Neuronal metabolic dysfunction in patients with cortical developmental malformations: a proton magnetic resonance spectroscopic imaging study.

Cortical developmental malformations are best diagnosed by MRI and are often the cause of refractory epilepsy. Little is known about the metabolic cell function on MR spectroscopy of these types of brain anomaly. We studied 23 patients with cortical developmental malformations and refractory epilepsy using proton MR spectroscopic imaging. Mean age was 28 years (range, 9 to 47 years). The lesions examined were focal cortical dysplasia (n = 5), heterotopia (four band, six periventricular, two subcortical), polymicrogyria (n = 3), tuberous sclerosis (n = 2), and polymicrogyria and periventricular

Clinicopathologic studies on leptomeningeal glioneuronal heterotopia in congenital anomalies.

Leptomeningeal glioneuronal heterotopia was observed in 40 of 129 autopsied infants (31%). It was present in 49% of patients who had congenital anomalies in general and in 65% of patients who had central nervous system malformations. Most of the leptomeningeal glioneuronal heterotopias appeared in the base of the brain (62.5%), midbrain (40%), frontal lobe (37%), and pons (35%). Leptomeningeal glioneuronal heterotopia is closely related to migration disorders on the basis of frequent association with polymicrogyria or neuronal heterotopias.

Aicardi's syndrome - report of an autopsy case.

A Japanese girl aged 19 months had generalized convulsions, agenesis of the corpus callosum, peculiar chorioretinopathy, scoliosis and marked retardation of psychomotor development. Pathological investigation revealed polymicrogyria, cortical heterotopias, disorganization of the cerebellar cortex. In ocular histopathology, lacunae were composed of hypo- or depigmented zones of the retinal pigment epithelium accompanied with dispersion of pigments into the rod and cone layer. Coloboma of the optic nerve was also evident. There was no evidence of inflammation or pathogenic organisms.

Unilateral closed lip schizencephaly with septo-optic dysplasia.

Schizencephaly is a rare malformation of the central nervous system. Both genetic and non-genetic etiologies like prenatal infections or ischemia have been postulated. Clinical manifestations most often include varying degrees of developmental delay, motor impairment and seizures. It can be associated with septo-optic dysplasia (SOD), optic nerve hypoplasia and absence of septum pellucidum, pachygyria, polymicrogyria, heterotopia and arachnoid cysts. We report a case of unilateral closed lip schizencephaly with SOD.

Developmental cortical anomalies in Asperger's syndrome: neuroradiological findings in two patients.

Recent neuropathological and magnetic resonance imaging (MRI) studies have demonstrated developmental disturbances of neuronal migration in the cerebral cortex of autistic subjects. We report neuroradiological findings in two patients with Asperger's syndrome, a less severe variant of autism. One patient had left frontal macrogyria; the other had bilateral opercular polymicrogyria. A computer tomography (CT) scan performed on one of the patients' first-degree relatives (who had a bipolar disease) also showed cortical migration anomalies.

Biophysical modeling of anatomically realistic prenatal cortical folding development.

Cortical folds encode the architecture of human cognition, yet the mechanisms that transform the smooth fetal cortex into its convoluted geometry remain elusive. Biophysical modeling enables mechanistic insight into cortical morphogenesis, but existing models often lack anatomical realism and fail to capture key hallmarks and morphometrics of dynamic cortical folding in the developing human brain. Here, we introduce a whole-brain developmental framework that integrates region-specific, data-driven growth laws with anatomically realistic cortical geometry to enable biologically interpretable mo

Altered excitatory and inhibitory neocortical circuitry leads to increased convulsive severity after pentylenetetrazol injection in an animal model of schizencephaly, but not of microgyria.

OBJECTIVE: Malformations of the polymicrogyria spectrum can be mimicked in rodents through neonatal transcranial focal cortical freeze lesions. The animals presenting the malformations present both altered synaptic events and epileptiform activity in the vicinity of the microgyrus, but the comprehension of their contribution to increased predisposition or severity of seizures require further studies. METHODS: In order to investigate these issues, we induced both microgyria and schizencephaly in 57 mice and evaluated: their convulsive susceptibility and severity after pentyleneterazol (PTZ) tr

Neuroimaging features in a neonate with rhizomelic chondrodysplasia punctata.

Rhizomelic chondrodysplasia punctata is a rare genetic disorder of peroxisomal metabolism that is characterized clinically by shortening of the proximal limbs, cataracts, a characteristic facial appearance, failure to thrive, and psychomotor retardation. This report describes a newborn with a severe phenotype whose neuroimaging showed pachygyria-polymicrogyria, severe spinal stenosis causing compression of the cervical cord and brainstem, and tethering of the spinal cord. Imaging of the brain and spinal cord in patients with this disorder may aid prognosis and guide management decisions.

Neuropathological findings in the cerebro-oculo-facio-skeletal (Pena-Shokeir II) syndrome.

An autopsy case is described of an infant with cerebro-oculo-facio-skeletal (COFS) (Pena-Shokeir II) syndrome who died of pneumonia at the age of 5 months, and the pathology of the CNS in this case and the cases in literature were reviewed. Neuropathological examination revealed a partial defect of the corpus callosum, lobulation of the caudate nucleus and putamen, polymicrogyria in the parietal lobes and neuronal heterotopia in the cerebral and cerebellar white matter. The migration disorders suggest that the onset starts in the early fetal period.

Aicardi's syndrome. A clinicopathologic study.

A 9-week-old female infant was seen with typical clinical features of Aicardi's syndrome. At autopsy (at 21 months of age), examination of the brain disclosed polymicrogyria and agenesis of the corpus callosum. Histopathologic studies of the posterior segments showed areas of intact but attenuated and depigmented retinal pigment epithelium, and atypical colobomatous defect of both posterior segments, and ectatic scleral channels lined by a retinal pigment epithelial choriocapillarislike complex in the left eye. The pathogenesis of the ocular defects is discussed.

Mutations of AKT3 are associated with a wide spectrum of developmental disorders including extreme megalencephaly.

Mutations of genes within the phosphatidylinositol-3-kinase (PI3K)-AKT-MTOR pathway are well known causes of brain overgrowth (megalencephaly) as well as segmental cortical dysplasia (such as hemimegalencephaly, focal cortical dysplasia and polymicrogyria). Mutations of the AKT3 gene have been reported in a few individuals with brain malformations, to date. Therefore, our understanding regarding the clinical and molecular spectrum associated with mutations of this critical gene is limited, with no clear genotype-phenotype correlations. We sought to further delineate this spectrum, study levels

Encephalocraniocutaneous lipomatosıs (Haberland syndrome) in a newborn baby: a case report with review of literature.

Encephalocraniocutaneous lipomatosis (ECCL) is an extremely uncommon, neurocutaneous disease, with a classical triad of ocular, skin lesions and central nervous system anomalies. We here report a case of ECCL in a newborn baby, characterized with naevus psiloliparus, choristoma, lipodermoids, cervical subcutaneous soft tissue mass, lowset ear, porencephalic cyst, polymicrogyria, arachnoid cyst, leptomeningeal angiomatosis and spinal lipomas. We here stress on the importance of early diagnosis to prevent misdiagnosis and employ a multidisciplinary approach in the management of these patients.

[Neuroimagings in neuronal migration disorders].

Recent progress in magnetic resonance imaging (MRI) has successfully visualized neuronal migration disorders (NMDs) without autopsy. NMDs are observed in the patients with hydranencephaly, porencephaly, cerebellar hypoplasia, agenesis of corpus callosum, agenesis of septum pellucidum and holoprocencephaly. Miller-Dieker syndrome, Fukuyama-type congenital muscular dystrophy, Walker-Warburg syndrome, hypomelanosis of Ito, congenital rubella syndrome, toxoplasmosis, Zellweger syndrome and other diseases are also associated with lissencephaly, pachygyria and polymicrogyria. Magnetic resonance imaging of intracranial malformations in dogs and cats.

Intracranial malformations may occur because of an inherent developmental defect or secondary to in utero injury to the brain with subsequent hypoplasia and atrophy. They can cause neurologic deficits in growing animals, although some anomalies may not produce clinical signs until adulthood. Malformations of the brain include hydrocephalus, hydranencephaly/porencephaly, holoprosencephaly, corpus callosum agenesis/dysgenesis, lissencephaly, polymicrogyria, meningoencephalocele, intracranial cysts, cerebellar malformations, and hamartomas. These conditions are defined and reviewed with an emphas

Disturbance of cerebral neuronal migration following congenital parvovirus B19 infection.

OBJECTIVE: We describe the clinical course of an infant who presented with severe fetal anemia and fetal hydrops following congenital parvovirus B19 infection before 16 gestational weeks. The fetus was treated by cordocentesis and intrauterine transfusion at 18 weeks. RESULTS: The infant demonstrated mild unilateral ventriculomegaly on antenatal magnetic resonance imaging, and polymicrogyria and heterotopia on postnatal magnetic resonance imaging. CONCLUSION: This adds to the evidence in recent literature of central nervous system damage associated with congenital parvovirus B19 infection.

Congenital microcephaly, infantile spasms, psychomotor retardation, and nephrotic syndrome in two sibs.

Two boys are described with congenital microcephaly, infantile spasms, psychomotor retardation and an early-onset nephrotic syndrome. The autopsy findings of one patient are described in detail. Polymicrogyria was the most prominent feature and the kidneys showed focal segmental glomerulosclerosis. These findings have been described as a clinical entity, the leading symptoms being congenital microcephaly, early-onset nephrotic syndrome and mental retardation, accompanied by various other clinical symptoms. A review of the literature suggests an autosomal recessive mode of inheritance.

COL4A2 -Related Disorder Presenting in Adulthood With Rhabdomyolysis.

The alpha 1 and 2 chains of type IV collagen, encoded by the COL4A1 (MIM 120130) and COL4A2 (MIM 120090) respectively, play essential roles in the vascular basement membranes. Pathogenic variants in COL4A1/ COL4A2 are associated with autosomal dominant cerebral angiopathies. The clinical manifestations of COL4A1/COL4A2-related disorders include: aneurysms, intracerebral hemorrhage, polymicrogyria, porencephaly, heterotopia, periventricular leukomalacia, epilepsy, and neurodevelopmental disorders. COL4A1 pathogenic variants that are in exons 24 and 25 have been associated with hereditary angiop

Biallelic COL3A1 mutations result in a clinical spectrum of specific structural brain anomalies and connective tissue abnormalities.

Vascular Ehlers-Danlos syndrome (type IV) is an autosomal dominant disorder caused by heterozygous variants of COL3A1. We identified biallelic COL3A1 variants in two unrelated families. In a 3-year-old female with developmental delay the nonsense variant c.1282C>T, p.(Arg428*) was detected in combination the c.2057delC, p.(Pro686Leufs*105) frame shift variant. Both compound heterozygous variants were novel. This patient was born with bilateral clubfoot, joint laxity, and dysmorphic facial features. At the age of 2 years she developed an aneurysmal brain hemorrhage. Cerebral MRI showed a peculi
