Appearance of maternal anti-JK a antibody following intrauterine transfusions and amniocentesis.

A case is presented that suggests that intrauterine transfusion carries the risk of sensitization to red cell antigens other than Rho. In this case intrauterine transfusions of JK a-positive red cells were followed by the appearance in the mother's serum of anti-JK a, an antibody known to cause hemolytic disease of the newborn. The use of the mother as a donor of red cells for her fetus will circumvent the problem of further maternal sensitization.

[Problematic blood transfusion (author's transl)].

Two patients were admitted with signs of haemolysis due to severe transfusion reactions. Serological investigations showed the presence of the antibodies anti-IKb and anti-Fya. After a short period of haemodialysis the patient with Kidd antibodies died. A further patient with triple antibodies (anti-c, -K, -Lub) was so pre-immunised that with an antigen frequency of over 99.9% no compatible blood could be found in Europe. By means of information from the European Central Index in Amsterdam compatible blood for the operation could be obtained from Australia. In the fourth case the antibody anti

High immunogenicity of red blood cell antigens restricted to the population of African descent in a cohort of sickle cell disease patients.

BACKGROUND: Sickle cell disease (SCD) patients undergo multiple red blood cell (RBC) transfusions and are regularly exposed to low-prevalence (LP) antigens specific to individuals of African descent. This study evaluated the prevalence of antibodies against LP antigens in SCD patients and the need to identify these antibodies in everyday practice. STUDY DESIGN AND METHODS: Plasma from 211 SCD patients was tested with RBCs expressing the following LP antigens: RH10 (V), RH20 (VS), RH23 (DW ), RH30 (Goa ), KEL6 (Jsa ), and MNS6 (He). RESULTS: Nine LP antibodies were found in eight patients (3.

The Prevalence of Kidd, Lutheran, and Duffy Antigens and Phenotypes Among Blood Donors in Al-Qurayyat, Al-Jouf, Saudi Arabia: A Cross-Sectional Study.

OBJECTIVE: Blood group antigen frequencies vary across populations and influence transfusion compatibility and alloimmunization risk; data on minor systems in northern Saudi Arabia are limited. This study determined Kidd (Jk), Lutheran (Lu), and Duffy (Fy) antigen and phenotype frequencies among Al-Qurayyat blood donors and compared them with other populations. METHODS: In this cross-sectional study, 240 healthy blood donors were serologically typed for Kidd (Jka, Jkb), Lutheran (Lua, Lub), and Duffy (Fya, Fyb) antigens. Using the chi-square test, Phenotype frequencies were compared with publ

Post-transfusion alloimmunization in patients with sickle cell disease.

The transfusion histories over a 33-month period of 50 patients with sickle cell disease were reviewed to determine the frequency of alloimmunization to red cell antigens following transfusion in these patients. There were 30 females and 20 males, aged 19--49 years. Eighteen (36%) were immunized of which thirteen were females. Five of the patients have formed only one antibody so far, while the other 13 have formed two or more. Thirty-six antibodies were identified: 16 against various Rh antigens, 12 anti-Lewis, 5 anti-Kell and one each of anti-Jka, -Fya and -M. The immunized patients received

Auto-anti-Jka in Evans' syndrome with negative direct antiglobulin test.

A patient presented with haemolytic anaemia and a negative direct antiglobulin test (DAT), and was found to have an IgG antibody with anti-Jka specificity in his serum. His red cells were typed as Jk(a-b+). Later he developed idiopathic thrombocytopenic purpura (ITP), and had a positive DAT due to anti-Jka bound to his red cells, which now typed as Jk(a+b+). Family studies suggested that the patient's true type was Jk(a+b+). Splenectomy and immunosuppression were required to treat the thrombocytopenia. The autoanti-Jka was no longer detectable following therapy.

[Hemolytic transfusion accident due to the blood group antibody anti-Jka].

A haemolytic transfusion incident caused by anti-Jka is reported about. In a 56 year old patient suffering from chronic iron deficiency anaemia an immunization in the Kidd system has taken place within 9 days. In the course of this immunization the fourth incompatible blood transfusion caused a severe transfusion incident. The necessity of serological compatibility testing by the indirect antiglobulin or enzyme test and the significance of making a strict indication for blood transfusions are referred to.

A delayed hemolytic transfusion reaction (DHTR) with multiple alloantibodies (Anti-E, Jka, Dia, Fyb, and S) induced by E-antigen-negative, crossmatch-compatible blood.

We describe the case of a 44-year-old woman with a delayed hemolytic transfusion reaction (DHTR). She had a history of two pregnancies and a blood transfusion, the details of which were unknown. At the time of her first vascular surgery on November 15, 1989, she received 1200 ml of crossmatch-compatible concentrated red blood cells (CRC). Before the first operation, screening for anti-RBC antibodies (Ab) was negative. At the time of the second admission on Feburary 15, 1996, anti-E Abs were detected by indirect antiglobulin test. She received 560 ml of E-antigen-negative, crossmatch-compatible

Blood chimeric twins: an example of blood cell chimerism.

A man and his twin sister were found to have two types of circulating red cells. The man had 89% A1 cells of his own and 11% A1B cells derived from his sister. The woman had 12% A1B cells of her own and 88% A1 cells derived from her brother. The two lines of red cells also had different Duffy and Kidd groups. The lymphocyte karyotype in the brother was 68% XY and 32% XX, and that in the sister was 60% XX and 40% XY. The possible mechanisms for producing different proportions of red cells and lymphocytes are discussed.

Notorious anti-Jk3 in a pregnant woman.

This is a case study of a 23-year-old pregnant Polynesian woman with anti-Jk3 identified in her plasma during her third visit to a hospital. This patient, with a history of mild anemia due to beta-thalassemia minor and two known transfusion of packed red cells came to an emergency room complaining of severe abdominal cramps. Her hemoglobin during her second hospital visit was 7.5 g/dL and her antibody screen was negative. Two units of crossmatch compatible packed cells were transfused with no adverse reaction and she was discharged. Four days post transfusion, she returned to the hospital comp

Unusual delayed haemolytic transfusion reaction characterised by slow destruction of red cells.

In a delayed haemolytic transfusion reaction due to an apparently typical complement-binding IgG anti-Jka, it was possible to show that transfused Jk (a+b+) cells were destroyed progressively over a period of about 3 weeks. The slow destruction may have been due partly to the fact that the patient had been splenectomised.

A detailed serological study of five anti-Jka sera reacting by the antiglobulin technique.

Detailed serological investigation of five examples of anti-Jka revealed considerable heterogeneity in their characteristics. All the sera were investigated by the antiglobulin technique, with and without complement and by a range of sensitization and testing methods. Three were readily detected in the presence of complement, while two were detectable with anti-IgG, providing a spin test was used. Complement was bound by all and resulted in enhanced reactions, particularly when slide/tile tests were used. The study emphasizes that anti-complement (C3) is an essential component of polyspecific

Donor anti-Jk(a) causing hemolysis in a liver transplant recipient.

BACKGROUND: Hemolytic transfusion reactions have been observed in recipients of ABO- and/or D-mismatched marrow, peripheral blood, and solid organs. Passenger lymphocyte syndrome occurs when immunocompetent donor lymphocytes transferred during transplantation produce alloantibodies against host antigens. CASE REPORT: The first case of a delayed, anti-Jk(a)-mediated hemolytic reaction in a liver transplant recipient, caused by passenger donor lymphocytes, is reported here. A 43-year-old man underwent liver transplantation. Six weeks later, the patient underwent a second liver transplant. On Da

Transfusion reaction. An immunologic hazard of blood transfusion.

Twenty-three cases of delayed hemolytic transfusion reaction (DHTR) occurring at the Mayo Clinic from 1964 through 1973 are reviewed. Nineteen patients had clinical manifestations of hemolysis, of which fever was the most frequent presenting symptom. The degree of hemolysis served as an index of morbidity. In four cases, there was oliguria, two of these patients experiencing renal shutdown. In one case, hemolysis led to a disseminated intravascular coagulation syndrome. Death occurred subsequent of DHTR in three patients. The direct antiglobulin test was positive in all but one case; this find

Genomic characterisation of the Jk(a-b-) phenotype in Thai blood donors.

BACKGROUND: The Kidd (JK) blood group antigens are encoded by the JK gene. The rare Jk(a-b-) phenotype can be caused by homozygosity for a silent JK allele. Currently, JK(null) alleles have been identified among different populations; however, information on its presence among Thais is not available. MATERIALS AND METHODS: Screening for the Jk(a-b-) phenotype by the urea lysis test was performed in 25,340 blood samples from Thai blood donors. The Jk(a-b-) phenotypes were confirmed by an indirect antiglobulin test (IAT). Additionally, polymerase chain reaction amplification and sequence analys

Severe hemolytic reaction due to anti-JK3.

A 35-year-old gravida 3, para 3 Filipino woman with a negative antibody screen, no prior history of transfusion, and no hemolytic disease of the newborn in her children suffered a massive postpartum hemorrhage requiring transfusion. A severe hemolytic transfusion reaction occurred 5 days after delivery. Subsequently, a panagglutinin on a routine antibody identification panel was identified as anti-Jk3. The patient's red blood cell phenotype was Jk(a-b-) and all of her children were Jk(a-b+), yet the antibody that formed reacted with equal strength against all Jk(a)- or Jk(b)-positive cells. Th

Possible insensitivity of the polybrene antibody screen to detect anti-Jka.

An acute hemolytic transfusion reaction (AHTR) occurred in a 28-yr-old gravida immediately after transfusion with leukocyte-reduced red cells. The patient gave no history of prior transfusion. Initial serologic testing by the polybrene method was negative for both antibody screening and cross-matching. Further testing by the indirect anti-globulin test (IAT) demonstrated the presence of anti-Jka antibodies. These observations suggest a limitation in polybrene testing for Jka antibodies associated with hemolytic transfusions. Caution is advised when the polybrene test is used as the sole determ
