Walking swing limb advancement after transfemoral amputation relies on the hip for proper limb positioning. We compared hip mechanics and stride characteristics during swing before and after bone-anchored limb (BAL) implantation. We hypothesized swing limb advancement hip mechanics would be different and between-limb differences reduced after BAL implantation without compromising stride characteristics. Twenty participants were included in this retrospective observational investigation including overground walking motion capture before and one-year after BAL implantation. Swing was measured fr

Osseointegration of a bone-anchored limb (BAL) is an emerging rehabilitation technique that offers significant advantages over traditional socket prostheses. By addressing functional limitations and recurrent cutaneous complications, BAL systems have shown an 82%-90% increase in daily prosthesis use among patients, who also report improvements in functional ability, balance, comfort, and overall quality of life. Despite these benefits, the process of patient selection for BAL remains underdeveloped, with evidence-based guidelines still in their infancy. This article aims to propose a workflow

Severe maxillary atrophy is a significant challenge for implant-supported rehabilitation, often requiring complex reconstructive or adaptive procedures. Contemporary patient-specific subperiosteal implants, designed through digital planning and fabricated using titanium additive manufacturing technology, have re-emerged as a viable alternative in patients for whom conventional implants are not feasible. This report describes the management of a 45-year-old patient with a class III facial deformity and severe maxillary atrophy. An initial diagnostic phase was performed to achieve anterior and i

We compared the indices of satisfaction and use among patients wearing an osseo-integrated prosthesis BAHA (bone anchored hearing aid) according to the indications: conductive or mixed hearing loss (CHL) and patients with single side deafness (SSD). The study was carried out among patients wearing a BAHA fitted in one of three French departments between November 2001 and November 2005. Each patient received a postal questionnaire relating to the ease of use and the daily utilization period of the prosthesis, as well as a satisfaction rating (from 1 to 10) evaluating improvement in quality of l

BACKGROUND: Patients with transfemoral and transtibial amputations generally rely on socket-suspended (SS) prostheses for ambulation. The use of these aids can be complicated by poor fit, leading to tissue damage, pain at the socket-limb interface, and inability to ambulate. Osseointegrated implants (OIs) directly anchor a prosthesis to the patient's residual limb, eliminating these issues. However, they require customized components and additional surgeries. The purpose of this study was to conduct the first cost-benefit analysis of OI prostheses compared to SS prostheses for lower limb amput

INTRODUCTION: Transcutaneous Osseointegrated Prosthetic Systems (TOPS) offer a good alternative for patients who cannot be satisfactorily rehabilitated by conventional suspension sockets. The Endo-Exo-Prothesis (EEP, ESKA Orthopaedic Handels GmbH®, Deutschland) is the most implanted TOPS in Germany. Previous studies have shown that cortical thickness increases after implantation of TOPS. The aim of this study is to determine changes of cortical thickness in relation to the time after implantation of the Endo-Fix-Stem.
PATIENTS AND METHODS: All transfemoral amputees treated by EEP from 2007 to

INTRODUCTION: Clinical data on osseointegration (OI) for limb replacement indicates a concerning increase in mechanical complications after five years post-implantation. Since adequate bone-implant contact and proper implant alignment are critical factors for successful osseointegration, it is essential to identify the factors influencing these outcomes. This study aimed to assess the effects of residual femur length and implant sizing on bone-implant contact and implant alignment.
METHODS: Three-dimensional models of eight cadaveric femora were reconstructed from CT scans, and transfemoral a

Our aim was to evaluate neurosensory symptoms after lateralisation of the inferior alveolar nerve (IAN). We studied a retrospective case series with one-year follow up that included 139 procedures in 123 patients. After the IAN had been located it was deflected from the mandibular body and the implant placed. Sensitivity was mapped 24hours, one month, six months, and one year after the intervention by gently pressing the skin and lips with the tip of a probe. A total of 337 implants were placed in 123 patients aged between 44 and 68 years. There were 33 men and 90 women and they all recovered.

BACKGROUND: There is growing interest in suture-button devices for syndesmosis injury, which are intended to offer less rigid fixation than screw fixation.
METHODS: The fixation strength with 2 different suture-button devices, ZipTight and TightRope, were compared using 5 cadaveric leg pairs (n = 10). In an additional 5 pairs (n = 10), ZipTight was compared to 3.5 mm quadricortical screw fixation. Ankle motion was measured intact, then following simulated syndesmosis injury and fixation. Cyclic loads (peak 750 N, 7.5 Nm) were applied. Finally, external rotation to failure was measured and fai

Only a few plate systems are available for anterior fixation of thoracolumbar vertebrae because of the difficulty in fastening a screw and a plate together. If the fixation is inadequate, the screws will become loose. The Rigix plate system consists of screws made of titanium alloy and a plate made of pure titanium. All screws used for internal fixation are screwed into the plate. This system permits the use of anchor screws, which facilitate exertion of force to compress the vertebral bodies together or to distract them from each other. In this study, Rigix plates were used in 24 patients (20

PURPOSE: Elective removal of bone-anchored hearing implants and epistheses is uncommon, limiting the understanding of the biological factors contributing to adverse reactions, implant success or failure. Therefore, the tissue-level response at the implant interface remains poorly understood. METHODS: This study examined 21 explanted bone-anchored implants and interfacial tissues from 19 patients using a controlled retrieval protocol. The clinical data were correlated with histological, immunohistochemical, microbiological, and structural analyses. RESULTS: Pain (12/19) and infection (7/19) wer

BACKGROUND: Individuals who have undergone transfemoral amputation often have difficulty donning and utilizing a conventional socket prosthesis. Osseointegrated prostheses may alleviate certain issues, potentially resulting in better outcomes, but few prospective studies have either evaluated these outcomes or quantified the purported benefits.
QUESTIONS/PURPOSES: (1) Did patient-reported outcome measures improve between baseline and 2-year follow-up among those who underwent osseointegration surgery after transfemoral amputation? (2) What was the frequency and severity of early complications

Visual evaluation of bone changes around an osseointegration (OI) implant in femoral amputees examined on plain radiographs shows that periprosthetic bone resorption takes place during the first years after OI surgery, but the bone mineral density (BMD) change has not been previously quantified by dual-energy X-ray absorptiometry (DXA). Precision is vital when monitoring BMD changes around implants, and thus the aim of this study was to evaluate the precision and feasibility of a scan protocol for BMD measurements in proximity of OI implants.
