Cofactor infections and HIV epidemics in developing countries: implications for treatment.

This article shows that the burden of certain tropical disease infections, after controlling for other factors, is positively correlated with HIV prevalence. Using cross-national data and multivariate linear regression analysis, we investigate the determinants of HIV prevalence in low- and middle-income countries. We begin with social and economic variables used in other cross-national studies and then incorporate data on parasitic and infectious diseases endemic in poor populations, which are found to be strongly and significantly correlated with--and are potent predictors of--HIV prevalence.

Coeliac disease in developing countries: Middle East, India and North Africa.

Following the application of simple serological tests for the diagnosis of coeliac disease (CD) in the 1980s, it gradually became clear that the prevalence of CD in different countries in the Middle East, North Africa and India is almost the same as that in Western countries. The prevalence of CD in at-risk populations in these regions is reported to range between 3 and 20% and the prevalence in people with type 1 diabetes is approximately 3-5%. Clinical manifestations of CD vary markedly with age, the duration and the extent of disease. Clinical studies showed that presentation with non-speci

Immune responses to tuberculosis in developing countries: implications for new vaccines.

Tuberculosis is out of control in developing countries, where it is killing millions of people every year. In these areas, the present vaccine - Mycobacterium bovis bacillus Calmette-Guérin (BCG) - is failing. Progressive tuberculosis occurs because the potentially protective T helper 1 (T(H)1)-cell response is converted to an immunopathological response that fails to eliminate the bacteria. Here, we discuss the data indicating that the problem in developing countries is not a lack of adequate T(H)1-cell responses but, instead, an exaggerated tendency to switch to immunopathological responses.

Cost effectiveness analysis of strategies to combat malaria in developing countries.

OBJECTIVE: To determine the cost effectiveness of selected malaria control interventions in the context of reaching the millennium development goals for malaria. DESIGN: Generalised cost effectiveness analysis. DATA SOURCES: Efficacy data came from the literature and authors' calculations supported by expert opinion. Quantities for resource inputs came from the literature and from expert opinion; prices came from the WHO-CHOICE database. METHODS: Costs were assessed in year 2000 international dollars, and effects were assessed as disability adjusted life years averted by a 10 year implement

Cost effectiveness analysis of strategies for tuberculosis control in developing countries.

OBJECTIVE: To assess the costs and health effects of tuberculosis control interventions in Africa and South East Asia in the context of the millennium development goals. DESIGN: Cost effectiveness analysis based on an epidemiological model. SETTING: Analyses undertaken for two regions classified by WHO according to their epidemiological grouping-Afr-E, countries in sub-Saharan Africa with very high adult and high child mortality, and Sear-D, countries in South East Asia with high adult and high child mortality. DATA SOURCES: Published studies, costing databases, expert opinion. MAIN OUTCOM

Classification of childhood hearing impairment: implications for rehabilitation in developing countries.

AIM: To ascertain the rehabilitation needs of hearing impaired school entrants in developing countries based on current criteria for evaluating classification of childhood hearing impairment. METHODS: The profile of hearing impairment was examined in 50 children from eight randomly selected mainstream schools, based on findings from medical history, physical examination, otoscopy, pure-tone audiometry and tympanometry. The hearing thresholds across frequencies 0.25 - 8.0 kHz were analysed. The results were compared with conventional pure-tone averages (0.5 - 4.0 kHz) and WHO's criteria/thresh

[Implication of health checkups of students from developing countries in Japan].

We investigated the prevalence of peripheral blood abnormalities, parasitic infestation, and hepatitis virus infection, by using the results of the primary screening health checkups for 423 students (male: 317, female: 106, average age +/- SD: 34.2 +/- 5.5 year-old) from abroad. Most of them were from Southeast Asia, Africa, Central and South America, and other developing countries in tropical or subtropical areas. Thalassemia-like hematological disorders, showing microcytic peripheral red blood cells without any anemia, were seen in 7.6 percent of the students, and intestinal parasites were r

Lactose digestion and maldigestion: implications for dietary habits in developing countries.

Milk is an important source of high quality protein, energy, calcium, potassium, phosphorus and riboflavin. It also has good functional properties and a highly acceptable taste, making milk a good alternative for the nutrition of children and for food programmes in developing countries. However, in some instances it has been advocated that milk or milk based products might not be appropriate for nutritional support, given the high prevalence of lactose intolerance among populations in those countries. After reviewing studies in different regions of the world it is concluded that: 1. Prevalence

Cervical cancer control in developing countries: memorandum from a WHO meeting.

This memorandum summarizes the report of a WHO Consultation on the Control of Cervical Cancer in Developing Countries, held on 6-7 November 1994, in New Delhi, India. Evaluated was the current situation with regard to cervical cancer and the relevance of current practices in screening. New pragmatic approaches to cervical cancer were proposed that are relevant for developing countries; this includes empowerment of women to come forward, and visual inspection-"downstaging".

HIV-associated tuberculosis in developing countries: epidemiology and strategies for prevention.

The association between tuberculosis and HIV presents an immediate and grave public health and socioeconomic threat, particularly in the developing world. In early 1992 WHO estimated that approximately 4 million people had been infected with both Mycobacterium tuberculosis and HIV since the beginning of the pandemic; 95% of them were in developing countries. The association between tuberculosis and HIV is evident from the high incidence of tuberculosis, estimated at 5-8% per year, among HIV-infected persons, the high HIV seroprevalence among patients with tuberculosis, the high occurrence of t

Animal diseases affecting human welfare in developing countries: impacts and control.

Animal diseases adversely affect human populations by reducing the amount and quality of food and fiber, and draft power. Epizootics of diseases such as rinderpest have caused massive mortality among domestic and wild animals. Extensive outbreaks of Venezuelan equine encephalltis have caused massive mortality in equines, and significant morbidity among people. Foot-and-mouth disease can cause high morbidity and great direct economic loss, and loss of markets as a trade barrier. Old diseases such as yellow fever and Rift Valley fever are still with us, and diseases such as the viral haemorrhagi

Diarrhea in developed and developing countries: magnitude, special settings, and etiologies.

Diarrheal diseases are major causes of morbidity, with attack rates ranging from two to 12 or more illnesses per person per year in developed and developing countries. In addition, diarrheal illnesses account for an estimated 12,600 deaths each day in children in Asia, Africa, and Latin America. The causes of diarrhea include a wide array of viruses, bacteria, and parasites, many of which have been recognized only in the last decade or two. While enterotoxigenic Escherichia coli and rotaviruses predominate in developing areas, Norwalk-like viruses, Campylobacter jejuni, and cytotoxigenic Clost

Thalassemia in Bombay: the role of medical genetics in developing countries.

This study of 200 families with thalassaemic children in Bombay showed that these children's treatment and needs place a significant, unavoidable and increasing demand on the public health services. At the same time, owing to the potentially large number of patients and the difficulties of long-term management, the situation is characterized by evasion of the problem, failure of planning, no provisions for prevention, and inadequate treatment leading to premature death among the affected children. The burden on such families is greater in developing than in developed countries because, besides
