Individualized current-shaping reduces DBS-induced dysarthria in patients with essential tremor.

OBJECTIVE: To investigate in patients with essential tremor (ET) treated with thalamic/subthalamic deep brain stimulation (DBS) whether stimulation-induced dysarthria (SID) can be diminished by individualized current-shaping with interleaving stimulation (cs-ILS) while maintaining tremor suppression (TS). METHODS: Of 26 patients screened, 10 reported SID and were invited for testing. TS was assessed by the Tremor Rating Scale and kinematic analysis of postural and action tremor. SID was assessed by phonetic and logopedic means. Additionally, patients rated their dysarthria on a visual analog

Clinical Reasoning: A 59-Year-Old Man With Progressive Dysarthria and Gait Instability.

We report a 59-year-old man with a progressively deteriorating neurologic presentation characterized by dysarthria and gait instability. Neurologic examination revealed pseudobulbar palsy, cerebellar ataxia, and pyramidal signs. Neuroimaging demonstrated irregular brainstem and cerebellar lesions with minimal mass effect and partial enhancement. Bilateral heel swelling, an uncommon clinical presentation, prompted further diagnostic evaluation. This case highlights the importance of a systematic reasoning approach, emphasizing the differential diagnosis, further investigative modalities, and tr

Do Acoustic Characteristics of Dysarthria in People With Parkinson's Disease Differ Across Languages?

PURPOSE: Cross-language studies suggest more similarities than differences in how dysarthria affects the speech of people with Parkinson's disease (PwPD) who speak different languages. In this study, we aimed to identify the relative contribution of acoustic variables to distinguish PwPD from controls who spoke varieties of two Romance languages, French and Portuguese. METHOD: This bi-national, cross-sectional, and case-controlled study included 129 PwPD and 124 healthy controls who spoke French or Portuguese. All participants underwent the same clinical examinations, voice/speech recordings,

A 52-Year-Old Woman With Dysarthria, Ataxia, Xanthelasmas, and Miliary Pulmonary Nodules.

A 52-year-old woman with no significant medical history was referred to our hospital for expedited workup of progressive dysarthria and ataxia over the past year. Prior CT angiography of the head and neck showed no relevant neurologic findings but did reveal miliary lesions in the lung apices, which was later confirmed via dedicated CT chest scan (Fig 1). Review of systems was negative for any respiratory, constitutional, or rheumatologic symptoms, except for new xanthelasma-like lesions over her forehead. She previously had smoked with 20 pack-years and had no TB risk factors. MRI of the face

Educational Information Improves Listener Attitudes Toward People With Dysarthria Secondary to Parkinson's Disease.

PURPOSE: People with dysarthria have been rated as less confident and less likable and are often assumed by listeners to have reduced cognitive abilities relative to neurotypical speakers. This study explores whether educational information about dysarthria can shift these attitudes in a group of speakers with hypokinetic dysarthria secondary to Parkinson's disease. METHOD: One hundred seventeen listeners were recruited via Amazon Mechanical Turk to transcribe sentences and rate the confidence, intelligence, and likability of eight speakers with mild hypokinetic dysarthria. Listeners were ass

Automatic boost articulation therapy in adults with dysarthria: Acceptability, usability and user interaction.

BACKGROUND: Imprecise articulation has a negative impact on speech intelligibility. Therefore, treatment of articulation is clinically relevant in patients with dysarthria. In order to be effective and according to the principles of motor learning, articulation therapy needs to be intensive, well organized, with adequate feedback and requires frequent practice. AIMS: The aims of this pilot study are (1) to evaluate the feasibility of a virtual articulation therapy (VAT) to guide patients with dysarthria through a boost articulation therapy (BArT) program; (2) to evaluate the acoustic models'

Lip function in subjects with upper motor neuron type dysarthria following cerebrovascular accidents.

The lip function of 16 speakers with upper motor neuron damage following cerebrovascular accident (CVA), was investigated using instrumental measures. Sixteen, non-neurologically impaired adults matched for age and sex served as controls. The results of the instrumental investigations revealed that the CVA speakers had patterns of lip function that were significantly different from the control speakers. Specifically, on maximum force tasks, the CVA speakers were found to produce significantly lower maximum lip force values. Maximum rate of attempts at lip movement was also slower in the CVA sp

Singing-Based Interventions for Treating Hypokinetic Dysarthria in Parkinson's Disease: A Systematic Review.

Parkinson's disease (PD) progressively affects motor functions, often resulting in hypokinetic dysarthria, characterized by reduced speech articulation and vocal intensity. Over the last few decades, singing-based interventions have demonstrated potential therapeutic benefits for treating neurological speech disorders. To review the effectiveness of singing-based interventions in treating hypokinetic dysarthria, articles were identified based on a search across MEDLINE, PubMed, Scopus, ScienceDirect, and Web of Science. After being independently screened by the authors, the information was rev

Speaker-independent dysarthria severity classification using self-supervised transformers and multi-task learning.

Dysarthria, characterised by slurred speech, is a hallmark of many neurological disorders and brain trauma. Clinical assessment requires an audio-visual investigation by a trained healthcare expert, who evaluates criteria such as respiration, phonation, articulation, resonance, and prosody during speech. Quantitative assessment of dysarthria is challenging due to its complexity, variability, and the subjective nature of human-observation-based scoring methods. We present a novel machine-learning framework using transformers for stratifying and monitoring patient speech. Our framework integrate
