Predictors of impaired pulmonary function in people living with HIV in an urban African setting.

BACKGROUND: Studies have associated HIV with an increased risk of obstructive lung disease (OLD). OBJECTIVES: We aimed to identify the predictive factors for impaired lung function in an urban, African, HIV-positive population. METHOD: A cross-sectional study was performed in Johannesburg, South Africa, from July 2016 to November 2017. A questionnaire was administered and pre- and post-bronchodilator spirometry conducted. The predictors investigated included age, sex, antiretroviral treatment (ART) duration, body mass index, history of tuberculosis (TB) or pneumonia, occupational exposure, e

Pathways of health care for people living with multimorbidity in two Southern African countries.

Multimorbidity, the presence of multiple chronic conditions in one person, is a growing global health concern. Integration of chronic care services is urgently needed, especially in low-resource settings including in Southern Africa, where care has been fragmented by vertical and siloed disease approaches. Many countries share similar challenges of integration, presenting rich opportunities for shared learning. Yet, rarely are these opportunities capitalised upon, in part because of a lack of systematic knowledge about the similarities and differences in health system contexts, challenges and

Electrocardiographic and echocardiographic abnormalities in urban African people living with HIV in South Africa.

BACKGROUND: Studies from high income countries report that HIV-positive people have an impaired systolic and diastolic cardiac function compared to HIV-negative people. It is unclear if results can be translated directly to the Sub-Saharan Africa context. This study assesses electro- and echocardiographic characteristics in an urban African population, comparing HIV-positive people (treated and not yet treated) with HIV-negative controls. METHODS: We conducted a cross-sectional study in Johannesburg, South Africa. We enrolled HIV-positive participants from three randomized controlled trials t

Managing hypertension in people of African origin with diabetes: Evaluation of adherence to NICE Guidelines.

BACKGROUND: The pathophysiology of hypertension in people of African origin differs from other ethnicities. This effect may be exacerbated in people with type 2 diabetes mellitus (T2DM), hence control of hypertension is particularly important in this population. AIMS: The primary aim was to evaluate the adherence to National Institute of Clinical Excellence (NICE) guidance (National Guidelines NG28) for hypertension management in African origin patients with T2DM. METHODS: This study was done using electronic health records at a large primary care centre based in Birmingham, UK. Strict exclu

Ethnicity and cardiovascular risk: variations in people of African ancestry and South Asian origin.

Mortality from coronary heart disease (CHD), stroke and end-stage renal failure are high in South Asian migrants in the UK. This is associated with high prevalence of diabetes and hypertension. These seem to be manifestations of a metabolic syndrome with insulin resistance (hyperinsulinaemia) and central obesity (based on high waist-to-hip ratio rather than on conventional measures of body mass index). This is associated with sedentary lifestyle, high serum triglycerides and low HDL-cholesterol. Mortality from stroke and end-stage renal failure are high in black migrants to the UK (both Caribb

HIV status disclosure among HIV-positive African and Afro-Caribbean people in the Netherlands.

HIV status disclosure is often characterized as a dilemma. On the one hand, disclosure can promote health, social support, and psychological well-being. On the other, disclosure can lead to stigmatization, rejection, and other negative social interactions. Previous research has shown that HIV status disclosure is a reasoned process whereby the costs and benefits to oneself and to others are weighed. As such, understanding disclosure requires understanding the reasons for and against disclosure employed by people living with HIV (PLWH). In this study, disclosure among a population disproportion

Risk of Cardiovascular Disease Associated with Serious Mental Illness among People of African Ancestry.

BACKGROUND: Individuals with severe mental illness (SMI)-schizophrenia, schizoaffective disorder, and bipolar disorder-are at higher risk for cardiovascular disease (CVD) than the general population. Black Americans are known to have a higher prevalence of cardiovascular risk factors. However, the association of SMI with CVD and its risk factors in this population has not been widely examined. METHODS: The analytic sample included 2305 participants from the African Ancestry-Genomic Psychiatric Cohort enrolled in Brooklyn, New York, between 2016 and 2020. SMI was identified by using the Diagno

Is nasal MRSA carriage a concern among African people and local communities in North Cyprus?

INTRODUCTION: The long-standing epidemic of antimicrobial resistance (AMR) has proven to be a challenging problem to solve and is becoming even more complex in a globalized world where there is little evidence of AMR patterns among multinational communities. This study aimed to determine the frequency of methicillin resistant Staphylococcus aureus (MRSA) carriage among African and local students in Northern Cyprus. METHODOLOGY: Nasal swabs were collected from African and local students between October 2023 and December 2023. The clinical samples were screened for the presence of MRSA. AMR bac

The relationship between immune markers and tryptophan-kynurenine metabolites in South African people with HIV.

OBJECTIVE: HIV-1 remains a global challenge, especially in high-prevalence areas like South Africa. This study explores the relationship between inflammation and metabolism in people with HIV, focusing on immune markers and the tryptophan-kynurenine (Trp-Kyn) pathway. DESIGN: This is a cross-sectional, observational study exploring the associations between peripheral inflammation and metabolism in treatment-naive South African people with HIV. METHODS: We examined immune markers (hsCRP, suPAR, IL-6, NGAL, and sCD163) and Trp-Kyn metabolites (QUIN, Trp, Kyn, Trp/Kyn ratio, and kynurenic acid)

Uneven burden of cardiac amyloidosis in people of African descent - global imbalance in resources and access.

Transthyretin cardiac amyloidosis (TTR-CA) is now increasingly becoming recognized as an important cause of heart failure, and some studies have shown that as much as a third of diastolic heart failure could be attributed to TTR-CA. Black populations are particularly at risk for TTR-CA as the most common form of the disease (hereditary TTR-CA) has a genetic basis and the gene responsible is most prevalent among people with West African ancestry. This perspective piece explores the challenges that individuals of African and Caribbean populations face when confronted with the burden of TTR-CA. K

Genetic Variants of APOL1 Are Major Determinants of Kidney Failure in People of African Ancestry With HIV.

INTRODUCTION: Variants of the APOL1 gene are associated with chronic kidney disease (CKD) in people of African ancestry, although evidence for their impact in people with HIV are sparse. METHODS: We conducted a cross-sectional study investigating the association between APOL1 renal risk alleles and kidney disease in people of African ancestry with HIV in the UK. The primary outcome was end-stage kidney disease (ESKD; estimated glomerular filtration rate [eGFR] of <15 ml/min per 1.73 m2, chronic dialysis, or having received a kidney transplant). The secondary outcomes included renal impairment

Opportunities for respiratory disease transmission from people to chimpanzees at an East African tourism site.

Respiratory illnesses, including COVID-19, present a serious threat to endangered wild chimpanzee (Pan troglodytes) populations. In some parts of sub-Saharan Africa, chimpanzee tracking is a popular tourism activity, offering visitors a chance to view apes in their natural habitats. Chimpanzee tourism is an important source of revenue and thus benefits conservation; however, chimpanzee tracking may also increase the risk of disease transmission from people to chimpanzees directly (e.g., via aerosol transmission) or indirectly (e.g., through the environment or via fomites). This study assessed

Difficult-to-treat hypertensive populations: focus on African-Americans and people with type 2 diabetes.

The awareness, treatment, and control of hypertension has risen steadily over the past three decades, until the early 1990s. However, blood pressure control to < 140/90 mmHg is attained in fewer than 25% of all hypertensive patients and fewer than 50% of drug-treated hypertensive patients, except for white women. Two special populations, African-Americans and diabetics, share several important attributes. First, they both have a high prevalence of hypertension, including stage 3 hypertension (as defined by the Joint National Committee on Prevention, Detection, Evaluation, and Treatment of Hype

South African speech-language therapists' practices regarding feeding tube placement in people with advanced dementia.

BACKGROUND: Studies related to tube feeding in people with dementia (PWD) remain a contested topic, neglecting the importance of speech-language therapists' (SLTs) role in dysphagia management. Furthermore, SLT practices and beliefs regarding tube feeding in people with advanced dementia in an upper-middle-income country, such as South Africa, are unexplored. OBJECTIVE: This study aimed to determine the practices and beliefs of SLTs in South Africa regarding tube feeding placement in PWD. METHOD: A self-compiled online survey was distributed using social media platforms and was completed by
