Expression of sphingosine-1-phosphate receptor 1 in the brain of fatal cerebral malaria.

The signaling pathway activated by sphingosine-1-phosphate (S1P) through S1P receptor 1 (S1PR1) plays specific roles in regulating vascular integrity and preventing vascular leakage during inflammatory response. Endothelial cell barrier dysfunction has been implicated in cerebral malaria (CM) pathology. To explore the S1P/S1PR1 signaling pathway in CM, the study investigated the expression of S1PR1 in the brain of fatal malaria and correlated with the level of S1P and malaria severity. Localisation of S1PR1 in brain tissues was evaluated using immunohistochemistry technique in archived brain t

Blocking Sphingosine 1-phosphate Metabolism With Fingolimod Prevents the Progression of Vascular Smooth Muscle Cells Calcification in Chronic Kidney Disease.

Patients with chronic kidney disease, and particularly those under hemodialysis, are prone to develop cardiovascular complications, mostly due to the exacerbation of vascular calcification. Vascular calcification relies on the transdifferentiation of vascular smooth muscle cells into calcifying cells. Sphingosine 1-phosphate is a pleiotropic sphingolipid and an important regulator of osteogenesis and the cardiovascular system. Therefore, we explored the role of sphingosine 1-phosphate metabolism in chronic kidney disease-derived vascular calcification. Vascular calcification progression in chr

Neuroprotective Therapies for Spontaneous Intracerebral Hemorrhage.

Patients who survive the initial ictus of spontaneous intracerebral hemorrhage (ICH) remain vulnerable to subsequent injury of the perilesional parenchyma by molecular and cellular responses to the hematoma. Secondary brain injury after ICH, which contributes to long-term functional impairment and mortality, has emerged as an attractive therapeutic target. This review summarizes preclinical and clinical evidence for neuroprotective therapies targeting secondary injury pathways following ICH. A focus on therapies with pleiotropic antiinflammatory effects that target thrombin-mediated chemotaxis

Safety of disease-modifying therapies in multiple sclerosis: real-world data from the Austrian MS Treatment Registry (AMSTR).

BACKGROUND: In the therapeutic landscape of multiple sclerosis (MS), more than a dozen disease-modifying therapies (DMT) are currently available. In Austria, only certified MS centers are authorized to prescribe DMT and are obliged to enter patient data into the Austrian MS Therapy Registry (AMSTR). Here, we report the safety profiles of different DMT documented in this registry. METHODS: Adverse events (AE) data from the AMSTR, collected at least biannually from August 2006 to July 2025, were extracted. AE were classified by system organ classes according to the Medical Dictionary for Regula

An Update on Bile Acid-Regulated Signalling in the Pathogenesis of Metabolic Dysfunction-Associated Steatotic Liver Disease.

Metabolic dysfunction-associated steatotic liver disease (MASLD) is a common metabolic disorder with rising global incidence without effective treatment. Emerging evidence highlights bile acids (BAs) as pivotal signalling molecules that modulate genes involved in their own metabolism and lipid homeostasis in hepatic and extrahepatic tissues, thereby influencing MASLD pathogenesis. These regulatory effects are primarily driven by the activation of nuclear receptors, including the farnesoid X receptor (FXR), vitamin D receptor (VDR), pregnane X receptor (PXR), and by the membrane-bound G protein

Emerging therapeutic strategies in multiple sclerosis: a focus on innovative and targeted approaches.

Multiple sclerosis (MS) is an immune-mediated disease of the central nervous system marked by damage to myelin and nerve cells. Current treatments help control inflammation but have limited success in progressive stages and repairing nerve damage. This review aims to summarize new therapies for MS that target both inflammation and neurodegeneration beyond traditional immunosuppressive drugs. We conducted a thorough literature search to summarize key findings from original studies, including clinical trials investigating novel MS treatments such as Bruton's tyrosine kinase (BTK) inhibitors, CAR

Apolipoprotein M expression modifies the sphingolipid landscape in murine blood and lymph.

Members of the diverse family of sphingolipids (SPL), such as ceramides (Cer) and sphingomyelins (SM), are well-known structural and bioactive signaling molecules. A key SPL family member and critical signaling lipid, sphingosine 1-phosphate (S1P), is carried in blood primarily by its "chaperone" protein apolipoprotein M (ApoM) on high-density lipoprotein (HDL) particles. S1P has been shown to regulate diverse biological pathways through specific G protein-coupled receptor signaling (GPCR) that can be modulated based upon chaperone: ApoM or albumin. Blood concentrations of ApoM itself are alte

The sphingosine kinase (SphK)1/sphingosine-1-phosphate (S1P) pathway is involved in multiple biological processes, including liver diseases. This study investigate whether modulation of the SphK1/S1P system associates to the beneficial effects of melatonin in an animal model of acute liver failure (ALF) induced by the rabbit hemorrhagic disease virus (RHDV). Rabbits were experimentally infected with 2 × 10(4) hemagglutination units of a RHDV isolate and received 20 mg/kg of melatonin at 0, 12, and 24 hr postinfection. Liver mRNA levels, protein concentration, and immunohistochemical labeling f

Testosterone regulates the expression and functional activity of sphingosine-1-phosphate receptors in the rat corpus cavernosum.

The bioactive lipid sphingosine-1-phosphate (S1P) regulates smooth muscle (SM) contractility predominantly via three G protein-coupled receptors. The S1P1 receptor is associated with nitric oxide (NO)-mediated SM relaxation, while S1P2 & S1P3 receptors are linked to SM contraction via activation of the Rho-kinase pathway. This study is to determine testosterone (T) modulating the expression and functional activity of S1P receptors in corpus cavernosum (CC). Adult male Sprague-Dawley rats were randomly divided into three groups: sham-operated controls, surgical castration and T supplemented gro

New drugs in the pipeline for the treatment of inflammatory bowel diseases: what is coming?

Over the past twenty years several biological drugs and the first small molecule have been approved for the treatment of patients with inflammatory bowel diseases (IBD). However, a large percentage of patients do not respond to therapies and the demand for new effective drugs is still an unmet need. The better understanding of the pathophysiological mechanisms of disease has allowed to identify new therapeutic targets to block inflammatory pathways. To date, many emerging drugs have been developed and are being tested for both ulcerative colitis and Crohn's disease patients. Here, we summarize

Blood-brain barrier dysfunction in hepatic encephalopathy: pathophysiology, diagnostic assessment and therapeutic perspectives.

Hepatic encephalopathy (HE) is defined as the spectrum of neurological and neuro-psychological disturbances that complicate liver insufficiency and/or portosystemic shunt and is associated with excess mortality, impaired quality of life and a higher risk of traffic accidents. In addition to hyperammonaemia, systemic inflammation is now recognised as a pivotal precipitant of HE. Accumulating evidence over the past three decades indicates that dysfunction of the blood-brain barrier (BBB)-the multicellular interface that regulates molecular, immune and haemodynamic exchanges between brain and blo

Sphingosine-1-Phosphate Receptor-2 Antagonists: Therapeutic Potential and Potential Risks.

The sphingosine-1-phosphate (S1P) signaling system with its specific G-protein-coupled S1P receptors, the enzymes of S1P metabolism and the S1P transporters, offers a multitude of promising targets for drug development. Until today, drug development in this area has nearly exclusively focused on (functional) antagonists at the S1P1 receptor, which cause a unique phenotype of immunomodulation. Accordingly, the first-in class S1P1 receptor modulator, fingolimod, has been approved for the treatment of relapsing-remitting multiple sclerosis, and novel S1P1 receptor (functional) antagonists are bei

Novel and Emerging Therapies for Inflammatory Bowel Disease.

Inflammatory bowel diseases (IBD) such as ulcerative colitis and Crohn's disease are chronic, relapsing and remitting disorders of intestinal inflammation with potential systemic manifestations. Despite the availability of current biologics, such as anti-tumor necrosis factor (anti-TNF), anti-integrins, anti-interleukins and small molecules such as tofacitinib, the rates of primary and secondary treatment failure remain high in IBD. This highlights the importance of continued development of new therapeutic targets and modifications of existing ones to improve the treatment response rates and t

Are Small Molecules Effective in Treating Inflammatory Pouch Disorders Following Ileal Pouch-Anal Anastomosis for Ulcerative Colitis? Here Is Where We Stand.

Ulcerative colitis (UC) management encompasses conventional and advanced treatments, including biological therapy and small molecules. Surgery, particularly in the form of ileal pouch-anal anastomosis (IPAA), is indicated in cases of refractory/severe disease. IPAA can lead to acute complications (e.g., acute pouchitis) as well as late complications, including chronic inflammatory disorders of the pouch. Chronic pouchitis, including the antibiotic-dependent (CADP) and antibiotic-refractory (CARP) forms, represents a significant and current therapeutic challenge due to the substantial need for

Inflammatory Bowel Disease: Emerging Therapies and Future Treatment Strategies.

Inflammatory bowel disease (IBD) is a term used to represent a group of chronic, relapsing inflammatory disorders of the gastrointestinal tract. Crohn's disease (CD) and ulcerative colitis (UC) are the two major clinical forms. The global incidence and prevalence of IBD have increased over the last 2-4 decades. Despite the specific etiopathogenesis of IBD still being unknown, it is widely recognized that immunological, genetic, and environmental factors are implicated. A greater understanding of the multiple signaling pathways involved has led to the development of biologic therapies in the la
