3D Knee Trochlear Morphology Assessment by Magnetic Resonance Imaging in Patients With Normal and Dysplastic Trochleae.

BACKGROUND: Trochlear dysplasia is recognized as a significant risk factor for patellar instability, but current imaging modalities fail to allow full visualization and classification of the complex 3-dimensional (3D) anatomy of the trochlea. The purpose of this study was to elucidate primary differences in trochlear morphology between patients with and without patellar instability by utilizing 3D magnetic resonance imaging (MRI) reconstructions of the trochlea. METHODS: An institutional review board-approved retrospective review included 24 patients with a diagnosis of patellar instability a

Patella alta is reduced following MPFL reconstruction but has no effect on quality-of-life outcomes in patients with patellofemoral instability.

PURPOSE: The primary purpose of this study was to determine if isolated medial patellofemoral ligament (MPFL) reconstruction for lateral patellofemoral instability altered the patellar height ratio. Secondary purposes were to use disease-specific quality-of-life scores to determine if MPFL reconstruction is as successful in patients with patella alta, compared to those without; and whether the change in the patellar height ratio after MPFL reconstruction is influenced by demographic and clinical factors. METHODS: Demographic and clinical data were collected pre-operatively on 283 patients wit

MRI of the knee with arthroscopic correlation.

Major advances in MRI and arthroscopy have allowed for enhanced diagnosis and subsequent management of ligamentous and soft tissue injuries of the knee. Recognition of the appearance of acute ACL and PCL injuries on MRI can enhance arthroscopic reconstruction. PCL injuries are often more subtle and can present with indirect signs. T2-weighted MRI imaging can examine which structures have been damaged in the posterolateral corner which may manifest arthroscopically as a drive-through sign. Characterization of PLC, meniscus, MCL injuries and OCD lesions on MRI have remarkable correlation with ar

Vertical dislocation of the patella: report of 2 cases.

Vertical dislocation of the patella is rare and usually occurs as a result of blunt trauma. The patella rotates around a vertical axis. It can be diagnosed by its typical clinical features. This article presents a case of 2 patients with vertical dislocation who presented with high-impact injuries and had classic signs of a vertically dislocated patella. Unlike lateral patella dislocation, reduction under sedation is usually not possible. This is mainly due to increased tension within the quadriceps mechanism, which makes reduction difficult. The other factor that makes reduction difficult is

Functional deficits and significant patellofemoral chondral deterioration two years after trochleoplasty and medial patellofemoral ligament reconstruction.

PURPOSE: Patellofemoral instability is a common knee condition and often associated with trochlear dysplasia. Sulcus-deepening trochleoplasty is a surgical option to reshape the trochlear groove and stabilise the patella. Although this procedure is considered safe and effective, the development of postoperative patellofemoral chondral lesions remains a topic of concern. The aim of this study was to evaluate the clinical and radiological outcomes following combined trochleoplasty and medial patellofemoral ligament reconstruction with a minimum follow-up of 2 years. METHODS: We assessed 23 pati

Outcomes After Isolated Medial Patellofemoral Ligament Reconstruction for Recurrent Patellar Instability: Influence of Persistent Postoperative Apprehension and J-Sign.

BACKGROUND: Patients with recurrent instability underwent isolated medial patellofemoral ligament (MPFL) reconstruction regardless of anatomic risk factors within the confines of strict exclusion criteria. Previous publications from this cohort have been limited in the ability to identify risk factors for failure of isolated MPFL reconstruction when recurrent instability was defined as the only mode of failure, likely because of the small patient numbers who experienced this outcome. PURPOSE: To investigate whether persistence of postoperative apprehension and J-sign indicate suboptimal outco

Thin flap sulcus-deepening trochleoplasty in patellar instability yields good functional outcomes without progressive cartilage deterioration in the short-term follow-up-A retrospective single-surgeon cohort study.

PURPOSE: Sulcus-deepening trochleoplasty (TP) effectively treats patellofemoral (PF) instability (PFI) caused by high-grade trochlear dysplasia (TD), but current evidence is based on small case series. We hypothesised, that TP would result in significant functional improvements and a low re-dislocation rate but would not accelerate the progression of PF cartilage deterioration. METHODS: We retrospectively reviewed all TP cases performed by a single surgeon between 2015 and 2021. Inclusion criteria were postoperative Magnetic resonance imaging (MRI) >6 and >12 months and a clinical follow-up >

Patellofemoral dysbalance and genua valga: outcome after femoral varisation osteotomies.

INTRODUCTION: Patellofemoral dysbalance may be caused by trochlear dysplasia, an elevated TTTG distance, femoral or tibial torsional deformities, patella alta, or a genu valgum. The surgical procedure for the treatment of a genu valgum is varisation osteotomy, usually in the femoral aspect. Several authors believe that a genu valgum is one cause of patellofemoral dysbalance, but studies about the outcome of the treatment with a varisation osteotomy are rare. MATERIALS AND METHODS: Nineteen knees in 18 patients, aged on average 28 (16-52) years were investigated in a retrospective study. The p

Alignment in the transverse plane, but not sagittal or coronal plane, affects the risk of recurrent patella dislocation.

PURPOSE: Abnormalities of lower extremity alignment (LEA) in recurrent patella dislocation (RPD) have been studied mostly by two-dimensional (2D) procedures leaving three-dimensional (3D) factors unknown. This study aimed to three-dimensionally examine risk factors for RPD in lower extremity alignment under the weight-bearing conditions. METHODS: The alignment of 21 limbs in 15 RPD subjects was compared to the alignment of 24 limbs of 12 healthy young control subjects by an our previously reported 2D-3D image-matching technique. The sagittal, coronal, and transverse alignment in full extensio

Medial patellofemoral ligament reconstruction: the superficial quad technique.

BACKGROUND: Medial patellofemoral ligament (MPFL) reconstruction is a routine procedure for patellar instability. The majority of the techniques require hardware fixation or a bony procedure at the patella. However, most of the complications described in the literature can be attributed to patellar fixation. The "superficial quad technique" uses the superficial slip of the quadriceps tendon as the graft material, which provides a better anatomic match to the native MPFL. HYPOTHESIS: The superficial quad technique provides anatomic patellar fixation without a bony procedure and without patella

Visual analog scale assessment after medial patellofemoral ligament reconstruction: with or without tibial tubercle transfer.

BACKGROUND: We studied the efficacy of medial patellofemoral ligament (MPFL) reconstruction, with or without tibial tubercle (TT) transfer, for recurrent dislocation of the patella, based on subjective functional assessment using the visual analog scale (VAS). METHODS: Forty-two consecutive knees of 40 patients who underwent MPFL reconstruction using a hamstring tendon were followed up for an average of 4.3 years. Twenty-nine knees underwent MPFL reconstruction (MPFL group) and 13 knees underwent MPFL reconstruction combined with medial transfer of TT (TT+MPFL group). Clinical results, includ

Effects of knee flexion and extension on the tibial tuberosity-trochlear groove (TT-TG) distance in adolescents.

BACKGROUND: Measurement of the tibial tubercle-trochlear groove (TT-TG) distance is used to assess patellofemoral instability and rotation. Since patellofemoral instability and acute patellar dislocation are common among adolescents, it is important to clarify the relationship between TT-TG distance and various flexion and extension angles in asymptomatic children. The purpose of the present study was to determine how knee flexion and extension influence TT-TG-distance values measured using 3D imaging in an anatomic axial plane among asymptomatic adolescents. METHODS: We performed magnetic re
