Use of LMA for awake intubation for caesarean section.

PURPOSE: This case report describes the use of a Laryngeal Mask Airway in a morbidly obese parturient with the H.E.L.L.P. syndrome. An urgent Caesarean section was required because of vaginal bleeding and fetal distress. CLINICAL FEATURES: The patient was a 32 year old G3, T1, P1, L1 who presented with epigastric pain, headache, vomiting, and diarrhoea. She was hypertensive (180/110 mmHg) and thrombocytopaenic (18 x 10(-9). L-1). Examination of the airway revealed a short neck, receded jaw, full dentition, large breasts and she was considered to be a potential intubation problem. The patient

Pedunculated subependymomas of lateral ventricle.

The authors describe two cases of extremely rare, pedunculated subependymomas of lateral ventricle. One patient presented with symptoms of intracranial hypertension (case 2); CT scan revealed evidence of intraventricular mass, but in second patient (case 1) coexisting symptomatic anaplastic astrocytoma masked on CT-scan the presence of asymptomatic intraventricular tumour, which was found no earlier than at autopsy. The cellular origin of this rare intracranial neoplasm is discussed.

Use of an angiotensin converting enzyme inhibitor in ocular hypertension and primary open-angle glaucoma.

We studied the effects of a new topical angiotensin converting enzyme inhibitor, SCH 33861, in lowering intraocular pressure in 20 patients with ocular hypertension or primary open-angle glaucoma. In a double-masked, four-way crossover study with placebo and timolol, SCH 33861 was well tolerated and effective in lowering intraocular pressure. The magnitude of the drug's effect in lowering intraocular pressure was less than that of timolol 0.5%.

The role of blood pressure variability in misdiagnosed clinic hypertension.

Blood pressure (BP) assessment may be vulnerable to bias by increased BP variability. Uncertainty in determining BP control is inherent to the clinic setting. We analyzed a registry of 3949 patients referred for ambulatory BP monitoring. The difference between clinic and ambulatory readings was plotted against ambulatory BP variability, assessed by standard deviation. In addition, BP variability of patients with clinic and awake ambulatory hypertension was compared with that of patients with controlled BP and sustained hypertension, respectively. The average clinic-ambulatory systolic BP diffe

Standardizing blood pressure phenotyping in acromegaly: A practical companion to Araujo-Castro et al.

Hypertension affects nearly half of patients with acromegaly, yet current studies inconsistently report blood pressure phenotypes due to variability in the use of office, home, and ambulatory monitoring. This companion article proposes a standardized, risk-stratified approach using ambulatory and home blood pressure monitoring to classify sustained, masked, and nocturnal hypertension and to guide antihypertensive de-escalation in relation to biochemical control. The framework incorporates key timepoints-diagnosis, early post-therapy reassessment, and long-term follow-up-while emphasizing modal

Test-retest reproducibility of optic disk deterioration detected from stereophotographs by masked graders.

PURPOSE: To assess the reproducibility of determining whether an eye has developed optic disk deterioration by the Optic Disc Reading Center (ODRC) in the Ocular Hypertension Treatment Study (OHTS). DESIGN: Test-retest reproducibility study. METHODS: Masked, certified graders at the ODRC determine the occurrence of optic disk deterioration in OHTS by comparing baseline with follow-up stereoscopic optic disk photographs. To assess reproducibility, re-gradings were obtained annually by inserting masked "quality control" photographs into the usual ODRC reading stream. RESULTS: Agreement (kappa

Primary aldosteronism detected after renal transplantation.

A 44-year old female was admitted to our hospital for evaluation of uncontrolled hypertension. She received renal transplantation 3 months ago and suffered from a tingling sensation and weakness on both hands. Laboratory findings (low serum potassium, low plasma renin activity and elevated plasma aldosterone level) was consistent with primary aldosteronism. Through the postural study for plasma aldosterone and the adrenal CT finding we diagnosed this case as adrenal adenoma, which was confirmed by surgical removal. A retrospective review of medical records showed that characteristic findings i

The many faces of hypertension in individuals with type 1 diabetes.

Several disturbed blood pressure (BP) patterns, including disparities between office and out-of-office BP measurements (such as white-coat and masked hypertension), disturbed circadian BP variability (such as abnormal dipping patterns and nocturnal hypertension) and treatment-resistant hypertension, are common in individuals with type 1 diabetes. Consequently, office or home BP measurements alone may not reflect real BP variation and may lead to inadequate diagnosis and treatment of hypertension. The early detection of these disturbed BP patterns is especially crucial in individuals with type

[A case of Takayasu arteritis diagnosed following posterior reversible encephalopathy syndrome].

A 53-year-old woman presented with chronic anorexia and gait disturbance. During evaluation at another hospital, brain MRI revealed abnormal high signal intensities in both supratentorial and infratentorial regions, prompting referral and admission to our hospital. On admission, she exhibited non-lateralized hypertension, decreased responsiveness, hypophonia, masked face, bradykinesia, shuffling gait, and hyperreflexia. Blood tests showed mild inflammatory markers. Based on her hypertension and MRI findings, posterior reversible encephalopathy syndrome (PRES) was diagnosed, and antihypertensiv

Comparison of day-to-day blood pressure variability in hypertensive patients with type 2 diabetes mellitus to those without diabetes: Asia BP@Home Study.

Blood pressure variability (BPV) has been shown to be independently associated with cardiovascular (CV) mortality and morbidity. Patients with type 2 diabetes mellitus (T2DM) have also been shown to have increased BPV. We aimed to compare BPV in hypertensive patients with diabetes with those without diabetes. A total of 1443 hypertensive patients measured their blood pressure (BP) twice in the morning and twice before bed at home for a week. Demographic data, history of T2DM, and anti-hypertensive use were captured. Clinic BP was measured twice in the clinic. Control of BP was defined as clini

Ambulatory daytime blood pressure versus tonometric blood pressure measurements in the laboratory: effect of posture.

OBJECTIVE: To compare blood pressure (BP) in tonometric radial artery recordings during passive head-up tilt with ambulatory recordings and evaluate possible laboratory cutoff values for hypertension. METHODS: Laboratory BP and ambulatory BP were recorded in normotensive (n = 69), unmedicated hypertensive (n = 190), and medicated hypertensive (n = 151) subjects. RESULTS: Mean age was 50.2 years, BMI 27.7 kg/m 2 , ambulatory daytime BP 139/87 mmHg, and 276 were male (65%). As supine-to-upright changes in SBP ranged from -52 to +30 mmHg, and in DBP from -21 to +32 mmHg, the mean values of BP s

Hypertensive Conditions: Essential (Primary) Hypertension in Adults.

Blood pressure (BP) screening using an office-based measurement is recommended for adults 18 years and older without a history of hypertension. If abnormal, the BP measurement should be repeated twice with the average of those final two readings used to determine the BP category. Home BP monitoring and ambulatory BP monitoring are beneficial in patients for whom there is a concern for masked or white-coat hypertension. Guidelines differ regarding the BP cutoff used for the diagnosis of hypertension. Lifestyle modifications are the foundation of hypertension management with the Dietary Approach

Is early diagnosis of hypertension a function of cuff width?

This study followed blood pressure in 11 subjects 5 years after slight or established hypertension had been revealed using a cuff of the correct width (appropriate to arm circumference); at that time, the use of a standard-width cuff (12 cm) had shown a blood pressure within the normal range. For the present study, blood pressure was determined indirectly under very strict conditions so that a faithful comparison between the readings with both cuffs could be achieved. An important underestimate of blood pressure was detected in thin arms when the standard width cuff was used. The present measu

A short note on hypertension management.

Hypertension occupies a unique position in public health care, because it is the major cause of cardiovascular disease and the most frequent non-communicable disorder seen in primary care globally. The prevalence, treatment, and control rates of hypertension vary greatly by ethnicity. Such disparities are mostly linked to genetic variances, although lifestyle and socioeconomic level may influence health behaviours such as food - both of which appear to be substantial factors. East Asian communities have distinct ethnic traits. Hypertensive individuals are more likely to have salt sensitivity t

Clinical characterization of blood pressure phenotypes: the BP phenotype score.

BACKGROUND: Evidence has linked blood pressure (BP) phenotypes with certain clinical, psychosocial, and occupational features, and characteristic BP variability. OBJECTIVE: We aimed to evaluate the value of a diagnostic score developed from these characteristics in predicting BP phenotypes, when used in a manner comparable to the application of out-of-office techniques. METHODS: Adult patients with no prior diagnosis of hypertension attending their office appointments, were prospectively enrolled. Their clinical, psychosocial, and occupational data were collected. 3-consecutive pre-appointme
