Magnetic-Field-Induced Secondary-Electron Effects in MR-Guided Radiotherapy: From Dosimetric Mechanisms to Workflow-Based Clinical Risk Management.

MR-guided radiotherapy (MRgRT) combines high-contrast MRI, online adaptation, and radiation delivery in a single treatment workflow. The static magnetic field, however, changes the transport of secondary electrons generated by megavoltage photons and can produce dose perturbations that depend strongly on anatomy and beam geometry. This review examines the underlying physics, with emphasis on Lorentz-force deflection, the electron return effect (ERE) at density interfaces, and the electron streaming effect (ESE) in favorable geometries. It also considers where conventional DVH-based review may

Design maps for the hyperthermic treatment of tumors with superparamagnetic nanoparticles.

A plethora of magnetic nanoparticles has been developed and investigated under different alternating magnetic fields (AMF) for the hyperthermic treatment of malignant tissues. Yet, clinical applications of magnetic hyperthermia are sporadic, mostly due to the low energy conversion efficiency of the metallic nanoparticles and the high tissue concentrations required. Here, we study the hyperthermic performance of commercially available formulations of superparamagnetic iron oxide nanoparticles (SPIOs), with core diameter of 5, 7 and 14 nm, in terms of absolute temperature increase ΔT and specifi

Shear-induced pressure localization in granular materials of nonspherical particles.

When a granular material composed of elongated grains is sheared in a split-bottom shear cell, a pressure difference develops within the material. This pressure difference depends on the interparticle friction, which affects shear localization and particle alignment. At high friction, alignment is confined to a narrow shear band, leading to localized increases in packing density and pressure. At low friction, the particles align over a wider region, leading to a nearly uniform packing density and pressure throughout the material. In contrast, assemblies of spherical particles exhibit a uniform

Autophagy and mitochondrial metabolism: insights into their role and therapeutic potential in chronic myeloid leukaemia.

Despite the development of selective BCR-ABL-targeting tyrosine kinase inhibitors (TKIs) transforming the management of chronic myeloid leukaemia (CML), therapy-resistant leukaemic stem cells (LSCs) persist after TKI treatment and present an obstacle to a CML cure. Recently, we and others have made significant contributions to the field by unravelling survival dependencies in LSCs to work towards the goal of eradicating LSCs in CML patients. In this review, we describe these findings focusing on autophagy and mitochondrial metabolism, which have recently been uncovered as two essential process

Heat transport in laminar flow of erythrocyte suspensions.

Measurements of thermal conductivity were made in laminar flow of dog and turkey erythrocyte suspensions in a stainless stell tube of about 1 mm ID. These measurements were independent of the shear rate, showing that the red cell motion relative to plasma in flowing blood had no effect on the heat transfer. Measurements of thermal conductivity were further made in suspensions of polystyrene spheres of 100 mum and were found to be dependent upon the shear rate. The Graetz solution corresponding to uniform wall temperature was used for determining the value of thermal conductivity in an apparatu

Beyond representations: from active inference to active interference.

Active inference describes the system-environment boundary through statistical constructs such as the Markov blanket, a substrate-independent formalism that specifies a boundary's functional role without committing to the physical structure realizing it. We ask what structure can perform that role, and propose Active Interference as a complementary, biophysically grounded layer beneath the statistical description. We argue that biological boundaries are physically instantiated, resonant interfaces-lipid membranes coupled to vicinal water and endogenous electromagnetic fields-capable of wave su

Experience-driven rate modulation is reinstated during hippocampal replay.

Replay, the sequential reactivation within a neuronal ensemble, is a central hippocampal mechanism postulated to drive memory processing. While both rate and place representations are used by hippocampal place cells to encode behavioral episodes, replay has been largely defined by only the latter - based on the fidelity of sequential activity across neighboring place fields. Here, we show that dorsal CA1 place cells in rats can modulate their firing rate between replay events of two different contexts. This experience-dependent phenomenon mirrors the same pattern of rate modulation observed du

How far away is spinal muscular atrophy gene therapy?

Spinal muscular atrophy (SMA) is a devastating motor neuron disease primarily affecting children, for which there is currently no known disease-modifying therapy or cure. The identification of the disease gene, survival motor neuron, led to an expansion in SMA research and allowed the creation of numerous animal and cellular models. This led to a significant increase in our understanding of the pathophysiology of SMA, culminating in the development of multiple SMN-dependent and -independent therapies. Among the most exciting options, viral gene therapy has emerged as one leading candidate. A g

Sampling genotypes on complex pedigrees with phenotypic constraints: the origin of the B allele among the Polar Eskimos.

Exact probability calculations are often infeasible on large complex pedigrees. Conditional independences, however, occurring as a natural consequence of Mendelian inheritance of genetic traits, define a locally dependent Markov random field on the state space of all genotypic configurations on the pedigree. The underlying Markov chain is irreducible for most traits determined by a diallelic locus. For a given pedigree and a known genetic model, the Gibbs sampler can be used to obtain good estimates of the posterior distribution of genotypes given the observed data. The areas of pedigree analy

Molecular basis of the apparent near ideality of urea solutions.

Activity coefficients of urea solutions are calculated to explore the mechanism of its solution properties, which form the basis for its well-known use as a strong protein denaturant. We perform free energy simulations of urea solutions in different urea concentrations using two urea models (OPLS and KBFF models) to calculate and decompose the activity coefficients. For the case of urea, we clarify the concept of the ideal solution in different concentration scales and standard states and its effect on our subsequent analysis. The analytical form of activity coefficients depends on the concent

Evaluation of the mechanism of action of paracetamol, drotaverine, and peppermint oil and their effects in combination with hyoscine butylbromide on colonic motility: human ex-vivo study.

INTRODUCTION: Drotaverine, paracetamol, and peppermint oil are often prescribed for the treatment of gastrointestinal spasm and pain. This study aimed to evaluate the effect of these drugs alone and combined with the well-known antispasmodic hyoscine butylbromide on the human colon. METHODS: Colon samples were obtained from macroscopically normal regions of 68 patients undergoing surgery and studied in muscle bath. Drotaverine, paracetamol, and peppermint oil were tested alone and in combination with hyoscine butylbromide on (1) spontaneous contractility induced by isometric stretch (in the p

Radiotherapy for patient with cardiac implantable electronic device, consensus from French radiation oncology society.

Radiotherapy in patients with cardiac implantable electronic device such as pacemakers or defibrillators, is a clinical situation that is becoming increasingly common. There is a risk of interaction between the magnetic field induced by accelerators and the cardiac implantable electronic device, but also a risk of device dysfunction due to direct and/or indirect irradiation if the cardiac implantable electronic device is in the field of treatment. The risk can be dose-dependent, but it is most often independent of the total dose and occurs randomly in case of neutron production (stochastic eff

Differentiated adipose-derived stem cells promote the recovery of nociceptor function in rats.

The loss of nociceptive function in the skin because of trauma or surgery can impair the quality of life. The recovery of nociceptor function is mediated by two different axonal responses: nerve growth factor (NGF)-dependent collateral sprouting of undamaged nerves and NGF-independent regeneration of damaged nerves. We reported previously that adipose-derived stem cells (ASCs) can transdifferentiate into Schwann cell (SC)-like cells (dASCs) and that transplantation of dASCs increases axonal density in skin flaps. In the present study, we used an animal model that allowed for the individual ass

Investigation of the hydrodynamic properties of a new MRI-resistant programmable hydrocephalus shunt.

BACKGROUND: The Polaris valve is a newly released hydrocephalus shunt that is designed to drain cerebrospinal fluid (CSF) from the brain ventricles or lumbar CSF space. The aim of this study was to bench test the properties of the Polaris shunt, independently of the manufacturer. METHODS: The Polaris Valve is a ball-on-spring valve, which can be adjusted magnetically in vivo. A special mechanism is incorporated to prevent accidental re-adjustment by an external magnetic field. The performance and hydrodynamic properties of the valve were evaluated in the UK Shunt Evaluation Laboratory, Cambri

Environmental absorbed-dose quantities for natural radionuclides in soil using a simplified block phantom with an ICRP 110 comparison.

Natural radionuclides in soil produce an outdoor photon field, but an organ absorbed dose also depends on source depth, body geometry, scored mass, emission data and charged-particle transport settings. This work calculates absorbed-dose quantities in defined scoring regions for 40K, the 238U decay series and the 232Th decay series with Geant4 and a simplified block phantom. The Geant4 source uses the full nuclear gamma-emission library recovered from the adopted RadioactiveDecay6.1.2 and PhotonEvaporation6.1.2 data. The production calculation consisted of 20 independent replicas and 4.0×109 s

Effect of 24-h blood pressure dysregulations and reduced ocular perfusion pressure in open-angle glaucoma progression.

BACKGROUND: Low ocular perfusion pressure (OPP), which depends on the mean arterial pressure (MAP) and intraocular pressure (IOP), is associated with glaucoma. We studied 24-h MAP dysregulations and OPP in relation to the progression of glaucoma damage. METHODS: We retrospectively analyzed 155 normal-tension glaucoma (NTG) and 110 primary open-angle glaucoma (POAG) patients aged 18 years old followed at the University Hospital Leuven with repeated visual field tests ( n  = 7000 measures, including both eyes) who underwent 24-h ambulatory blood pressure monitoring. Twenty-four-hour MAP dysregu

Role of clinical pharmacology in the development of antiplatelet drugs.

PURPOSE: This review discusses the role of clinical pharmacology in the development of low-dose aspirin and other antiplatelet agents during the past 30 years, emphasizing the main determinants of several success stories as well as of complete failures in the field. METHODS: The author employs personal appraisal of the literature, with emphasis on personal contributions to the field. FINDINGS: Low-dose aspirin provides an interesting paradigm of the independent development of a "new" antiplatelet agent by the medical/scientific community. Aspirin "resistance," improved dosing regimens for pe

A general deficit of the 'automatic pilot' with posterior parietal cortex lesions?

Lesions of the parieto-occipital junction (POJ) in humans cause gross deviations of reaching movements and impaired grip formation if the targets are located in the subjects' peripheral visual field. Movements to central targets are typically less impaired. This disorder has been termed "optic ataxia". It has been suggested that a general deficit of online corrections under central as well as peripheral viewing conditions might be sufficient to explain this discrepancy. According to this hypothesis, patients with optic ataxia should demonstrate an impaired online correction of grip aperture un

Diurnal IOP fluctuation: not an independent risk factor for glaucomatous visual field loss in high-risk ocular hypertension.

PURPOSE: To establish whether intraocular pressure (IOP) fluctuations contribute to the risk of developing glaucoma in patients with high-risk ocular hypertension. METHODS: Ninety patients included in the Malmö Ocular Hypertension Study were examined every 3 months with office-hours diurnal tension curves and computerised perimetry. Patients were followed up prospectively for 10 years or until glaucomatous visual field loss could be demonstrated. Poststudy data were included in the analyses, extending maximum follow-up to 17 years. RESULTS: After 17 years, 37 patients had developed glaucomat

Topological Kolmogorov complexity and the Berezinskii-Kosterlitz-Thouless mechanism.

Topology plays a fundamental role in our understanding of many-body physics, from vortices and solitons in classical field theory to phases and excitations in quantum matter. Topological phenomena are intimately connected to the distribution of information content that, differently from ordinary matter, is now governed by nonlocal degrees of freedom. However, a precise characterization of how topological effects govern the complexity of a many-body state, i.e., its partition function, is presently unclear. In this paper, we show how topology and complexity are directly intertwined concepts in
