Does fingolimod in multiple sclerosis patients cause macular edema?

Multiple sclerosis (MS) is a demyelinating disease of the central nervous system and is the common cause of optic neuritis. Fingolimod, an immunosuppressive agent, is used in MS to prevent acute exacerbations. We report a case of relapsing-remitting MS treated with fingolimod. The patient presented with an acute decrease in vision in the left eye. Eye examination showed clinical macular edema (ME) in the left eye, which was confirmed on fluorescein angiogram and optical coherence tomography (OCT). After discontinuation of fingolimod and treatment with topical corticosteroid medication, there w

Treatment of macular edema due to retinal vein occlusions.

Retinal vein occlusion (RVO) is a prevalent retinal vascular disease, second only to diabetic retinopathy. Previously there was no treatment for central retinal vein occlusion (CRVO) and patients were simply observed for the development of severe complications, generally resulting in poor visual outcomes. The only treatment for branch vein occlusion (BRVO) was grid laser photocoagulation, which reduces edema very slowly and provides benefit in some, but not all patients. Within the past year, clinical trials have demonstrated the effects of three new pharmacologic treatments, ranibizumab, tria

Intravitreal triamcinolone for intraocular inflammation and associated macular edema.

Triamcinolone acetonide (TA) is a corticosteroid that has many uses in the treatment of ocular diseases because of its potent anti-inflammatory and anti-permeability actions. Intraocular inflammation broadly referred to as uveitis can result from several causes, including the immune system and after ophthalmic surgery. One of the most common reasons for vision loss with uveitis is macular edema. TA has been used for many years as an intravitreal injection for the treatment of ocular diseases. Several case control studies have been reported showing the efficacy of TA in the treatment of intraoc

Vitreomacular traction syndrome confused with pseudophakic cystoid macular edema.

Vitreomacular traction syndrome causes visual loss as a consequence of persistent traction on the fovea from an attached posterior hyaloid. The diagnosis can be challenging because the attachment may be subtle. Cystoid macular edema secondary to cataract extraction is well known and can occur in the absence of any surgical complication. Herein, a patient is described who had vitreomacular traction syndrome diagnosed after uneventful cataract surgery. Pars plana vitrectomy with detachment of the posterior hyaloid restored vision to an acceptable level. Careful examination and clinical suspicion

Axial and para-axial fluorophotometry in diabetic macular edema.

The vitreous fluorophotometric examinations were used in nonproliferative diabetic retinopathy to recognize early diabetic macular edema. Thanks to additional fixation light, axial and para-axial fluorophotometry in patients with nonproliferative diabetic retinopathy was achieved (Fluorotron Master, Coherent). The angle of deviation from the foveal scan (f) was 10 degrees in temporal (t) and nasal (n) directions. In case of non clinically significant macular edema, the posterior vitreous penetration ratio (PVPR) in the foveal scan (f) was significantly higher (p < 0.05) than in the other resul

Prognosis of cystoid macular edema after retinal detachment repair.

Twenty eyes of 20 patients who had undergone successful retinal detachment repair by means of microsurgery had cystoid macular edema (CME) disclosed by fluorescein angiography 10-12 weeks postoperatively. Clinical evaluation war repeated at 6-month intervals for a period of 12-30 months. Sixteen eyes were followed for at least 18 months. CME spontaneously cleared or dramatically decreased in 12 of the 16 eyes (75%), which were followed for at least 18 months. CME had spontaneously cleared in 50% of the eyes by the end of 1 year after surgery. For those eyes with transient CME, the mean visual

Macular edema in association with severe carotid artery obstruction.

In seven eyes of six patients (four men and two women, 55 to 72 years old), with macular edema associated with severe carotid artery stenosis, the ipsilateral carotid obstruction was 90% or more. Concomitant fundus signs, such as midperipheral retinal hemorrhages, retinal arterial narrowing, and neovascularization of the optic disk, were observed. All eyes demonstrated leakage of fluorescein dye from the retinal vessels and optic disk.

Cystoid macular edema following penetrating keratoplasty: incidence and prognosis.

The incidence of clinically significant and angiographically documented cystoid macular edema (CME) following penetrating keratoplasty was studied in a consecutive series of 102 eyes over an average follow-up of 30 months. CME was seen only in the aphakic eyes. Twenty out of 68 eyes in this group developed CME. There was a statistically significant higher incidence of CME in eyes undergoing vitrectomy than in eyes not undergoing vitrectomy (p less than .01). Also, there was a higher incidence of CME in eyes with aphakic bullous keratopathy (p less than .01) and pseudophakic bullous keratopathy

Aphakic macular edema: some observations on prevention and pathogenesis.

A prospective controlled trial of topical 0.1% dexamethasone drops was undertaken with the purpose of determining its efficacy in the prevention of aphakic macular edema. Of the 56 eyes studied specifically at the second and sixth postoperative weeks, none of the 28 in the steroid group showed macular edema, whereas the incidence in the nonsteroid group was 17.1% (P less than .01). The study highlights the role of steroids in reducing the incidence of postoperative macular as well as corneal edema. On the basis of earlier evidence and our observations on the conjunctival hyperfluorescence, the

Prophylaxis of aphakic cystoid macular edema using topical indomethacin.

The purpose of the present investigation was to define clinically the effect of topical indomethacin in preventing CME. The study was conducted on 50 patients who underwent bilateral intracapsular extraction of senile cataracts. One eye was used as the experimental subject and the other as the control. Postoperative alterations in the permeability of the blood-aqueous barrier and the chronological relationship of the changes in the retina and the blood-aqueous barrier were also studied. The mechanism underlying these postoperative ocular changes is discussed. In cases treated with indomethacin

Cystoid macular edema occurring 13 years after cataract extraction.

Thirteen years after cataract extraction, a patient complained of diminished visual acuity. In the interim visual acuity was 6/6 (20/20), or 6/7.5 (20/25+2). Fluorescein angiography demonstrated cystoid macular edema. Except for vitreous adherence to the inner surface of the cataract wound, no ocular cause for the macular edema was apparent.

Characteristics of macular morphology and microcirculation in diabetic macular edema patients with serous retinal detachment.

BACKGROUND: To analyze and compare the characteristics of macular morphology and microcirculation in diabetic macular edema (DME) patients with and without macular serous retinal detachment (SRD). METHODS: One hundred eyes in 81 patients diagnosed with the DME (the central macular thickness (CMT) of ≥ 300 μm) from March 2020 to November 2020 were selected. According to whether complicated with SRD, patients were divided into DME with SRD (60 eyes) and without SRD (40 eyes) groups. We analyzed the following parameters: CMT, central retinal thickness (CRT), subfoveal choroidal thickness (SFCT),
