Trends over time in prescribing by English primary care nurses: a secondary analysis of a national prescription database.

BACKGROUND: A growing number of countries legislate for nurses to have medication prescribing authority although it is a contested issue. The UK is one of these countries, giving authority to nurses with additional qualifications since 1992 and incrementally widened the scope of nurse prescribing, most recently in 2006. The policy intention for primary care was to improve efficiency in service delivery through flexibility between medical and nursing roles. The extent to which this has occurred is uncertain. This study investigated nurses prescribing activities, over time, in English primary ca

Cancer genetics in rural primary care: a pilot nurse-led service using a new mobile IT system.

Somerset has a relatively high incidence of cancer. The county is very rural with pockets of deprivation and sizeable areas of poor access to services. The Cancer Plan advocates early identification of cancer and identified the genetic revolution as having the potential to predict risk, and detect and diagnose cancer early. This innovative Public Health led pilot takes specialist cancer genetics services into primary care. It explores a new service model that is patient focused, provides services close to home and reduces hospital visits and pressure on genetics departments. People who believe

Generative AI Needs a Pilot: Positioning Advanced Practice Nurses as the "Human-in-the-Loop" for Primary Care Safety.

Generative artificial intelligence (GenAI) is increasingly integrated into primary care, supporting documentation, clinical summaries, and patient-facing communication. While its potential to reduce cognitive workload is widely recognized, GenAI introduces a safety paradox: as outputs become more fluent, inaccuracies such as hallucinations and omissions may become harder to detect. This creates a largely unrecognized burden in routine care, referred to as the "verification tax," which risks redistributing rather than alleviating clinician workload. A central question is not whether GenAI is su

Teaching Relational Care for Health Inequalities: Evaluating the Impact of a Program for Community Nurses and Midwives in Primary Care.

INTRODUCTION: Internationally, nurses need to learn and develop relational skills that enable them to engage therapeutically with patients. Relational skills are crucial in contexts of socioeconomic deprivation, where patients face high levels of adversity, inequality, and poor health. They are defined as the interpersonal skills needed to bring about clinical change, while also enhancing staff well-being and support. Educators continue to grapple with how best to design and deliver educational programs that enhance relational skills in the nursing workforce. We report on a realist evaluation

eHealth Literacy and Professional Attitudes as Predictors of Influenza Vaccination and Recommendation Among Spanish Primary-Care Nurses.

BACKGROUND: Influenza vaccination among Spanish Healthcare Workers (HCWs) remains below the 75.00% benchmark. Primary-care nurses are both recipients and gatekeepers, yet the roles of digital competence and professional attitudes remain unclear. We examined how eHealth literacy (eHL) and professional attitudes predict influenza vaccination uptake and recommendation among Spanish primary-care nurses. MATERIALS AND METHODS: We conducted a nationwide cross-sectional online survey (November 2024 to March 2025) of 526 primary-care nurses. Instruments included the Questionnaire on Professional Atti

Unleashing the Potential of Primary Care Nurses in Chronic Pain Management: A Delphi Study to Identify Priority Activities.

Chronic pain (CP) is a major cause of global disability, with patients often reporting inadequate access to primary care. Relevant primary care nursing activities for CP management are poorly defined, limiting the effective use of nurses' competencies in managing chronic diseases. This study aimed to identify and prioritize nursing activities for CP management. A three-round Delphi study was conducted among primary care nurses and individuals experiencing pain for over 3 months in Québec, Canada. In the first round, participants identified important nursing activities for CP management through

Gaps in the primary care of rural and underserved populations: the impact of nurse practitioners in four Mississippi Delta states.

PURPOSE: The shortage and distribution of primary care providers (PCPs) has left a gap in the health care of society, particularly those residents within the Lower Mississippi River Delta region, where pockets of rural and underserved populations reside. Described in this study are the workforce distribution, characteristics, and likelihood of advanced practice nurses (APNs) to practice in these communities. DATA SOURCES: Surveys were completed by 479 APNs in Arkansas, Louisiana, Mississippi, and Tennessee. Other sources included census data and the designation of Health Professional Shortage

Problems and needs for improving primary care of osteoarthritis patients: the views of patients, general practitioners and practice nurses.

BACKGROUND: Osteoarthritis (OA) is highly prevalent and has substantial impact on quality of life as well as on healthcare costs. The general practitioner (GP) often is the first care provider for patients with this chronic disease. The aim of this study was to identify health care needs of patients with OA and to reveal possible obstacles for improvements in primary care management of OA patients. METHODS: We performed semi-structured interviews with a stratified sample of 20 patients, 20 GPs and 20 practice nurses. RESULTS: Diagnosing OA posed no major problem, but during the course of OA,

Eliciting and Honoring What Matters Most in the Nursing Home: A Primary Palliative Care Approach to Improving End-of-Life Outcomes.

BACKGROUND: Nursing homes provide a substantial proportion of end-of-life care but often face fragmented communication, inconsistent care planning, and limited access to specialty palliative care. These gaps highlight the need for scalable primary palliative care models. METHODS: The authors conducted a descriptive quality improvement project across 28 community nursing homes in northern California. A multicomponent primary palliative care model included clinician education, nurse care navigation, and system-level supports. Outcomes included documented goals-of-care conversations, hospice len

The Scope of Care Coordination for Older Adults Whose Dominant Provider Is a Nurse Practitioner Versus a Primary Care Physician.

BACKGROUND: Nurse practitioners (NPs) play an increasingly prominent role in primary care, particularly in low-income and rural communities. Yet the differences in care coordination demands between NP-dominant and physician-dominant primary care remain unknown. As the number of providers involved in a patient's care increases, the complexity of care coordination also increases. OBJECTIVE: To determine if the scope of care coordination (ie, the number of other providers involved in a patient's care) varies by NP versus generalist physician. METHODS: In 2425 participants from the nationwide Re
