nerve hypoplasia, dysgenesis of the septum pellucidum, and hypothalamic-pituitary dysfunction. (1,2) In Duane retraction syndrome, there is absence of the sixth nerve nucleus with congenital retraction of the globe and narrowing of the lid fissure in adduction, frequent abduction deficiency, and variable limitation to adduction of the affected eye. (3) The purpose of this report is to present a patient with the uncommon and previously unreported concurrence of both of these congenital

The Wildervanck syndrome: cervico-oculo-acoustic dysplasia.

The Wildervanck syndrome consists of the Klippel-Feil deformity of the spine, eyeball retraction, lateral gaze weakness, and hearing loss. Conductive hearing loss, as well as a more frequently occurring sensorineural hearing loss, is caused by dysplasia of the inner ear. Typically, polytomograms demonstrate a bulbous vestibule and dilated lateral semicircular canals with basilar impression of the skull. The Duane's eyeball retraction phenomenon completes the syndrome; this may be explained either by misdirected innervation of the extraocular muscles or by atrophy or fibrosis of the lateral rec

Wildervanck or cervico-oculo-acoustic syndrome and MRI findings.

In 1952, Wildervanck described the first case of what he styled the cervico-oculo-acoustic (COA) syndrome. This comprises Klippel Feil's (KF) anomaly (congenitally fused cervical vertebrae), congenital sensorineural deafness and Duane's retraction syndrome (deficient abduction with retraction on adduction). Since that original paper, there have been further reports describing this triad, either completely or incompletely. A further case of this syndrome is reported and the first report of MRI head scan findings in this condition is presented. In addition, the origin of mirror movements observe

[Bilateral Duane syndrome associated with hypogonadotropic hypogonadism and anosmia (Kallmann syndrome)].

A case of bilateral Duane's retraction syndrome (DRS) type III associated with Kallmann's syndrome (hypogonadotropic hypogonadism and anosmia) is reported. Both anomalies may result from a disturbance of embryogenesis during the second month of gestation. The authors assimilate bilateral type III DRS to congenital horizontal gaze paralysis.

A unique case report of Wildervanck syndrome in association with facial nerve paresis and arachnodactyly.

Wildervanck syndrome, also called as cervico-oculo-acoustic syndrome. This is characterized by the triad of Klippel-Feil syndrome, Duane retraction syndrome, and congenital deafness. Other associations described are cardiovascular and musculoskeletal abnormalities. Some reports, in fact, state that nearly one percent of hearing-impaired females may be affected by Wildervanck syndrome. In our case report, a child who had the complete triad of the syndrome along with arachnodactyly (long slender digits), camptodactyly, short stature, and facial nerve paresis has been described.

Left-sided Duane's syndrome and retinal coloboma associated with contralateral microphthalmia.

Case report of a 15-year-old girl presenting with a left-sided Duane's retraction syndrome (DURS) in combination with ipsilateral retinal coloboma and contralateral microphthalmia. Abduction limitation as well as narrowing of the palpebral fissure and globe retraction during attempted adduction (type I DURS) was demonstrated in the left eye. Additionally, a retinal coloboma was observed in the nasal inferior quadrant of the left eye. No other somatic or developmental abnormalities were observed and karyotypic analysis was normal. This is the first case of non-syndromic unilateral DURS associat

Augmented vertical rectus transposition surgery with single posterior fixation suture: modification of Foster technique.

PURPOSE: To describe a modification of the lateral posterior fixation augmentation suture of the vertical rectus muscles that is proposed to enhance the abducting vector while balancing the vertical vectors, therefore limiting stress on the sclera and preventing vertical deviations. METHODS: Full tendon temporal transposition of the vertical rectus muscles was performed on 5 patients with Duane retraction syndrome and 5 with sixth (abducens) nerve palsy. Augmentation was created by the use of a single lateral fixation suture of 5-0 polyester that incorporated the muscle bellies of both vertic

Trigemino-abducens synkinesis: serial review over 4 years.

Congenital cranial dysinnervation disorders involve abberant connections between cranial nerves, often resulting in synkinetic activation of target muscles. Marcus-Gunn jaw winking ptosis and Duane retraction syndrome are some of the more well-known examples of this phenomenon. We present serial video of a child with congenital trigemino-abducens synkinesis, a lesser known congenital cranial dysinnervation disorder, to demonstrate the characteristics of this likely under-diagnosed presentation.

Refractive error in unilateral Duane syndrome.

PURPOSE: To investigate whether chronic co-contraction of the medial and lateral rectus muscles leads to higher against-the-rule astigmatism in affected versus fellow eyes in Duane syndrome. METHODS: A database of 4,103 patients of Dr. Arthur Jampolsky was queried for diagnosis of Duane syndrome. Inclusion criteria were unilateral Duane syndrome and complete data set. Refractive error was compared between Duane and fellow eyes. RESULTS: A total of 84 patients with unilateral Duane syndrome were identified. The spherical equivalent of Duane eyes and fellow eyes was similar (0.4 vs. 0.2 [P = 0

Paralytic strabismus and syndromes.

In the past year, there have been interesting communications regarding third nerve palsies in children and acquired fourth nerve palsy. Several interesting articles have confirmed the benign natural history of untreated Brown's syndrome and addressed its management. Two major reviews, one on vertical diplopia and one on Duane's retraction syndrome, have provided a wealth of information and useful bibliographies.

Superior Rectus Transposition Surgery: Safety, Efficacy, and Place in Therapy.

Abduction limitation in esotropic Duane retraction syndrome (DRS), esotropic Mobius syndrome, and sixth nerve palsy is one of the difficult-to-manage problems in strabismus surgery. The procedure of superior rectus transposition (SRT) was introduced by Johnston et al. In this procedure, the superior rectus (SR) muscle is disinserted and sutured adjacent to the insertion of lateral rectus (LR) muscle. The purpose of this review is to explore literature about efficacy and safety of SRT and its usage in strabismus surgery.

Imaging of congenital cranial dysinnervation disorders: What radiologist wants to know?

We aim to review the imaging features of congenital cranial dysinnervation disorders. Characteristic imaging findings can define subtypes of these disorders through assessment of cranial nerves, extraocular muscles, orbital, and brain abnormalities. Duane retraction syndrome shows absent or hypoplasic 6th cranial nerve and preserved extraocular muscles (EOM). Mobius syndrome shows absent 7th and 6th cranial nerves, absence of facial colliculus, flattening of the dorsal aspect of the pons, hypoplasia of the pons and medulla, and flattening of the 4th ventricular floor. Congenital fibrosis of th
