Percutaneous coronary intervention in left main coronary artery disease with or without intravascular ultrasound: A meta-analysis.

This meta-analysis compared IVUS-guided with angiography-guided PCI to determine the effect of IVUS on the mortality in patients with LM CAD. Current guidelines recommend intravascular ultrasound (IVUS)-guided percutaneous coronary intervention (PCI) in patients with left main coronary artery disease (LM CAD; Class IIa, level of evidence B). A systematic search of the MEDLINE, Embase, and Cochrane Central Register of Controlled Trials databases was conducted to identify randomized or non-randomized studies comparing IVUS-guided PCI with angiography-guided PCI in LM CAD. Ten studies (9 non-rand

Comparison of safety and efficacy of image-guided enema reduction techniques for paediatric intussusception: A review of the literature.

There is variable international practice regarding the technique used for image-guided enema reduction in paediatric intussusception. A recent meta-analysis found pneumatic reduction to be more effective than hydrostatic techniques. Clinical practice variation may impact on perforation rates, ionizing radiation exposure and requirement for surgery. The aim of this study was to review the literature regarding the efficacy and safety of fluoroscopically guided pneumatic reduction (FGPR) compared to ultrasound (US) guided intussusception reduction (USGIR) techniques for paediatric intussusception

Biliary tract anatomical variance - the value of MRCP.

Duplication of the common bile duct (CBD) is a rare congenital anomaly of the bile ducts that should be diagnosed prior to surgery in order to optimise management and prevent complications. We report a case of a patient presenting with choledocholithiasis and type Va duplicated extrahepatic bile duct that was missed on ultrasonography. The atypical course prompted further imaging with magnetic resonance cholangiopancreatography (MRCP), which identified the aberrant bile duct and assisted in safe preoperative and operative management. This case highlights the importance of accurate pre-interven

Endoscopic ultrasonography-guided drainage of the pancreatic duct.

Over the last 2 decades there has been continuing development in endoscopic ultrasonography (EUS). EUS-guided pancreatic drainage is an evolving procedure that can be offered to patients who are high-risk surgical candidates and in whom the pancreatic duct cannot be accessed by endoscopic retrograde pancreatography. Although EUS-guided pancreatic drainage is a minimally invasive alternative option to surgery and interventional radiology, owing to its complexity and potential for fulminant complications it is recommended that these procedures be performed by highly skilled endoscopists. Mechanism of topiramate-induced acute-onset myopia and angle closure glaucoma.

DESIGN: Interventional case report. METHODS: In an institutional practice setting, two women, aged 25 and 45, developed acute myopia after starting topiramate for epilepsy. One patient also developed bilateral angle closure glaucoma. RESULTS: Topiramate was discontinued. Anterior chamber shallowing was noted in both patients at presentation. Ultrasonography showed ciliochoroidal effusion. Baseline measurements of anterior chamber depth and lens thickness were obtained. CONCLUSIONS: Topiramate may be associated with ciliochoroidal effusion with forward displacement of the lens-iris diaphragm

Advances in Endoscopic Ultrasonography for Pancreatobilairy Diseases.

Endoscopic ultrasonography (EUS) has become an indispensable tool for the diagnosis and treatment of biliary and pancreatic diseases. Originally introduced for high-resolution imaging and tissue acquisition through fine-needle aspiration (EUS-FNA), the clinical role of EUS-FNA has expanded remarkably with the evolution of interventional EUS (iEUS). EUS-guided drainage procedures targeting the bile duct, gallbladder, pancreatic duct, and peripancreatic fluid collection have emerged as minimally invasive alternatives to traditional surgical or percutaneous approaches. The advent of the lumen-app

Varicocele and Interventional Radiology: A Comprehensive Case Study on Diagnosis, Therapeutic Intervention, and Implications for Reproductive Health.

Male infertility is known to be influenced by varicocele, a common vascular aberration marked by aberrant dilatation of the pampiniform plexus veins within the scrotum. In this instance, a 38-year-old man showed up with scrotal discomfort and swelling, which are clinical signs of varicocele. Scrotal ultrasonography provided diagnostic confirmation, confirming the existence of varicocele and identifying a concomitant minor hemorrhage in the left scrotal sac. Despite the initiation of conservative measures, such as bed rest, cryotherapy, and analgesics, the patient's clinical course was marked b

Endoscopic-assisted gland preserving therapy for the management of parotid gland sialolithiasis: Our preliminary experience.

OBJECTIVE: To analyze the efficacy and safety of sialendoscopy and a combined transoral or transcutaneous and sialendoscopic approach in the removal of parotid gland sialoliths. METHODS: This retrospective study included 29 patients diagnosed with parotid gland sialolithiasis who required endoscopic-assisted gland preserving therapy. Ultrasonography and computed tomography were used to diagnose parotid sialolithiasis. The use of interventional sialendoscopy, sialendoscopic-transoral, or sialendoscopic-transcutaneous procedures was determined by the characteristics of the parotid gland stones.

Ischemic events after carotid interventions in relationship to baseline cerebrovascular reactivity.

BACKGROUND: We aimed to investigate whether baseline cerebrovascular reactivity could predict subsequent ischemic event after intervention and identify the patient group for more aggressive medical and interventional management paradigms. METHODS: Patients with more than 70% cervical carotid stenosis (from ultrasonography) were reviewed. Patients, who had baseline cerebrovascular reactivity test before intervention and had either carotid endarterectomy (CEA) or carotid angioplasty and stenting (CAS) performed, were recruited for analysis. Transcranial Doppler ultrasonography was used to exami

A pilot feasibility study of awake laryngeal ultrasonography in children.

BACKGROUND: Flexible nasal laryngoscopy is the gold standard examination for laryngeal assessment, but can be uncomfortable, poorly tolerated or even refused by children. Ultrasonography is non-invasive and well-tolerated and could provide an alternative. Data in children, however, are scarce. AIMS: To assess the feasibility of in-office ultrasonography in children for the identification of laryngeal structures using reference imaging planes, and to determine factors influencing examination quality. MATERIAL AND METHODS: A single-center non-interventional prospective pilot feasibility study

Ultrasonographic diagnosis of parathyroid glands and percutaneous ethanol injection therapy.

The first choice for imaging diagnosis of parathyroid gland (PTG) abnormalities is ultrasonography with a high-frequency probe. The patient must be positioned correctly when performing either imaging or percutaneous ethanol injection (PEIT) of the PTG. The enlarged PTGs are examined by ultrasonic tomography using 3D measurements, and it is important to evaluate blood flow; the PTGs are hypervascular in comparison with a nodular lesion of the thyroid. Based on the 3D data, the estimated volume of the gland is calculated (a x b x c x pi/6), from which the volume of ethanol to be injected is dete

Ultrasound imaging in aesthetic medicine: safety and precision in injectable procedures.

Injectable procedures for aesthetic purposes are on the rise in recent years, and, with them, the number of related complications. Ultrasound (US) imaging is a reliable tool that can be used in conjunction with such procedures to minimize adverse outcomes. US may be used prior to the actual injection procedure to identify anatomical anomalies and for vascular mapping. US-guided injections allow to improve precision, prevent complications, reduce the injectable volume, and to treat complications. At follow-up, US imaging allows assessment of treatment success by visualizing filler placement but

Improvement of central vein ultrasound-guided puncture success using a homemade needle guide-a simulation study.

BACKGROUND: Out-of-plane (OOP) approach is frequently used for ultrasound-guided insertion of central venous catheter (CVC) owing to its simplicity but does not avoid mechanical complication. In-plane (IP) approach might improve safety of insertion; however, it is less easy to master. We assessed, a homemade needle guide device aimed to improve CVC insertion using IP approach. METHOD: We evaluated in a randomized simulation trial, the impact of a homemade needle guide on internal jugular, subclavian and femoral vein puncture, using three approaches: out-of-plane free hand (OOP-FH), in-plane f
