BACKGROUND: Corynebacterium is known as a pathogen for man, particularly C. diphtheriae, while other species may cause disease and particularly cardiac valve infection, mainly in immunosuppressed patients, intravenous drug addicts, valve prosthesis carriers, patients with previous valvular disease, with congenital heart diseases or those submitted to cardiothoracic surgery.
METHODS: Seven episodes of endocarditis due to Corynebacterium no diphtheriae among six patients admitted to our hospital between 1989 and 1992 are analyzed.
RESULTS: The mitral valve was often affected with a predominanc

Purinergic signalling pathways play a vital role in the biological functions of the aortic valve (AV) through nucleotide and adenosine-dependent receptor effects. This study focused on characterizing a side-specific purinergic cascade in human non-stenotic and stenotic AVs, ovine native AVs and a novel bacterial nanocellulose (BNC) bio-prosthesis in an ovine model. Human stenotic AVs were collected during replacement surgeries, while non-stenotic AVs came from heart transplant patients. Ovine native AVs were sourced from domestic sheep, and the BNC prosthesis was implanted in the ovine aorta f

Atrioventricular (AV) valve disease is a major burden in our Indian subcontinent, where rheumatic heart disease is still prevalent, when compared to the Western world, where degenerative heart disease is more prevalent. Worldwide, nearly 300,000 valve replacements are done every year but not without complications. These challenges can be multidimensional and multiscalar with the macroscopic and microscopic properties of the native patient tissue interacting with the mechanical and bioprosthetic heart valves and rings. Understanding the complex and variable anatomy of the AV valves is essential

OBJECTIVES: The presence of a mechanical prosthesis has been regarded as an increased risk of thromboembolic complications and as a relative contraindication for a left ventricular assist device (LVAD). Five patients in our center had a mechanical aortic valve at the time of device implantation and were studied regarding thromboembolic complications.
DESIGN: Five patients operated upon with an LVAD (1 HeartMate I™, 4 HeartMate II™) between 2002 and 2011 had a mechanical aortic valve at the time of implantation. The first patient had a patch closure of the aortic valve. In four patients, the p

The surgical management of mitral and aortic valvular heart disease has changed significantly over the last decade. Meanwhile, tricuspid valve (TV) surgery has remained less commonly performed than left-sided valve surgeries, and there is a relative paucity of reports in the literature. There are ongoing controversies as to the optimal surgical management of TV disease, including repair versus replacement, the type of prosthesis preferred, optimal repair techniques, appropriate management of functional tricuspid regurgitation (TR) with concomitant left heart valve surgery, and others. In this

By incorporating the best features of the Carpentier-Edwards PERIMOUNT Magna Mitral Ease valve (Edwards Lifesciences Corporation, Irvine, California) and INSPIRIS RESILIA tissue (Edwards Lifesciences Corporation, Irvine, California), the mitris valve inherits the advantages of the remarkable hemodynamic performance of the former and the durability of the latter. In this paper, we will summarize the process that led to the creation of this new valve and report on the first implant's feasibility and first impression. The mitris valve has an overall implantability profile, overlapping the previou

Although transcatheter valve therapy is rapidly evolving, surgical valve replacement is still required in many patients with severe left-side valve stenosis or regurgitation, the mechanical bi-leaflet heart valve being the standard prosthesis type in younger patients. Moreover, the prevalence of valvular heart disease is steadily increasing, especially in industrialized countries, and the problem of lifelong efficient anticoagulation of these patients remains fundamental, especially in the context where vitamin K antagonists continue to be the current standard of anticoagulation despite a leve

BACKGROUND: Aortic valve damage is the most common valvular heart disease in developed countries. The Ross procedure is an alternative to the aortic valve replacement with a prosthesis, providing a longer survival without reoperation and without the need for anticoagulation therapy. The unavailability of homografts for the pulmonary valve replacement is one of the limiting factors for a more common utilization of this therapeutic method.
CASE SUMMARY: This case report presents a 35-year-old Caucasian male with a bicuspid aortic valve and severe aortic regurgitation. The patient underwent the

We report our experience with 30 children under 15 years of age treated surgically for congenital mitral valve disease from March 1972 to July 1986. Valve reconstruction was possible in 26 patients (87%), whereas in four the valve was replaced with a mechanical prosthesis. Five patients died in the hospital (16.6%), four after conservative operations and one after mitral valve replacement. There was only one late death in a child in chronic congestive heart failure. Three patients, treated conservatively, required valve replacement 2, 22, and 24 months, respectively, after the operation. Follo

Trans-apical approach has been proved successful in failing surgical bio-prosthesis in both mitral and aortic position in adult patients. Recently, valve-in-valve treatments have been applied even in patients with complex congenital heart disease. Here, we report the case of a 32 years old lady with left atrial isomerism, complete AV septal defect, interrupted inferior vena cava with azygos continuation who underwent Kawashima procedure with atrial Fontan. Severe systemic atrioventricular valve regurgitation necessitated a 33 mm Perimount valve implantation and conversion to lateral tunnel Fon

Seven patients underwent postoperative right heart catheterization implantation of the St. Jude Medical prosthetic valve in the tricuspid position. Six patients were in atrial fibrillation at catheterization, and 1 was in normal sinus rhythm. At postoperative catheterization, the mean right atrial pressure ranged between 4 and 16 mm Hg (mean, 9.7 mm Hg); right ventricular systolic pressure was normal in 1 patient, mildly elevated (less than 50 mm Hg) in 4 patients, and moderately elevated (65 and 70 mm Hg) in 2. The cardiac output ranged between 3.0 and 7.0 L/min (mean, 4.2 L/min). There was n

Between May, 1975, and May, 1979, 184 glutaraldehyde-preserved valve bioprostheses were implanted (92 aortic, 76 mitral, and 16 multiple). In the last two years, anticoagulants have been administered for the first three post-operative months. Of 176 patients, 10 patients had simultaneous coronary surgery, and five had previous valve surgery; endocarditis had occurred in eight patients before operation, and heart block existed in four patients. In the group of patients who underwent aortic valve replacements, there were four early deaths, all in patients with associated severe coronary artery d

Transcatheter heart valve interventions have transformed the outcomes of patients with valvular heart disease (VHD) who are unfavourable candidates for surgery. Technological advances have allowed extension of these interventions to younger or lower risk patients and those with other forms of VHD and may in the future permit earlier treatment of VHD in less symptomatic patients or those with moderate disease. The balance of risks and benefits is likely to differ between lower and higher risk patients, and more evidence is needed to evaluate the net benefit of transcatheter technology in these

BACKGROUND: Transcatheter mitral valve replacement (TMVR) may mature to become a therapeutic option for high-risk patients with severe mitral regurgitation (MR), particularly in patients at high or prohibitive surgical risk. MR patients with preexisting aortic valve prosthesis have been excluded from most TMVR trials because of the potential risks of left ventricular outflow tract obstruction or interaction between the TMVR anchoring mechanism and the aortic prosthesis. We describe the procedural and short-term outcomes of transapical TMVR with the Tiara valve in patients experiencing severe s

A case of acute bacterial endocarditis with aortic valve abscess, aortic insufficiency, and congestive heart failure at 32 weeks' gestation is described. Prompt valve replacement is indicated due to the risks of embolism to the coronary arteries and brain, and to the high mortality of such patients with medical management only. The infant was delivered prematurely to avoid the intraoperative risks to the fetus of cardiac surgery. General rather than regional anesthesia was chosen because venous pooling from a regional block would necessitate preoperative fluid loading and vasopressor therapy,

Transcatheter mitral valve replacement is increasingly being used as a treatment for high-risk patients who have native mitral valve disease; however, no comprehensive studies on its effectiveness have been reported. We therefore searched the literature for reports on patients with native mitral valve disease who underwent transcatheter access treatment. We found 40 reports, published from September 2013 through April 2017, that described the cases of 66 patients (mean age, 71 ± 12 yr; 30 women; 30 patients with mitral stenosis, 34 with mitral regurgitation, and 2 mixed) who underwent transcat

We describe the case of a 38-year-old man with a history of metastatic testicular cancer who had undergone multiple thoracic surgical procedures, including tricuspid valve replacement with a bioprosthetic valve as a result of tricuspid involvement of his malignancy. He presented at our outpatient cardiology clinic with worsening fatigue, shortness of breath, and peripheral edema, investigation of which revealed severe tricuspid bioprosthesis stenosis with central regurgitation. Because of the patient's medical history, he was considered to be a high-risk surgical candidate. Therefore, transcat

OBJECTIVES: This study sought to evaluate the feasibility of minimally invasive transapical repeat valve-in-a-valve (VinV) implantation.
BACKGROUND: Reoperative heart valve replacement for degenerated xenografts is associated with an increased surgical risk.
METHODS: Conventional Carpentier Edwards porcine aortic (n = 5) and mitral (n = 2) valve prostheses were implanted in 7 pigs. Transapical VinV implantation of a pericardial xenograft fixed within a 23-mm stainless steel, balloon expandable stent (Cribier Edwards, Edwards Lifesciences, Irvine, California) was then performed under fluorosc
