Hypothesis for cognitive effects of transcranial direct current stimulation: Externally- and internally-directed cognition.

A comprehensive explanation is lacking for the broad array of cognitive effects modulated by transcranial direct current stimulation (tDCS). We advanced the testable hypothesis that tDCS to the default mode network (DMN) increases processing of goals and stored information at the expense of external events. We further hypothesized that tDCS to the dorsal attention network (DAN) increases processing of external events at the expense of goals and stored information. A literature search (PsychINFO) identified 42 empirical studies and 3 meta-analyses examining effects of prefrontal and/or parietal

Efficacy of transcranial direct current stimulation for depression: individual patient data meta-analysis.

BACKGROUND: Transcranial direct current stimulation (tDCS) shows promise for treating depression, but heterogeneous findings from randomised controlled trials (RCTs) - likely due to patient characteristics and methodological differences - limit clear conclusions about its efficacy. AIMS: This individual patient data meta-analysis (IPD-MA) aims to evaluate the efficacy of tDCS for depression and explore moderators of clinical depression improvement. METHOD: Databases (PubMed, Embase, Web of Science, Cochrane Library) were searched up to 1 February 2025 for RCTs comparing active versus sham tD

From Protocol to Practice: Hidden Sources of Variability in Transcranial Direct Current Stimulation.

Transcranial direct current stimulation (tDCS) is widely used in neuroscience research and clinical contexts because it is non-invasive, relatively inexpensive, technically accessible, and has a highly favorable safety profile. However, its apparent simplicity can obscure implementation challenges and practical sources of variability that influence stimulation quality, participant comfort, reproducibility, and interpretation of results. Standard protocols typically specify current intensity, stimulation duration, electrode size, and montage, but less attention is often given to device selectio

Objective neural face recognition neuromodulation within a single transcranial direct current stimulation session.

Due to considerable technical and methodological challenges, neuromodulatory effects of online and post-stimulation transcranial direct current electrical stimulation (tDCS) on human cognition remain unclear. Here we evaluated these effects of tDCS on a sensitive, objective and reliable neural marker of human face identity recognition. Eighteen participants took part in this combined tDCS-EEG study. Following a sham tDCS period during rapid periodic individuation of pictures of unfamiliar faces (Phase 1), tDCS was applied for 10 min during the same visual stimulation over the right occipito-te

Anodal Transcranial Direct Current Stimulation as an Adjunct to Physiotherapy in Lacunar Stroke.

Lacunar strokes affecting deep subcortical motor pathways often lead to significant disability despite their small lesion size, and recovery can remain incomplete with physiotherapy alone. Transcranial direct current stimulation (tDCS) has emerged as an accessible neuromodulation tool that may enhance post-stroke rehabilitation, though evidence from resource-limited settings in India is sparse. This case report describes a 56-year-old man with hypertension and type 2 diabetes mellitus who developed acute left-sided weakness, imbalance, and mild facial asymmetry, with CT imaging confirming a ri

Transcranial Direct Current Stimulation (tDCS) for Borderline Personality Disorder (BPD): Why and How?

BACKGROUND: Borderline Personality Disorder (BPD) is a severe psychiatric condition characterized by pervasive emotional dysregulation, impulsivity, and unstable interpersonal relationships. Affecting over 1% of the general population, BPD carries significant morbidity, frequent hospitalizations, and an increased risk of suicide. Although specialized psychotherapeutic approaches have shown efficacy, their impact is often constrained by availability, lengthy treatment durations, moderate effect sizes, and high dropout rates. Pharmacological treatments for BPD remain inadequate and are usually a

Transcranial direct current stimulation alters cerebrospinal fluid-interstitial fluid exchange in mouse brain.

BACKGROUND: Transcranial direct current stimulation (tDCS) is a non-invasive brain stimulation technique that has gained prominence recently. Clinical studies have explored tDCS as an adjunct to neurologic disease rehabilitation, with evidence suggesting its potential in modulating brain clearance mechanisms. The glymphatic system, a proposed brain waste clearance system, posits that cerebrospinal fluid-interstitial fluid (CSF-ISF) exchange aids in efficient metabolic waste removal. While some studies have linked tDCS to astrocytic inositol trisphosphate (IP3)/Ca2+ signaling, the impact of tDC

Improving mentalizing deficits in older age with region-specific transcranial direct current stimulation.

Older adults have difficulties to detect the intentions, thoughts, and feelings of others, indicating an age-associated decline of socio-cognitive abilities that are known as "mentalizing". These deficits in mental state recognition are driven by neurofunctional alterations in brain regions that are implicated in mentalizing, such as the right temporo-parietal junction (rTPJ) and the dorso-medial prefrontal cortex (dmPFC). We tested whether focal transcranial current stimulation (tDCS) of the rTPJ and dmPFC has the potential to eliminate mentalizing deficits in older adults. Mentalizing defici

Bilateral High-Definition Transcranial Direct Current Stimulation for Upper Extremity Rehabilitation in Stroke.

Previous research shows that both anodal and cathodal high-definition transcranial direct current stimulation (HD-tDCS) may improve function of the upper extremity post stroke. However, most research has focused on the effects separately, therefore the purpose of this study was to determine the effects of performing simultaneous anodal-cathodal HD-tDCS. Five stroke participants received the stimulations in four visits with a two-week washout period: 1) anodal HD-tDCS to the ipsilesional primary motor cortex, 2) cathodal HD-tDCS to the contralesional dorsal premotor cortex, 3) bilateral anodal-

Applications of transcranial direct current stimulation over vagus nerve on dysphagia after stroke.

Vagus nerve stimulation (VNS) has been commonly employed for the functional rehabilitation of stroke patients. This study aimed to investigate the therapeutic effects of transcranial direct current stimulation on the vagus nerve (TDCSVN) in improving dysphagia in stroke patients. Patients experiencing dysphagia following a stroke were diagnosed with dysphagia by a water swallow test. Swallowing function was evaluated with the standard swallowing scale score and the functional dysphagia scale. Serum levels of interleukin (IL)-1β and IL-8 were measured with enzyme-linked immunosorbent assays. TD

DeeptDCS: Deep Learning-Based Estimation of Currents Induced During Transcranial Direct Current Stimulation.

OBJECTIVE: Transcranial direct current stimulation (tDCS) is a non-invasive brain stimulation technique used to generate conduction currents in the head and disrupt brain functions. To rapidly evaluate the tDCS-induced current density in near real-time, this paper proposes a deep learning-based emulator, named DeeptDCS. METHODS: The emulator leverages Attention U-net taking the volume conductor models (VCMs) of head tissues as inputs and outputting the three-dimensional current density distribution across the entire head. The electrode configurations are also incorporated into VCMs without in

Transcranial direct current stimulation (tDCS) targeting the postcentral gyrus reduces malevolent creative ideation.

Malevolent creativity (MC) is defined as a manifestation in which people propose to materially, mentally or physically harm themselves or others in a novel manner. Malevolent creative ideation can be inhibited by high moral emotions (i.e. sympathy, guilt and shame) and low negative emotions, which promote prosocial behaviors. Given that the right postcentral gyrus (PCG) is involved in generating sympathy and emotional recognition for others and the right middle frontal gyrus (MFG) is involved in emotional regulation, we suggest that the right PCG and right MFG may play important roles in malev

Transcranial direct current stimulation as a preventive treatment in multiple sclerosis? Preclinical evidence.

Multiple sclerosis (MS) is an inflammatory demyelinating disease of the central nervous system, presenting with optic neuritis in about 20-30% of cases. Optic nerve demyelination, associated with delay of visual evoked potentials (VEPs), is also observed prior to motor signs in the preclinical MS model Experimental Autoimmune Encephalomyelitis (EAE). Transcranial direct current stimulation (tDCS), inducing polarity-dependent changes in neuronal excitability, is widely used to promote neuroplasticity in several neurological disorders. However, its potential effects on inflammation and demyelina
