BACKGROUND: Transvenous lead extraction (TLE) plays a critical role in managing patients with cardiovascular implantable electronic devices. Mechanical TLE tools, including rotational sheaths, are used to overcome fibrosis and calcification surrounding leads. Prospective clinical data are limited regarding the safety and effectiveness of use of mechanical TLE devices, especially rotational tools.
OBJECTIVE: To prospectively investigate the safety and effectiveness of mechanical TLE in real-world usage.
METHODS: Patients were enrolled at 10 sites in the United States and Europe to evaluate th

BACKGROUND: Venous obstruction involving the axillary, subclavian, and brachiocephalic veins, and less commonly the superior vena cava, is a significant complication of transvenous pacemaker leads. Lead-related venous obstruction may be asymptomatic but can result in significant morbidity in individuals who develop superior vena cava syndrome.
CASE SUMMARY: We present the first reported case of a 54-year-old man with lead-related venous obstruction at the level of the right atrium, manifesting as pseudo-Fontan physiology, including hepatic fibrosis, lymphatic leakage, and protein-losing enter

INTRODUCTION: The incidence of cardiac implantable electronic device (CIED) infections is increasing. Complete device and lead removal are recommended for all patients with definite CIED system infection. In patients with pacemaker dependency, temporary pacing before reimplantation is necessary. In this study, temporary pacing using active fixation leads (TPAFL) was evaluated.
METHODS: We reviewed data from consecutive patients implanted with TPAFL after transvenous lead extraction at our center between November 2014 and October 2020.
RESULTS: TPAFL were placed in 334 patients. The mean age

BACKGROUND: The increasing number of patients with cardiac implantable electronic devices (CIEDs) causes a rise in the absolute percentage of individuals qualifying for a transvenous lead extraction (TLE) due to infectious, vascular or lead failure related indications. As the survival time prolongs, TLE procedures more and more often concern the electrodes of long- -term functioning. Authors provide a retrospective analysis of the effectiveness and safety of TLE performed on leads implanted at least 10 years before the extraction.
METHODS: Between 2008 and 2012 we performed TLE of 364 electro

BACKGROUND: Venous complications of implantable cardioverter defibrillator (ICD) systems may cause significant problems when the need for system revision or upgrades arises. Such revisions require venous access close to the site of the previous ICD implantation. The internal and external jugular vein have disadvantages due to a long subcutaneous course crossing the clavicle and problems with lead extraction if infection occurs.
METHODS: In seven patients with ICD revisions due to lead dysfunction (n = 4) and upgrade to a biventricular device (n = 2) and status after system removal due to infe

Flash glucose monitoring (FGM) systems are widely used in diabetes care, although cutaneous complications remain uncommon. However, FGM insertion-site infections can serve as a portal for Staphylococcus aureus, potentially leading to systemic complications in high-risk patients. We report the case of a 46-year-old man with poorly controlled type 1 diabetes who presented with fever, confusion, and cellulitis at an FGM site. Cultures confirmed persistent S. aureus bacteremia. Imaging revealed deep glenohumeral joint infection requiring surgical washout. Despite an initially normal transthoracic

Misplacement of pacemakers lead into the left ventricle (LV) is a rare but clinically important complication, often facilitated by unrecognized intracardiac shunts such as a patent foramen ovale (PFO). Early recognition is essential to avoid systemic embolization and ensure safe device function. We report a man in his 70s with a background of bioprosthetic aortic valve replacement, coronary bypass grafting, hypertension, chronic kidney disease, Parkinson's disease, and prostate cancer, who underwent permanent pacemaker implantation for symptomatic sinus pauses. Follow-up echocardiography 1 yea

BACKGROUND: Given recent approval of percutaneous tricuspid valve devices, clinical evidence is needed to guide lead management (extraction vs "jailing" [ie, entrapping]) for patients with preexisting cardiac implantable electronic devices (CIEDs).
OBJECTIVES: The goal of this study was to describe nationwide trends in both surgical and percutaneous tricuspid interventions and resulting clinical outcomes, particularly for patients with CIEDs.
METHODS: Tricuspid interventions were identified among Medicare beneficiaries with and without CIEDs. In-hospital and 30-day procedural complications w

BACKGROUND/OBJECTIVES: Children with congenital long QT syndrome (cLQTS) are at extremely high risk of torsades de pointes (TdP) and sudden cardiac death (SCD). Although the implantable cardioverter-defibrillator (ICD) is a cornerstone in SCD prevention, traditional transvenous ICDs are associated with venous access occupation, lead wear or fracture due to somatic growth, and difficulties with long-term lead revision or extraction. The extravascular ICD, which places the lead in the substernal extravascular space, has the potential to circumvent these lead-related complications. We report the

BACKGROUND: Epicardial pacemaker leads are used when transvenous access is contraindicated, but retained leads can rarely result in serious long-term complications.
CASE SUMMARY: A 40-year-old male with intravenous drug use, tricuspid valve replacement, and prior epicardial pacemaker implantation presented with hemoptysis. Imaging revealed a left lower lobe lung abscess containing retained epicardial leads, left in situ after incomplete extraction years earlier. He underwent lobectomy and resection of infected leads. Postoperatively, he developed respiratory failure and arrhythmias requiring

INTRODUCTION: In patients with a left ventricular assist device (LVAD), the subcutaneous implantable cardioverter-defibrillator (S-ICD) can be an alternative to transvenous ICD systems due to reduced risk of systemic infection, which could lead to extraction of the ICD as well as the LVAD. S-ICD eligibility is lower in patients with LVAD than in patients with end-stage heart failure without LVAD. Several reports have shown inappropriate S-ICD therapy in the coexistence of LVAD and S-ICD. The aim of the present study was to evaluate S-ICD eligibility in patients with LVAD using the established

Laser lead extraction is a relatively new adjunct in managing patients with chronically implanted devices. It carries the potential for a number of procedure-related complications which can present acutely or in a delayed fashion. We present a case of a rather uncommon delayed complication of laser lead extraction and its management with a successful outcome.

BACKGROUND: Lactational mastitis is a common painful and debilitating inflammation of breast tissue, generally treated conservatively or with pus puncture in case of breast abscess. However, treating mastitis in patients with implantable surgical material located in the affected breast region can be extremely challenging. We present an unusual case of lactational mastitis complicated by pacemaker pocket infection in a breastfeeding mother.
CASE PRESENTATION: A 35-year-old pacemaker-dependent female developed lactational mastitis seven weeks postpartum. Initially, the condition was treated con

The process of platinum recovery from used car catalysts is highly desirable for both economic and environmental reasons. From the many available methods of processing used car catalysts, the article conducted both numerical and experimental studies using a device based on the collector metal method with lead as a modified medium through a magnetohydrodynamic pump for washing platinum from the channels of the ceramic catalyst carrier. It was assumed that lead alloys with the addition of lithium increase the extraction of platinum from thin catalytic layers and accelerate the platinum dissoluti

INTRODUCTION: The use of temporary cardiac pacing is frequent in critical care units for severe bradycardia or electrical storm, but may be associated with frequent and potentially severe complications, especially when indwelling for several days. In some cases, transient indication or ongoing contraindication for a permanent pacemaker justifies prolonged temporary pacing. In that case, the implantation of an active-fixation lead connected to an externalized pacemaker represents a valuable option to increase safety and patient comfort. Yet, evidence remains scarce. We aimed to describe the pop

BACKGROUND: Transvenous lead extraction (TLE) is increasingly performed in older adults, but literature has reported divergent outcomes in very old populations.
OBJECTIVE: This study aimed to investigate the impact of patient age on TLE outcomes, with an emphasis on older patients.
METHODS: Using the ExTRACT registry, the largest TLE registry to date, we analyzed 5090 patients who underwent TLE between August 1996 and 2022 at the Cleveland Clinic, a high-volume center. Patients were stratified into 4 age groups: younger adults (<65 years; n = 2345), youngest old (65-74 years; n = 1430), midd

INTRODUCTION: Venous stenosis or occlusion related to an intracardiac device is a well-known complication of that procedure. There are numerous studies tried to determine predictors of venous stenosis or occlusion; however, most of them investigate the venous system prior to device upgrade, generator replacement, or transvenous lead extraction. Therefore, we aimed to assess the prevalence and determine the predictors of venous stenosis or occlusion following first transevnous cardiac device implantation.
METHODS: Observational, prospective study included 71 consecutive patients admitted for f
