Fully implantable hearing device with transducer on the round window as a treatment of mixed hearing loss.

OBJECTIVE: To demonstrate the safety and efficacy of the Otologics Carina Middle Ear Transducer for treatment of mixed hearing loss through a case report. METHODS: A Carina fully implantable device with the MET transducer for conductive applications was implanted in a 48-year-old woman suffering from right mixed hearing loss (mean PTA loss: 80 dB). A facial recess approach was used to access the middle ear. Sclerotic tissue obliterated the stapes footplate so the approach selected was to place the transducer directly on the round window. The mounting bracket was placed on the mastoid and the

Long-term hearing results following stapedotomy.

Stapedotomy, or small fenestra stapes surgery, offers several advantages over total stapedectomy, including less postoperative vertigo and better closure of the air-bone gap at higher frequencies. Although early postoperative results are encouraging for the small fenestra technique, few data are available for longer follow-up periods. In this retrospective study, the hearing results of 35 stapedotomy patients, representing 37 operated ears, were analyzed. Each patient had a follow-up period ranging from 5 to 11 years, with audiograms obtained preoperatively, postoperatively, and at the longter

Insidious Cases of Enlarged Vestibular Aqueduct (EVA) Syndrome Resembling Otosclerosis: Clinical Features for Differential Diagnosis and the Role of High-Resolution Computed Tomography in the Pre-Operative Setting.

BACKGROUND: Enlarged vestibular aqueduct (EVA) syndrome can mimic otosclerosis in adults, presenting with an air-bone gap (ABG) and even absent stapedial reflexes. The ABG in inner-ear disorders is currently the object of several authors' studies and seems to be related to a third mobile window (TMW) phenomenon. This can lead to misdiagnosis and inappropriate treatment. Given that it would be inappropriate and harmful to perform CT scans in all patients with a clinical diagnosis of otosclerosis, this study aims to highlight some clinical features useful for the differential diagnosis between o

Histopathologic Analysis of Temporal Bones With Otosclerosis Following Cochlear Implantation.

OBJECTIVE: Analyze changes in osteoneogenesis and fibrosis following cochlear implant (CI) surgery in patients with otosclerosis and compare differences based on insertion technique. BACKGROUND: When advanced otosclerotic disease extends to the otic capsule, severe and profound sensorineural hearing loss necessitates consideration of a cochlear implant. Histopathological analysis of the human temporal bone after implantation in the patient with otosclerosis may reveal important variables that predict CI success. METHODS: Histopathological evaluation of archival human temporal bones from subj

Ossiculoplasty in chronic otitis media: Surgical results and prognostic factors of surgical success.

BACKGROUND AND OBJECTIVES: The goal of ossiculoplasty is to improve hearing. Successful ossiculoplasty depends on several factors. This retrospective study was carried out to analyze hearing results of ossiculoplasty in ears with chronic otitis media (COM) and evaluate clinical outcomes and factors predictive of hearing improvement. SUBJECTS AND METHODS: We reviewed the results of 153 patients with COM (with cholesteatoma (COMC) and without cholesteatoma (COMWC)) who underwent ossiculoplasty between January of 2002 to December of 2011. Several potential prognostic factors were evaluated: chol

Otosclerosis and measles virus - association or causation?

Otosclerosis is a frequent condition which occurs exclusively in the human temporal bone. This peculiar disease affects mainly Caucasians and Indians and may cause conductive, mixed conductive-sensorineural or occasionally merely sensorineural hearing loss. Morphological investigations of the otosclerotic focus show all three phases of a chronic inflammation with bone resorption, formation of new bone and finally eburnation. Various hypotheses about the cause of inflammation were proposed in the past. Immunological reactivity to collagen, the existence of otosclerosis genes (OTSC 1-5) includin

Phenotype description of a Dutch otosclerosis family with suggestive linkage to OTSC7.

This study reports on a clinical investigation of a Dutch family that shows suggestive linkage to OTSC7. Cross-sectional as well as longitudinal analyses of audiometric data were performed. Also, high-resolution computed tomography (CT) images of the temporal bones from genetically affected family members were obtained to study the incidence and extent of otospongiotic foci. Audiometric data showed a considerable degree of phenotypic variability. Cross-sectional regression analysis did not show age-dependent progression of bone conduction (BC), air conduction (AC), and air-bone gap (ABG) level

Long-term results of transmeatal cochleovestibular neurectomy: an analysis of 100 cases.

One hundred patients have been treated over a 13-year period with a transmeatal approach to the internal auditory canal for cochleovestibular neurectomy. Ninety-one of these patients have followup of more than 3 months (average, 4.67 years). The most common indication for the procedure was Meniere's disease (71%). Chronic labyrinthitis, usually following stapes, middle ear, or mastoid surgery, was the next most common indication. Patients with the preoperative diagnosis of Meniere's disease had better results in the curing of vertigo (89%) than those having chronic labyrinthitis (68%). Overall

Value of CT scan in the diagnosis of temporal bone diseases in children.

CT scan of the temporal bone has become a frequently ordered test for diagnosis of temporal bone pathology. We retrospectively examined our experience with CT scans of the temporal bones in 124 consecutive children from January 1983 to September 1984 in an attempt to assess its usefulness in diagnosis and treatment of ear disease in children. Patients were divided into six categories according to their pre-scan diagnosis, (trauma, congenital aural atresia, dizziness, facial nerve paralysis, middle ear disease, congenital sensorineural hearing loss) and CT findings were compared to data found b
