Full block or split block?-Comparison of two different autogenous block grafting techniques for alveolar ridge reconstruction.

OBJECTIVE: To evaluate radiographic bone gain after alveolar ridge augmentation with two different designs of autogenous block graft harvested from the mandible. MATERIALS AND METHODS: Alveolar ridge defects were evaluated by preoperative cone beam computed tomography (CBCT) and grafted in a staged approach using intraoral block grafts. The ridge augmentation was either performed using the full-block technique (group 1) or the split-block technique (cortical plate with autogenous bone chips) (group 2). After 4 months of bone healing, a further CBCT scan was performed before implant placement.

Alveolar Distraction.

Alveolar distraction osteogenesis (ADO) has been used for the reconstruction of atrophic alveolus for decades. The advantage of this technique is that it augments the bone and soft tissues together, creating a better alveolar platform for subsequent surgeries and dental rehabilitation. It is especially useful in patients with large and/or complex alveolar clefts for which approximating the alveolar segments reduces the size of the bony cleft and associated fistula. Displacement of the transported segment is the most frequently encountered complication of ADO but can be managed by constructing

Objective computerised assessment of residual ridge resorption in the human maxilla and maxillary sinus pneumatisation.

OBJECTIVES: Atrophic resorption of the maxillary alveolar ridge is a complication that makes implantological rehabilitation critical. Our aim was to develop a novel computer aided procedure for the accurate quantitative assessment of maxillary residual ridge resorption including pneumatisation of the maxillary sinus that goes beyond previously described approaches and to apply it to a large dataset. MATERIALS AND METHODS: To develop and refine the method, we performed a retrospective analysis using computed tomography data from 405 patients to generate segmented, three-dimensional models of z

A randomized controlled trial on the impact of healing time on wound healing following ridge preservation using a 70%/30% combination of mineralized and demineralized freeze-dried bone allograft.

BACKGROUND: To compare the histologic difference in healing between ridge preservation sites treated with a combination allograft of 70% mineralized and 30% demineralized freeze-dried bone allograft (FDBA) evaluated at 8 to 10 weeks versus 18 to 20 weeks post-extraction. Changes in morphological ridge dimensions were also evaluated. METHODS: Forty-four patients with a single-rooted tooth to be extracted and replaced by a dental implant were recruited for this study. At time of extraction, measurements were taken with a custom acrylic stent, and the extraction socket was grafted with the combi

Prosthetically Driven Alveolar Reconstructions: A Retrospective Study.

This study aims to evaluate the post-extraction alveolar bone reconstruction amongst 12 patients exhibiting loss of buccal bone plate in a tooth of the anterior region of the maxilla using the prosthetically-driven alveolar reconstruction technique (PDAR). In PDAR, a partial fixed provisional prosthesis (PFPP [conventional or adhesive]) with a specially designed pontic maintains the clot in a mechanically stable position during alveolar regeneration. Moreover, the pontic design, in hourglass shape and located in the subgingival area, also prevents gingival margins from collapsing. Gingival rec

A novel accurate volumetric analysis protocol for evaluating secondary alveolar cleft reconstruction.

An accurate volumetric analysis protocol for secondary alveolar cleft reconstruction is essential. It can help confirm favorable times for bone grafting, determine which graft material is more effective, and improve surgical techniques. This study aimed to introduce a novel protocol for precisely calculating the bone formation ratio (BF%) using computer-aided engineering. The helical computed tomography (CT) datasets of 14 patients who underwent alveolar cleft reconstruction was included in this study. CT scans performed preoperatively and 1 year postoperatively were evaluated by two investiga

The Alveolar Ridge Splitting Technique on Maxillae: A Biomechanical Human Cadaveric Investigation.

The alveolar ridge splitting technique (ARST) offers an alternative to classic ridge augmentation techniques for successful insertion of dental implants. However, the buccal lamella is at risk of fracturing during ARST distraction. To better understand the fracture mechanisms and displacement limits of the split lamella, this study conducted biomechanical tests on human cadaveric maxilla specimens having extremely atrophied alveolar ridges treated with ARST. A total of 12 standardized alveolar splits were prepared on the maxillae of 3 elderly female donors using an oscillating piezoelectric sa

[Implant-prosthetic rehabilitation of patients with absence of maxillary lateral incisors].

INTRODUCTION: The absence of maxillary lateral incisors caused by traumatic or genetic reasons or as a result of endodontic failure is a difficult problem for an implantologist. The difficulties are the result of bone deficiency in all three dimensions, irregularity of interdental papillae locations and often convergence of roots of the neighbouring teeth. The aim of this study was to present the complexity of orthodontic and implant-prosthetic treatment of the absence of maxillary lateral incisors. MATERIAL AND METHODS: Between 2003 and 2007, 11 patients with absence of maxillary lateral inc

Alveolar bone grafts: the surgical/orthodontic management of the cleft maxilla.

The purpose of this paper is to discuss the management of the cleft maxilla with emphasis on the controversies concerning the decisions which a cleft palate team makes with limited evidence but strong beliefs in the anticipated outcomes. The orthodontist and the surgeon need to collaborate in determining the timing and sequencing of alveolar bone grafting. Currently the emphasis on secondary bone grafting has superseded primary bone grafting in its effectiveness and efficiency. Contemporary management of infants born with clefts of the lip and palate is to delay bone grafting until the early m

Evaluation of collagen sponge and xenografts versus collagen sponge alone in alveolar ridge preservation: a randomized controlled clinical trial.

This study evaluated the clinical, radiographic, and histological outcomes of alveolar ridge preservation (ARP) using collagen sponge with xenografts (CS + Xenografts) versus collagen sponge (CS) alone, compared to spontaneous healing. 36 extraction sockets were randomly allocated into three groups: Group I (CS + Xenografts), Group II (CS alone), and Group III (control) spontaneous healing. Soft tissue assessment and CBCT imaging were conducted before tooth extraction (baseline) and 6 months post-extraction, followed by histologic and histomorphometric analysis of bone biopsies. Groups I and I

Radiographic changes after alveolar ridge preservation using autogenous raw tooth particles versus xenograft: A prospective controlled clinical trial.

OBJECTIVE: The use of extracted teeth has been introduced as an option for bone grafting. However, the current method requires special machines and solutions, posing significant time and cost. The aim of this study was to evaluate the clinical performance of autogenous raw tooth particles (RTP), a grafting material made from a ground tooth using basic equipment, for alveolar ridge preservation. MATERIALS AND METHODS: Twenty-three patients (12 study/11 control), having 14 and 13 sites were included for the study and control groups (commercially available xenograft), respectively. Influence on alveolar resorption of the buccal bony plate width in the edentulous ridge expansion (E.R.E.)--an experimental study in the dog.

OBJECTIVE: To compare the hard tissue changes at implants installed applying edentulous ridge expansion (E.R.E.) at sites with a buccal bony wall thickness of 1 or 2 mm. MATERIAL AND METHODS: In six Labrador dogs, the first and second maxillary incisors were extracted, and the buccal alveolar bony plates and septa were removed. After 3 months of healing, partial-thickness flaps were dissected, and the E.R.E. was applied bilaterally. Hence, an expansion of the buccal bony crest was obtained in both sides of the maxilla with a displacement of either a 1- or a 2-mm-wide buccal bony plate at the

Impact of alveolar ridge preservation on soft tissue contour changes after extraction of a periodontally compromised molar: A prospective controlled clinical trial.

STATEMENT OF PROBLEM: Tooth extraction can lead to changes in the soft tissue ridge morphology, which could jeopardize prosthetically driven implant placement. However, studies that comprehensively analyze the outcomes of alveolar ridge preservation in terms of soft tissue contour changes after extracting periodontally compromised molars are lacking. PURPOSE: The purpose of this prospective controlled clinical trial was to compare alveolar ridge preservation and natural healing in terms of soft tissue volumetric and linear contour changes in periodontally compromised molars 6 months after ext

Microvascular reconstruction and dental rehabilitation of benign severely atrophic jaws and defects of the alveolar ridge: our philosophy in 2019.

We deal regularly with patients who present with severe atrophy of the jaws, compromised soft tissue, or penetrating defects of the alveolar ridge that are not the result of malignant disease. For these patients we use microvascular bony flaps together with dental implants and implant-supported prostheses. The purpose of this retrospective study was to present our current management and the lessons we have learned over a 16-year period while treating 86 patients for these indications with 87 microvascular bone flaps. We used a transoral approach for the anastomosis in 60 flaps, thereby avoidin
